Anti-Hla Antibodies Complicating Infectious Mononucleosis With Thrombocytopenia And Neutropenia, Warren L. Kupin, Maria Sawdyk

Henry Ford Hospital Medical Journal

A case concerning a 21-year-old male college student with thrombocytopenia and neutropenia complicating infectious mononucleosis is presented. Although the patient had no prior history of alloimmunization, a broad spectrum anti-HLA antibody was strongly positive in his serum. Concomitant with resolution of the hematologic abnormalities, the titer of the antibody diminished. This case is unique both in the severity of the thrombocytopenia and neutropenia and for the circulating HLA antibody most likely of viral origin.

This edition of SEEDS focuses on a project developed to integrate low-income women into Jamaica's construction industry. In two years, 34 women passed through the project's basic training and skills upgrading courses. More than 90 percent of these women became employed, the majority as masons and carpenters. The story of how this field was identified as a potential source of income for women, and how the project developed and evolved in response to changing circumstances, presents many useful lessons. These should be of particular interest to those seeking to identify employment areas where women's participation is feasible and in helping …

Decision making for catastrophically ill newborns has recently emerged as a social issue. Advances in biomedical technology and practice, and changes in other economic, social and political factors have led to controversy about norms to guide treatment choice. While much has been written on how such decisions should be made, there has been little social science research on how such decisions are actually made.

The purpose of this dissertation is to elucidate the way that clinicians think about treatment decisions for catastrophically ill newborns. The focus is on decision making with respect to the limitation of treatment in those situations …

The risk for professional burnout has become commonly recognized and in the past several years has been a focus of research efforts. Nursing is one of the professions generating a high level of stress which results in burnout.

The purpose of this study was to test the effects of an intervention program for nurses who felt they were experiencing burnout. Nursing burnout was defined as a chronic condition of stress among nurses, which leads to a syndrome of emotional exhaustion, depersonalization, and lack of personal accomplishment. This study, conducted by a Health Maintenance Organization in Southern California, evaluated the impact of a stress management program in reducing stress/anxiety, stressors/hassles, and perception of stressors. Impact of the program in reducing negative coping mechanisms and increasing positive coping mechanisms as well as the retention of stress management training over a three month follow-up period was examined.

A quasi-experimental -- non-equivalent control group design was used. The experimental group consisted of 54 clients who registered for stress management classes. The control group consisted of 105 matched subjects selected from a larger group of 212. All subjects were tested prior …

The evolution of artificial intelligence since the 1950s is discussed, especially as it is being applied in radiology to image processing. Developments in artificial intelligence are now being used to provide a new approach to image processing. Initially, the computer dealt with numeric representations using languages such as FORTRAN and BASIC. Now symbolic languages such as LISP and PROLOG have expanded the use of the computer into nonnumeric symbolic reasoning that is just being applied to image understanding. This paper explains the new languages and their application to image understanding.

Nonadherence to therapeutic regimens which necessitate major lifestyle change is recognized as one of the most serious problems in medical practice today. Attempts to prevent and control chronic disease, including the development and progression of cardiovascular disease, are only effective when recommendations to the patient are carried out. Few studies have systematically studied the relationship of psychosocial and behavioral factors to adherence outside the theoretical framework of a specific behavioral model; no study has done so with a carefully measured diet as the outcome variable in a controlled prospective clinical trial with patients suffering from cardiovascular disease.

Studies have confirmed that many hemodialysis patients experience numerous psychological difficulties as a result of their physical illness. One behavioral manifestation of these psychological states is noncompliance to imposed dietary restrictions. In spite of life threatening implications, many hemodialysis patients find it very difficult to maintain dietary compliance.

Recently, guided imagery and progressive relaxation have been used as methods to assist clients to overcome negative psychological states. In most cases, the treatments are adjunctive to on-going counseling or psychotherapy. The purpose of this study was to determine predictors of smoking behavior for the senior student nurses in the state of Alabama using Ajzen and Fishbein's Theory of Reasoned Action. Smoking prevalence by level of educational preparation and selected health behaviors were examined for relationship to cigarette use.

A sample of 11 schools of nursing were selected from Alabama's 34, using a random numbers table. There were 13 total programs as two of the schools had both A.D. and B.S. nursing programs. Thirty-two percent (N=555) of senior A.D., B.S. & diploma students nurses in Alabama responded to an 87 item …

Given knowledge of a test's sensitivity and specificity, physicians may use Bayes' theorem to appropriately modify their initial assessment of the likelihood of disease subsequent to obtaining a positive or negative test result. A graphical representation of Bayes' theorem was constructed in order to provide a simple tool to aid in the selection and interpretation of diagnostic tests.

In major prospective studies it has been reported that high heart rate at rest predicts the development of coronary heart disease (CHD) or cardiovascular disease (CVD) in men, but the mechanisms producing these relationships are unknown. Since lipoprotein levels contribute strongly to the risk of CHD and CVD, we examined the relationship of resting heart rate to plasma concentrations of high-density (HDL), low-density (LDL), and very low-density (VLDL) lipoproteins, apolipoprotein (apo) A-I and A-II, and serum concentrations of lipoprotein subfractions in 81 men to determine if atherogenic lipoproteins could potentially induce the reported association of heart rate with development of …

Quantitative assessments of the relative effects on health of various aspects of water supply are virtually absent from the literature. Despite the lack of information, resources are being allocated throughout the developing world, for projects related to water and sanitation. The present study was designed specifically to overcome many of the methodological problems that other researchers have faced. Data were collected concerning the nutritional status of 627 children in three urban communities in South India. Information was also collected on water quality, water quantity, household sanitation, socioeconomic conditions, and housing. Weight-for-height and height-for-age data were compared for preschool-age Haitian children enrolled in a community health and nutrition intervention programme and children measured in the Haiti national nutrition survey of 1978. Cross-sections of the longitudinal data of the intervention programme corresponding to the season when the national survey was conducted (May to September) were chosen for the three years of available programme data (1969, 1970, 1971). Significantly less stunting was found in children in the 1970 and 1971 intervention group than in the children covered by the national survey. Background: There is emerging interest in obesity and its prevalence, outcomes, and management in people with multiple sclerosis (MS). Body mass index (BMI) is the traditional marker of obesity in MS, whereas body composition, inclusive of specific body tissue compartments (e.g., fat, bone, and muscle), is often overlooked despite its relevance.

Objective: This narrative review (a) underscored the use and utility of dual-energy X-ray absorptiometry (DEXA) as an accurate and reliable measure of body composition; (b) thematically analyzed and synthesized the current evidence regarding body composition (using DEXA); and (c) determined gaps to be addressed in future research.

Using a mainframe computer connected to the Henry Ford Hospital computer network, we developed a database for gastrointestinal disease which includes data from radiologic and endoscopic gastrointestinal examinations, along with corresponding pathologic diagnosis. Because of the large volume of procedures in our practice, we developed several unique features for our system. The user enters data by responding to a series of question-and-answer sets constructed by the clinical staff, who do not have experience in computer programming. Data is stored in a hierarchical format using natural language. Boolean logic is used to retrieve data so that different procedures can be correlated …

Obtaining appropriate information in a timely fashion in medical practice has always been a burden for the practitioner. In a large hospital which undertakes major computer projects, this burden is intensified because information is not always available in a form directly usable by the physician or support personnel; it is now accumulated on diverse magnetic media where it is moved and processed as electronic pulses. This paper describes a solution which freely allows continued automation at different rates throughout a large hospital while expediting the movement of information where it is needed in a form understandable by the recipient.

The Vascular Registry of Henry Ford Hospital was initiated by one of the authors in 1957 to provide a consistent and accurate method of documenting and correlating the clinical results of the treatment of occlusive and aneurysmal arterial diseases. Data collection for each patient includes demographic information, treatment of clinical problems, and followup observations. Initially, the system consisted of keypunch cards and eventually magnetic tape processing. By 1980, it was necessary to further update the system because of the increase in patient volume, the need to minimize manual manipulations, and the demand for research data. To assess risk of chronic respiratory disease symptoms due to long-term exposure to the ambient levels of TSP, oxidants, SO2 and NO2 in photochemical air pollution, symptoms were ascertained using NHLBI questions on 8,572 Southern California Seventh-day Adventist, nonsmokers, 25 years and older, who lived eleven years or longer in their 1977 residential area. Tobacco smoke, active and passive, and occupational exposures were measured by questionnaires, as well as lifestyle characteristics relative to pollution exposure such as time spent outside and residence history. A pulmonary function feasibility study was done on a subsample of 86 women 50-64 years of age …
