The Aids Epidemic: A Prism Distorting Social And Legal Principles, Alec Gray
The Aids Epidemic: A Prism Distorting Social And Legal Principles, Alec Gray
New England Journal of Public Policy
The AIDS epidemic is affecting American society in far-reaching and unexpected ways. It touches our institutions, our value systems, and our private lives. Social issues seem to change and become distorted by the epidemic 's prismlike effect. This article examines some of the major public health issues raised by the epidemic, ranging from testing to contact tracing and quarantine. It argues that while the civil rights of individuals may have to be sacrificed to stem the spread of the disease, those rights should not be abandoned unless a clear benefit to the public health would result.

The Clinical Spectrum Of Hiv Infections: Implications For Public Policy, Kenneth H. Mayer
The Clinical Spectrum Of Hiv Infections: Implications For Public Policy, Kenneth H. Mayer
New England Journal of Public Policy
The term acquired immunodeficiency syndrome (AIDS) is a definition developed by the Centers for Disease Control to explain the epidemic of immunosuppression first seen in the United States among gay and bisexual men and intravenous drug users in the early 1980s. It is now known that the human immunodeficiency virus (HIV) is the necessary agent for the compromise of the immune system which results in AIDS; however, there is a wide range of manifestations associated with HIV infection. Individuals with AIDS tend to have severe opportunistic infections or malignancies, and the vast majority ofindividuals die within two years after the …

Aids: Prophecy And Present Reality, Victor De Gruttola, William Ira Bennett
New England Journal of Public Policy
Mathematical modeling of the AIDS epidemic can be useful for policymakers even though precise projections are not possible at this time. Models are useful in establishing ranges for current and future prevalence of HIV infection and incidence of AIDS, as well as in predicting the effect of a given intervention strategy. Most decision makers are using models implicitly when they use epidemiological information as a basis for policy; formulating a model explicitly permits examination of the underlying assumptions. By creating and testing a variety of models, an investigator can determine whether the models reflect more the underlying assumptions or the …

Hiv Antibody Screening: An Ethical Framework For Evaluating Proposed Programs, Ronald Bayer, Carol Levine, Susan M. Wolf
New England Journal of Public Policy
The acquired immunodeficiency syndrome (AIDS) poses a compelling ethical challenge to medicine, science, public health, the legal system, and our political democracy. This report focuses on one aspect of that challenge: the use of blood tests to identify individuals who have been infected with the retrovirus human immunodeficiency virus (HIV). In this article, we follow the terminology recently proposed by the International Committee on the Taxonomy of Viruses; that is, we use the term human immunodeficiency virus. This replaces the more cumbersome dual terminology of human T-cell lymphotropic virus type III/lymphadenopathy-associated virus (HTLV-III/LAV).
The issue is urgent: the tests are …
Human Immunodeficiency Virus In Intravenous Drug Users: Epidemiology, Issues, And Controversies, Donald E. Craven
Human Immunodeficiency Virus In Intravenous Drug Users: Epidemiology, Issues, And Controversies, Donald E. Craven
New England Journal of Public Policy
Intravenous drug users are the second most common risk group for acquired immunodeficiency syndrome (AIDS) in the United States, and they account for approximately 25 percent of the cases. Drug users may spread human immunodeficiency virus (HIV) by sharing contaminated drug injection paraphernalia and through sexual contact; women who use drugs can transmit the virus to their children. The rapid spread of HIV in this risk group and the fact that intravenous drug users are a source for heterosexual and perinatal transmission underscore the need for immediate intervention. In addition, many drug addicts are poor, have limited career possibilities, and …

Comparison Of Four High-Sensitivity Immunoradiometric Assays For Thyrotropin And Results Of Preliminary Clinical Studies, Malachi J. Mckenna, Earl Goad, Mohini Pimputkar, Carolyn S. Feldkamp
Henry Ford Hospital Medical Journal
New immunoradiometric assays (IRMAs) that detect low concentrations of thyroid-stimulating hormone (TSH) have recently become available for routine diagnostic use. These new assays have the putative advantage over conventional radioimmunoassay in that they can distinguish hyperthyroidism from euthyroidism by the finding of a serum TSH below the normal limit. In the present study we sought to evaluate four of these kits according to analytical performance characteristics and clinical utility. All IRM As could detect TSH at a concentration substantially below the lower limit of normal and thus effectively identify hyperthyroid samples. Although differences in the performance characteristics were found, all …

Neurobehavioral Symptoms In Mild Primary Hyperparathyroidism: Related To Hypercalcemia But Not Improved By Parathyroidectomy, Gregory G. Brown, Richard C. Preisman, Michael Kleerekoper
Henry Ford Hospital Medical Journal
The neurobehavioral symptomatology of severe primary hyperparathyroidism (PHPT) has been thoroughly described. Less is known about more mild cases. We evaluated 34 patients with mild PHPT and followed 19 for an average of six months. Of those cases followed, ten had parathyroidectomy (PTX). Patients with no neurobehavioral abnormalities had the lowest serum calcium levels (10.9 ± 0.88 mg/dL), patients with signs of affective disorder had intermediate levels (11.25 ± 0.66 mg/dL), and patients with signs of cerebral dysfunction had the highest levels (12.17 ± 1.17 mg/dL). Serum calcium significantly correlated with motor speed, psychomotor speed, fluid intelligence, and short term …

Prolonged Remission Of Cushing's Disease Following Bromocriptine Therapy, Malachi J. Mckenna, Marc Linares, Raymond C. Mellinger
Henry Ford Hospital Medical Journal
A 33-year-old woman developed hypercorticism of fulminant onset following delivery of a full-term, normal child. An ectopic hormone-producing neoplasm was excluded by extensive studies. Pituitary dependent hypercorticism of intermediate lobe origin was suggested on the basis of onset following pregnancy, failure of Cortisol suppression by high-dose dexamethasone, hyperresponsiveness of prolactin to thyrotropin-releasing hormone stimulation, and reduction in adrenocorticotropin titers following oral administration of bromocriptine. Initial remission of disease achieved with bromocriptine was followed by recurrence on discontinuation of the agent. However, complete remission which occurred following a prolonged course of bromocriptine has persisted for a total of 22 months.

Primary Aldosteronism At Henry Ford Hospital In The 1980s, Max Wisgerhof
Primary Aldosteronism At Henry Ford Hospital In The 1980s, Max Wisgerhof
Henry Ford Hospital Medical Journal
This report describes the experience with the diagnosis and treatment of primary aldosteronism at Henry Ford Hospital since 1980. Of the 28 patients who received the diagnosis. 13 had unilateral primary aldosteronism and 15 had idiopathic hyperaldosteronism. Individual cases are used to demonstrate clinical points. The clue to the presence of primary aldosteronism in a hypertensive patient is hypokalemia. The diagnosis is established by showing 1) high plasma aldosterone after intravenous saline or high urinary aldosterone after treatment with sodium chloride orally, and 2) low stimulated plasma renin activity. Treatment with potassium supplement should be given during the testing. Unilateral …

A 35-year-old woman, who had a renal transplant flve years ago, developed malignant lymphoma of the mediastinum. The lymphoma was of lymphoblastic type and had T-cell immunophenotype. Most transplant-related lymphomas are of B-cell type. T-cell lymphoma in this setting is extremely rare, and the mechanism of development may be different from that of B-cell lymphomas.

Henry Ford Hospital Medical Journal
Prospective annual screening for hereditary medullary thyroid carcinoma (MTC) in the J-kindred, currently a 117-member family with multiple endocrine neoplasia type 2A, began in 1969. During the initial screening, 12 patients were found to have MTC. Subsequent screening has detected C-cell abnormalities (C-cell hyperplasia or microscopic MTC) in 22 of 23 addilional family members thyroidectomized for abnormal calcium- or pentagastrin-provocative calcitonin (CT) test results. Seven of the initial 12 patients thyroidectomized in 1970 to 1971 and 19 of 23 individuals thyroidectomized since 1971 remain disease-free by all criteria; three patients thyroidectomized since 1971 have had clearly abnormal serum CT measurements …

This report presents pairwise linkage results from our search for the locus of the gene (MEN2A) for the multiple endocrine neoplasia type 2A (MEN-2A) syndrome in one large kindred (the N kindred), clearly segregating for an autosomal dominant form. About 25% of the autosomal genome is excluded when these new results are combined with those we have published previously. The genetic markers employed are distributed across at least 19 of the 22 autosomes. Seven genetic markers whose chromosomal locations are not yet established have also been studied.

Different Mechanisms Of Calcitonin, Calcitonin Gene-Related Peptide, And Somatostatin Regulation By Glucocorticoids In A Cell Culture Of Human Medullary Thyroid Carcinoma, Gilbert J. Cote, Robert F. Gagel
Henry Ford Hospital Medical Journal
We have employed the TT cell line, a model for the human medullary thyroid carcinoma cell, lo study the regulation of peptide hormone production by glucocorticoids. Complementary DNA probes were used to measure the calcitonin (CT), CT gene-related peptide (CGRP), and somatostatin (SRIF) mRNA levels. Dose-response experiments in serum-free medium showed that dexamethasone (six-day treatment) lowered somatostatin (to 1% of basal) and CGRP mRNA (to 50% of basal) and stimulated CT mRNA (threefold to thirteenfold) with a half-maximal effective concentration of 10−8 M. Time course studies for cells continuously exposed to 10−6 M dexamethasone showed a rapid (within …

Calcitonin Gene Peptides: The Diagnostic Value Of Measurement In Medullary Thyroid Carcinoma, Samia I. Girgis, Catherine Lynch, Carmel J. Hillyard, John C. Stevenson, Patricia A. Hill, David W. R. Macdonald, Iain Macintyre
Henry Ford Hospital Medical Journal
The calcitonin gene encodes a family of peptides, at least three of which normally circulate in man: calcitonin (CT), a calcium-lowering hormone; katacalcin (KC), a peptide of unknown function; and calcitonin gene-related peptide (CGRP), a neuropeptide and potent vasodilator. In a study of 45 patients with medullary thyroid carcinoma (MTC), plasma CGRP was elevated in approximately 50% of cases. Furthermore, CGRP levels did not correlate with CT levels. However, plasma KC was elevated in all cases, with a good correlation with CT levels, as has been noted previously. Measurement of CT or KC appears to be superior to measurement of …

Where Is The Locus For Multiple Endocrine Neoplasia Type 2a?, Nancy E. Simpson, Kenneth K. Kidd
Henry Ford Hospital Medical Journal
Attempts to map the locus of the gene (MEN2A) for the multiple endocrine neoplasia type 2A (MEN-2A) syndrome over the past II years are reviewed. Lod scores from family linkage studies of polymorphisms for 26 proteins and 64 DNA markers (restriction fragment length polymorphisms or RFLPs) were pooled from the published literature and the authors' unpublished data. The pooled data excluded 32% of the genome as possible sites for the MEN2A locus. This represents a considerable increase in the area of exclusion over that of the 6% reported at the first workshop. Linkage studies of RFLPs recognized by DNA sequences …

Forest Conservation In Nepal: Encouraging Women's Participation, Augusta Molnar
Poverty, Gender, and Youth
This issue of SEEDS focuses on ways in which women have been involved in a government forest conservation and restoration program in Nepal. As is common with many large-scale projects with a general impact, women were not a direct focus of the project's original design. As activities got underway, however, both the Nepali staff and their expatriate colleagues quickly realized that the direct involvement of women was crucial to the success of the project's participatory strategy. Over the initial five years, 1980 to 1985, a number of approaches to addressing women's needs and generating their active participation were tried. The …
