Symptom Burden At End Of Life In Patients With Terminal And Life-Threatening Illness In Intensive Care Units, Peggy G. Kalowes Phd, Msn, Rn, Cns

The purpose of this study was to examine the symptom burden and severity of symptom distress reported by ICU patients at high-risk for death and to compare patient-rated symptoms and distress for concordance with symptoms reported by a family member. A prospective, correlational design with two data-points was used to study a convenience sample of 80 patients and 53 family members. The Condensed Memorial Symptom Assessment Scale (CMSAS) was used to gain patient/proxy report of symptoms on the first and third day after admission to the ICU. The majority of patients were symptomatic (98%), reporting an average of 10.23 symptoms. …

The construct of resilience has gained considerable attention over the last four decades since researchers observed that children and youth could cope and adapt in spite of adversity. Resilience involves a dynamic process involving an interaction between both risk and protective processes, internal and external to the individual, that can modify the effects of an adverse life event. Diabetes patients who understand their disease and how to manage it can achieve a lower complication rate. The American Diabetes Association recommends that patients receive self-management education and that patients' skills and knowledge be reassessed annually. The Healthy People 2010 objectives include a goal of at least 60% of diabetes patients receiving diabetes education. However, to date overall, far fewer (20%–50%) obtain comprehensive diabetes education and skills development to effectively implement self-management techniques. Self-esteem and body image disturbances prominently figure into many physical and psychological health disorders such as depression, eating disorders, anxiety disorders, and chemical dependency. In Euro-American culture, media images of femininity and physical attractiveness reinforce generally held perceptions of the idealized female beauty as tall, white, slender, and often blond and blue eyed. The physical morphology of African-American women does not genetically "fit" this westernized standard of beauty with implications for their mental health. The socialization of African-American women in a culture that embraces a different ethnic standard of beauty influences their perceptions of how physically attractive they see themselves. …

Purpose: This study explores ICU patient's family member needs, particularly Vietnamese and Latino families. Design: Convenience sampling at 24 bed ICU in acute care community hospital serving ethnically diverse population. Methods: Non-experimental survey with pretest-posttest design using Demographic sheet, Critical Family Needs Inventory (CCFNI), and Needs Met Inventory (NMI). Information pamphlets were distributed. Data analysis was by ethnic groups using measures of central tendency and descriptive statistics. Findings: CCFNI results indicate family members of all ethnicities experience the same priority of needs; support and information are top two needs. English, Spanish and Vietnamese pamphlets met information needs of the majority …

Recently, there has been a great deal of attention to the fact that there are few learning opportunities regarding end-of-life (EOL) care in nursing undergraduate education. One purpose of this research study was to provide an educational seminar for nursing students with information necessary to communicate with patients at the end of life and their families. The educational intervention consisted of an interdisciplinary panel discussion, a brief segment of a video, a case study with small group discussion, and an optional reflective activity. Adolescent depression and teenage suicide should not be neglected and underestimated. While mental health is one of the ten health indicators that Healthy People 20 I 0 is focused on, and there are goals for the health community to work on, statistics are showing that teenagers in a county in Northern California were at high risk for mental health problem. This study was an introductory screening of the depression level of 7th graders attending middle school, and explored sources of adolescent perceived stress and routes of assistance. The purpose of this research is to examine the functional status (ADLs and IADLs) in patients with a primary/secondary diagnosis of cardiac disease in the home health setting. The independent variables of dyspnea and service were examined for the effect on discharge functional status while controlling for the effects of age, gender, race, admission functional status, primary and secondary diagnosis. Objective: This study examines individual and contextual correlates of cigarette smoking in a randomly selected, community-based sample of low-income African American women.
Design: The study sample was selected by using a two-stage area probability sample design.
Setting: Participants were recruited from > 12,000 housing units selected from 39 census tracts in the city of Detroit.
Participants: Participants for this study include a total of 921 women who completed the baseline assessment of a randomized clinical trial aimed at improving the oral health of African American families.

Descriptions of parenting in a homeless shelter were obtained from nine single mothers housed in one of two shelters in a major southwestern border metroplex. Case study methodology was used to obtain their perspectives on parenting in this unique context. Content analysis was used to organize and analyze emerging themes. Major themes that emerged from the study were (a) a lack of autonomy, (b) gaining access to material resources, (c) lack of privacy, (d) constant change and insecurity in the environment, (e) differing values, and (e) psychosocial support.

Purpose: To explore the perspectives of teenage girls on how life contexts influenced sexuality and sexual abstinence.
Study Design and Methods: A qualitative descriptive study with a convenience sample of 14 sexually abstinent African American adolescent females who were interviewed to obtain their life histories. Narrative analysis was used to identify unique and common experiences and to develop themes.
Results: For these participants, being abstinent was a way to demonstrate their emerging identities as adult women. They described themselves as faithful, unique persons who defied negative stereotypes, avoided risky situations, wanted to be strong women like their …

Objective
We conducted a systematic review to examine the effectiveness of educational interventions in increasing mammography screening among low-income women.
Data Sources
Bibliographic databases, including MEDLINE, The Cochrane Central Register of Controlled Trials, The Cochrane Database of Systematic Reviews, and the ISI Web of Science, were searched for relevant articles.
Study Inclusion and Exclusion Criteria
Randomized, community-based trials targeting low-income women and published between January 1980 and March 2003 were included.
Data Extraction
The search yielded 242 studies; 24 met all inclusion criteria.

Background
This study examines the current prevalence of cigarette smoking and the number of cigarettes smoked in a community-based sample of 1021 low-income African-American men and women.
Methods
Participants were selected using a two-stage, area probability sample design. Data were collected in 2002–2003 in face-to-face interviews and analyzed in 2005. All data and analyses were weighted to account for the complex sampling design.
Results
Fifty-nine percent of men and 41% of women were current smokers, with younger individuals apparently initiating smoking at an earlier age than older individuals.
Conclusions

The purpose of this column is to describe the implementation of an international multisite Roy adaptation model-based study of women’s perceptions of and responses to cesarean birth. The need for the study arose from the concern that women’s childbearing needs may not be met to their full satisfaction, especially if the infant is born by cesarean. Serendipity and networking played a part in the selection of four study sites in the United States (Boston, Milwaukee, Norfolk, Oklahoma City) and two in other countries (Finland, Australia). Background: As the population ages, many quality of life issues will generate discussion. One such issue is sexuality of the aging adult. While there is substantial documentation related to the physiology and desires of the aging adult, little is known about medical practitioners' beliefs and practices regarding sexuality. Method: A 14 question survey with a comment section was developed and mailed to 269 physicians and nurse practitioners in Santa Clara County, California. The responses were evaluated using descriptive statistics. The comments received were grouped related to intent or topic. Abstract
Background
Many caregivers with chronically ill relatives suffer from depression. However, the relationship of depression to other outcomes of chronic caregiving remains unclear. This study tested a hypothesized model which proposed that hours of care, stressful life events, social support, age and gender would predict caregivers' outcomes through perceived caregiver stress. Depression was expected to mediate the relationship between perceived stress and outcomes of chronic caregiving (physical function, self-esteem, and marital satisfaction).
Methods

Childhood Experiences Of Appalachian Women Who Have Experienced Intimate Partner Violence During Adulthood., Amy L. Reeves

Recent national surveys in the United States estimate one in five females will experience abuse by an intimate partner during her lifetime. Previous quantitative research linked childhood victimization to repeated victimization in adult relationships. This qualitative study explored the lived experiences of childhood in eight female victims of intimate partner violence who were born and reared in southern Appalachia. Interviews were analyzed using a descriptive-interpretative phenomenological method, as described by Van Manen. The three essential themes from childhood were identified as: living 'as if' an orphan; surviving in chaos; and, manifesting a devalued self. These themes were congruent with findings …

This article is a commentary on the life history approach, which specifically described aspects of and offered suggestions for conducting life history research with adolescents. The life history approach is well suited to a wide variety of types of health-related inquiry. In this article, the author used an exemplar study to show how well the life history approach was suited for questions regarding adolescent issues. This approach was also a good match for the developmental skills and needs of adolescents. Our hospital led a multidisciplinary community team to improve the quality of care delivered to HIV-infected clients utilizing a disease management approach in a US metropolitan community of 150,000 people. Community needs assessment and client and community surveys were used to define the problems. Patient care flowcharting and the creation of an electronic patient database facilitated patient tracking across the entire community. Clinical guidelines and a consultation and referral immunology clinic standardized care practices. Measurable improvements in the quality of care were noted in multiple areas. Flowchart completion rates rose from 44% to 100%; medication adherence assessment rose from 82% …

The demography of the United States is rapidly changing. The impressive growth rate of Latino populations within the United States has great impact on health care. This article includes discussion of a significant increase from 1988 to 2000 in Latino utilization of emergency department and inpatient services at a pediatric medical center in Salt Lake City, Utah, information on health care barriers for Latinos, and suggestions for providing culturally competent care.

Technological changes, interpersonal relationships, and scarce resources have created a complex environment for health professionals. Nurses, as the largest group of health care providers, are confronted with moral issues when dealing with managed care policies, end-of-life care and workplace or institutional issues. Moral issues are expressed as moral distress, which has been linked to decreased satisfaction, termination of employment and leaving the profession of nursing altogether.
