Dental students are significantly confronted with mental health problems. We conducted a prospective recruitment of students at the Faculty of Dentistry of the Mohammed VI University of Sciences and Health to investigate the determinants of stress specific to this population in order to find solutions to improve their quality of life. In our study, 410 students responded to the survey, the average age of the students was 21.51 ± 2.102 years, 70% were female, 30% were in their 6th year, and 60% of the students were from Casablanca. Background: Delirium is a prevalent acute brain dysfunction in patients within intensive care units. Delirium is marked by a fluctuating mental state, inattention, and altered consciousness. Inpatient hospitalization provides an opportunity to provide evidence-based diabetes education to patients. Introduction: Peri-postmenopausal women with the chronic condition polycystic ovary syndrome (PCOS) remain at cardiometabolic risk and/or subsist with established comorbidity while continuing to manage persistent PCOS signs and symptoms, such as hirsutism. Thus, PCOS transcends the reproductive years, yet there is sparse scientific literature on the peri-postmenopausal years of women with PCOS.

Background: Geographic, demographic, and socioeconomic differences in health outcomes persist despite the global focus on these issues by health organizations. Barriers to accessing care contribute significantly to these health disparities. Prevalence And Determinants Of Diarrhoea And Acute Respiratory Infections Among Children Aged Under Five Years In West Africa: Evidence From Demographic And Health Surveys, Derrick Nyantakyi Owusu, Henry Ofori Duah, Duah Dwomoh, Yakubu Alhassan

Background Diarrhoea and pneumonia are the leading causes of morbidity and mortality in children aged < 5 y (under five) globally. This study sought to investigate the prevalence and determinants of diarrhoea and acute respiratory infections (ARIs) among children under five in West Africa. Methods The most recent demographic and health survey (DHS) standard for 13 West African countries was used in the study. We calculated the prevalence of diarrhoea and ARIs (2 wk prior to the survey) and performed multivariable complex logistic regression analysis to identify possible predictors of diarrhoea and ARIs. Results The weighted prevalence of diarrhoea and ARI was 13.7% and 15.9%, respectively. The prevalence of comorbid diarrhoea and ARI was 4.4%. Children aged < 2 y (p< 0.001), mothers aged < 30 y (p< 0.003), mothers without formal education (p< 0.001), poor households (p< 0.001) and poor nutritional status, wasting (p=0.005) and underweight (p< 0.001), were the independent predictors of diarrhoea. The independent predictors of ARIs were children with no childhood vaccinations (p=0.002), use of solid fuel in the household (p=0.007), being underweight (p=0.05) and diarrhoea (p< 0.001). Conclusions The findings imply the need for holistic public health interventions such as increased vaccination coverage, population-based nutritional programmes and campaigns on the use of cleaner cooking fuel targeted at high-risk subgroups in the population to reduce the burden and adverse effects of diarrhoea and ARIs in the West African region.

ภาวะเซพซิส คือ ภาวะคุกคามต่อชีวิต ต้องได้รับการดูแลในหอผู้ป่วยวิฤต โดยเฉพาะอย่างยิ่งในผู้สูงอายุที่มีแนวโน้มเกิดภาวะแทรกซ้อนร้ายแรง ใช้เวลาในการรักษาตัวนาน และมีอัตราการเสียชีวิตสูงเมื่อเทียบกับผู้ป่วยที่มีอายุน้อยกว่า ประกอบกับปัจจัยทางด้านกระบวนการสูงอายุ ได้แก่ ระบบภูมิคุ้มกันลดลงและระบบร่างกายมีการเสื่อมถอย และลักษณะสำคัญที่พบได้ในผู้สูงอายุคือ อาการและอาการแสดงไม่เฉพาะเจาะจง เช่น ไม่มีไข้ ภาวะสับสนเฉียบพลัน รับประทานอาหารได้ลดลง และอ่อนเพลีย เป็นต้น ส่งผลให้เกิดปัญหาสุขภาพที่ซับซ้อนและรุนแรง การคัดกรองเพื่อให้ผู้สูงอายุได้รับการรักษาอย่างถูกต้องและรวดเร็วจะสามารถช่วยลดภาวะอวัยวะในร่างกายล้มเหลวหลายระบบและอัตราการเสียชีวิต ดังนั้นพยาบาลวิกฤติต้องมีความรู้ ความเข้าใจ และความสามารถในการดูแลผู้สูงอายุที่มีภาวะเซพซิส ซึ่งต้องได้รับการดูแลจากพยาบาลที่มีความเชี่ยวชาญ เพื่อให้ได้รับการคัดกรองอย่างรวดเร็ว วินิจฉัยที่ถูกต้อง และได้รับการรักษาที่มีมาตรฐาน อีกทั้งทำให้ผู้ป่วยปลอดภัยและไม่เกิดภาวะแทรกซ้อนรุนแรง

Background: The trauma intensive care unit (ICU) at University of Kentucky (UK) Healthcare uses a ventilator separation protocol which provides specific guidelines on the successful weaning of patients from mechanical ventilation. However, many of the nurses are not aware of this protocol as formal education is not included in their orientation or training. This lack of knowledge can lead to risks such as increased patient morbidity, mortality, ICU length of stay, and healthcare-related costs. Abstract

Background: Minimally invasive esophagectomy surgery is commonly performed by thoracic surgeons on patients with esophageal cancer. The post-operative care of these patients is meticulous and follows a protocol developed by the thoracic surgeons at the University of Kentucky. At UK Chandler Medical Center, concerns have been raised regarding nurses’ lack of education of the protocol, and therefore decreased implementation. Increasing nursing knowledge pertaining to this protocol by performing educational in-services periodically and standardizing and implementing formal education as part of nursing orientation will decrease the knowledge gap and improve patient outcomes and increase utilization of the post-operative protocol.

Background: In the United States, it is estimated that there are more than 2 million cases of Hepatitis C (HCV), with over 66,000 new cases since 2020 However, an estimated 75% of those with HCV go undiagnosed and less than 35% receive treatment within a year of diagnosis. In 2018, UK Healthcare (UKHC) implemented a universal HCV screening method using a Best Practice Advisory (BPA) for anyone 18 and older who presented to the Emergency Department and had labs ordered. While this is the gold standard, there are barriers to implementation in primary care. OBJECTIVES AND METHODS: We analyzed upper endoscopic and histological findings in 3 cohorts of children undergoing upper gastrointestinal endoscopy over a 10-year period. Five hundred seventy-nine patients were identified, with 244 (42%), 199 (35%), and 136 (23%) in the 2011, 2015, and 2019 cohorts, respectively. The most common symptoms and signs were abdominal pain, vomiting, failure to thrive, and diarrhea.

RESULTS: The number of patients who had histological evidence of chronic gastritis increased from 2011 (n = 70, 29%) to 2015 (n = 106, 53%) and 2019 (n = 92, 68%;

การวิจัยแบบกึ่งทดลองนี้มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการพัฒนาความรอบรู้ด้านสุขภาพผ่านแอปพลิเคชันไลน์ต่อระดับน้ำตาลสะสมในเลือดในผู้ป่วยเบาหวานชนิดที่ 2 รายใหม่ กลุ่มตัวอย่างเป็นผู้ป่วยเบาหวานชนิดที่ 2 รายใหม่ อายุ 45-59 ปี คัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจงตามคุณสมบัติที่กำหนด โดยจับคู่ให้มีลักษณะคล้ายคลึงกันในเรื่อง ระดับการศึกษา ระยะเวลาในการป่วยเป็นโรคเบาหวาน และอายุ แบ่งเป็นกลุ่มทดลอง 22 คน กลุ่มควบคุม 22 คน รวม 44 คน โดยกลุ่มควบคุมได้รับการพยาบาลตามปกติ กลุ่มทดลองได้รับโปรแกรมการพัฒนาความรอบรู้ด้านสุขภาพผ่านแอปพลิเคชันไลน์ร่วมกับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการวิจัย ได้แก่ แบบสอบถามข้อมูลส่วนบุคคลของผู้ป่วยเบาหวานชนิดที่ 2 รายใหม่ โปรแกรมการพัฒนาความรอบรู้ด้านสุขภาพผ่านแอพพลิเคชั่นไลน์ เครื่องมือกำกับการทดลอง ได้แก่ แบบวัดความรอบรู้ด้านสุขภาพสำหรับผู้ป่วยโรคเบาหวาน ผ่านการตรวจสอบจากผู้ทรงคุณวุฒิ 5 คน ได้ค่าดัชนีความตรงเนื้อหาเท่ากับ .98 และมีค่าความเชื่อมันเท่ากับ .87 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและเปรียบเทียบความแตกต่างด้วยสถิติที ผลการวิจัยสรุปดังนี้ 1. ระดับน้ำตาลสะสมในเลือดของผู้ป่วยเบาหวานชนิดที่ 2 รายใหม่ กลุ่มทดลองหลังได้รับโปรแกรมพัฒนาความรอบรู้ด้านสุขภาพผ่านแอปพลิเคชันไลน์ต่ำกว่าก่อนได้รับโปรแกรมพัฒนาความรอบรู้ด้านสุขภาพผ่านแอปพลิเคชันไลน์อย่างมีนัยสำคัญทางสถิติตที่ระดับ .05 2. ระดับน้ำตาลสะสมในเลือดของผู้ป่วยเบาหวานชนิดที่ 2 รายใหม่ ที่ได้รับโปรแกรมพัฒนาความรอบรู้ด้านสุขภาพผ่านแอปพลิเคชันไลน์ต่ำกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนับสำคัญทางสถิติที่ระดับ .05

การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาพฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยของรัฐ ศึกษาความสัมพันธ์ระหว่างการเรียนรู้ตลอดชีวิต ภาวะผู้นำเชิงนวัตกรรม และบรรยากาศการส่งเสริมนวัตกรรมในองค์การกับพฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยของรัฐ และศึกษาตัวแปรพยากรณ์ที่สามารถร่วมทำนายพฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยของรัฐ กลุ่มตัวอย่าง คือ พยาบาลวิชาชีพระดับปฏิบัติการ โรงพยาบาลมหาวิทยาลัยของรัฐ ที่มีประสบการณ์ทำงานตั้งแต่ 1 ปีขึ้นไป จำนวน 210 คน เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามการเรียนรู้ตลอดชีวิต แบบสอบถามภาวะผู้นำเชิงนวัตกรรม แบบสอบถามบรรยากาศการส่งเสริมนวัตกรรมในองค์การ และแบบสอบถามพฤติกรรมการทำงานที่เน้นนวัตกรรม ซึ่งผ่านการตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิ จำนวน 5 ท่าน ได้ค่า CVI เท่ากับ .94, .89, .88 และ .93 ตามลำดับ และทดสอบค่าความเที่ยง โดยคำนวณหาค่าสัมประสิทธิ์แอลฟ่า ได้ค่าความเที่ยง เท่ากับ .90, .97, .95 และ .96 ตามลำดับ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา สถิติสหสัมพันธ์เพียร์สัน และสถิติถดถอยพหุคูณ ผลการวิจัยพบว่า พฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพในโรงพยาบาลมหาวิทยาลัยของรัฐ อยู่ในระดับปานกลาง (M = 3.49, S.D. = 0.76) การเรียนรู้ตลอดชีวิต ภาวะผู้นำเชิงนวัตกรรม และบรรยากาศการส่งเสริมนวัตกรรมในองค์การ มีความสัมพันธ์ทางบวกกับพฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยของรัฐ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r = .683, .524 และ .499 ตามลำดับ) ปัจจัยที่สามารถร่วมทำนายพฤติกรรมการทำงานที่เน้นนวัตกรรมของพยาบาลวิชาชีพ โรงพยาบาลมหาวิทยาลัยของรัฐ คือ การเรียนรู้ตลอดชีวิต บรรยากาศการส่งเสริมนวัตกรรมในองค์การ และภาวะผู้นำเชิงนวัตกรรม โดยมีอำนาจในการทำนายร้อยละ 53.6 (Adjusted R² = .536) สามารถเขียนสมการทำนายในรูปคะแนนมาตรฐาน ดังนี้ ZIWB = .540 ZLLL + .192 ZOIC + .149 ZIL

การวิจัยครั้งนี้เป็นการวิจัยเชิงบรรยาย เพื่อศึกษาปัจจัยคัดสรรที่สัมพันธ์กับความต้องการฟื้นฟูสมรรถภาพผู้ป่วยสูงอายุที่มีภาวะพึ่งพิงที่เข้าสู่การดูแลระยะกลางในโรงพยาบาล กลุ่มตัวอย่าง คือ ผู้ที่มีอายุเกิน 60 ปีบริบูรณ์ขึ้นไปที่พ้นระยะเฉียบพลัน มีสภาวะทางการแพทย์คงที่และมีสัญญาณชีพปกติ ทั้งเพศชายและเพศหญิง ที่มารับบริการในแผนกผู้ป่วยในโรงพยาบาลระดับตติยภูมิ 3 แห่ง โรงพยาบาลเลิดสิน โรงพยาบาลเจริญกรุงประชารักษ์ และโรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย จำนวน 93 คน คัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง เครื่องมือที่ใช้ในการเก็บข้อมูล ได้แก่ แบบสอบถามข้อมูลส่วนบุคคล แบบประเมินความสามารถในกิจวัตรขั้นพื้นฐานของผู้สูงอายุ แบบประเมินภาวะรู้คิด แบบประเมินภาวะเปราะบางอย่างง่าย วิเคราะห์ความเที่ยงด้วยวิธีการวัดซ้ำเท่ากับ 1.0, 0.82, 1.0 ตามลำดับ แบบวัดความเศร้าในผู้สูงอายุไทย แบบคัดกรองภาวะโภชนาการ และแบบประเมินความต้องการฟื้นฟูสมรรถภาพผู้ป่วย วิเคราะห์ความเที่ยงด้วยค่าสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ .94, .73, .89 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา สถิติสหสัมพันธ์อีต้า และสถิติสหสัมพันธ์ของ สเปียร์แมน ผลการศึกษา พบว่า1. ผู้ป่วยสูงอายุที่มีภาวะพึ่งพิงที่เข้าสู่การดูแลระยะกลางในโรงพยาบาลมีความต้องการฟื้นฟูสมรรถภาพในระดับปานกลาง (mean = 55.04, SD = 10.55) 2. ภาวะเปราะบาง มีความสัมพันธ์กับความต้องการฟื้นฟูสมรรถภาพผู้ป่วยสูงอายุที่มีภาวะพึ่งพิงที่เข้าสู่การดูแลระยะกลางในโรงพยาบาลอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (η=.259) 3. ภาวะโภชนาการ ภาวะซึมเศร้า และจำนวนชนิดของยาที่ใช้ประจำ มีความสัมพันธ์กับความต้องการฟื้นฟูสมรรถภาพผู้ป่วยสูงอายุที่มีภาวะพึ่งพิงที่เข้าสู่การดูแลระยะกลางในโรงพยาบาลอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (rs = 313, rs = -247, rs = -264 ตามลำดับ) 4. เพศ ไม่มีความสัมพันธ์กับความต้องการฟื้นฟูสมรรถภาพผู้ป่วยสูงอายุที่มีภาวะพึ่งพิงที่เข้าสู่การดูแลระยะกลางในโรงพยาบาล

การวิจัยกึ่งทดลองนี้มีวัตถุประสงค์เพื่อศึกษาโปรแกรมการเตรียมความพร้อมการดูดกลืนต่อประสิทธิภาพการดูดนมมารดาของทารกเกิดก่อนกำหนดที่มีอายุหลังมีประจำเดือนระหว่าง 32-36+6 สัปดาห์และมารดา จำนวน 34 คู่ แบ่งเป็นกลุ่มที่ได้รับโปรแกรมการเตรียมความพร้อมการดูดกลืน 17 คู่ และกลุ่มที่ได้รับการพยาบาลตามปกติ 17 คู่ เครื่องมือที่ใช้ในการวิจัยประกอบด้วย แบบประเมินความพร้อมในการดูดนมมารดาของทารกเกิดก่อนกำหนด และแบบประเมินประสิทธิภาพการดูดนมมารดาของทารก โดยเครื่องมือทั้งหมดผ่านการตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิจำนวน 5 คน มีค่าความตรงเชิงเนื้อหาเท่ากับ 0.87 และ 0.85 และมีค่าสัมประสิทธิ์แอลฟาครอนบาคเท่ากับ 0.88 และค่าสัมประสิทธิ์ Cohen’s Kappa เท่ากับ 0.84 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติการวิเคราะห์ Repeated Measure ANOVA ผลการวิจัยพบว่า ประสิทธิภาพการดูดนมมารดาของทารกเกิดก่อนกำหนดหลังได้รับโปรแกรมการเตรียมความพร้อมการดูดกลืนวันที่ 1, 4 และ 7 ดีกว่าก่อนได้รับโปรแกรมฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 และประสิทธิภาพการดูดนมมารดาของทารกเกิดก่อนกำหนดหลังได้รับโปรแกรมการเตรียมความพร้อมการดูดกลืนวันที่ 1, 4 และ 7 ดีกว่าทารกเกิดก่อนกำหนดที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ผลการศึกษานี้สามารถนำไปใช้เป็นแนวทางดูแลทารกเกิดก่อนกำหนดในการเตรียมความพร้อมการดูดกลืน เพื่อให้ทารกมีประสิทธิภาพการดูดนมมารดา

Background: Clinicians and researchers often assume that symptom burden is associated with self-care management (SCM) in patients with heart failure (HF). However, that association is often not borne out in simple regression analyses and may be because another variable mediates the association. Fatalism is an appropriate candidate for mediation and is the belief that circumstances are predetermined without opportunity for control by individuals.

Objective: Our objective was to determine whether fatalism mediated the relationship of symptom burden with SCM among adults with HF.

Overweight/obese adults are at an increased risk for both physical and mental comorbidities (CDC, 2019). Depression is a common comorbidity in those that are obese (CDC, 2019) and the standard of care currently to treat depression may increase the risk of physical comorbidities because weight gain is a common side effect of antidepressants (Hall-Flavin, 2018). The aim of this capstone research paper is to present evidence that an aerobic exercise regimen can be a valid treatment option for depression.
