This project reflects on the observations and experiences that occurred during the firestorms in Southern California in October and November 2003. It specifically focused on elders during this disaster as a vulnerable population. Recommendations are made for the role of the nurse in preparation and planning for evacuation of the elderly.

Delirium is one of the most serious and prevalent cognitive disorders among hospitalized elders. Fourteen elders participated in this phenomenologic study describing the "lived experience " of delirium from the patient's perspective. Implications for nursing practice are derived from their reality and insight.

OBJECTIVES: To examine the effect of organizational characteristics on physical restraint use for hospitalized nursing home residents.

DESIGN: Secondary analysis of data obtained between 1994 to 1997 in a prospective phase lag design experiment using an advanced practice nurse (APN) intervention aimed at reducing physical restraint for a group of hospitalized nursing home residents.

SETTING: Eleven medical and surgical units in one 600-bed teaching hospital.

PARTICIPANTS: One hundred seventy-four nursing home residents aged 61 to 100, hospitalized for a total of 1,085 days.

The increasing life span of people within the developed world may produce major health problems for the elder. These health problems lead to premature declines in function and cognition. Many healthcare professionals do not recognize these declines until the need for institutionalized care is urgent. Research has demonstrated that healthcare professionals either fail to recognize or address these declines, contributing to poor outcomes for the elder and the exponential rise in healthcare expenses o f the Medicare and Medicaid funds. Data released by the Department of Health and Human Services in February, 2001, revealed that Nevada has the lowest number of registered nurses per 100,000 population in the nation. The state’s population is growing faster than the nursing programs are currently able to produce new graduates. Current predictions are that the existing nursing shortage will become more severe and have a longer duration than has ever before been experienced. While there may not be one single identifiable causative factor, the aging nursing workforce, low unemployment, and the universal nature of the shortage magnify the problem. Exercise Level And Age As Related To Self-Perceived Physical Health, Mental Health, And Social Health In Older Adults, Barbara Jean Leach Phd, Msn, Rn

The purpose of this study was to examine the relationship between the level of exercise and age and self-perception of physical, mental and social health in older adults. Voluntary quota sampling was used to recruit 224 older adults aged 55 and up. The Physical Activity Questionnaire for the Elderly (Voorrips, Ravelli, Dongelmans, Deurenberg, & Van Staveren, 1990) was used to determine the current level of exercise of the older adult. Age was measured using the Demographic Questionnaire by Leach (1999) and self-perception of physical health and mental health was measured using the SF-36 Health Survey by Ware (1993). The Texas …

Functional Health Literacy In An Urban Primary Care Clinic, Nancy Trygar Artinian, M. Patricia Lange, Thomas Templin, Lynda G Stallwood, Christopher E. Hermann

The purpose of this study was to determine the level of functional health literacy in a sample of patients treated in 1of our 5 primary care clinics. A total of 92 randomly selected adults (mean age = 59 years) completed the TOFHLA while waiting for a scheduled appointment. Twenty eight percent of the sample had less than adequate levels of functional health literacy. Income, car ownership and education were significant and independent predictors of literacy level in this sample. Low levels of functional health literacy limit a patient's ability to read, understand and act on health information. It is essential …

The purpose of this study was to (a) determine which caring behaviors of the nurse do patients’ perceive as being the most and the least important, (b) examine patients’ perceptions of nurse caring behaviors in relationship to their age and perception of illness; and (c) compare and contrast responses between patients and parents of children who are patients. Data for this questionnaire (N = 59) were collected over a two-month period at a not-for-profit, teaching hospital, which specializes in the care of women and children in an Appalachian state located in the southeastern United States. The prevalence of heel ulcers across settings is high and is increasing. Prevention of ulcers requires knowledge of their etiology and the scientific basis for preventive care. The interaction between external pressure and the heel vasculature is central to the prevention of heel ulcers. This article focuses on the prevention of heel pressure ulcers. The physiology of heel tissue perfusion, the effect of external pressure on heel perfusion, as well as what is known about strategies to reduce external pressure and approaches to improve heel skin blood flow will be discussed. Fracturing a hip is a devastating experience for the older adult, and mortality and morbidity following a hip fracture are high among this vulnerable population. Mortality rates from 12 to 37 percent have been reported within one year after a hip fracture. It is estimated that 50 percent of individuals who experience hip fractures become partially dependent in activities of daily living, and 33 percent become totally dependent in activities of daily living. Reducing morbidity by improving rehabilitation outcomes is an important healthcare goal for older adults. Transportation arrangements are an integral but fragile element in the effectiveness of adult day care services in Vermont and nationwide. Almost by definition, adult day center participants generally cannot drive due to cognitive and/or physical limitations. Since adult day care services are congregate in nature and serve community-residing elders, this long-term care option is feasible only when there are arrangements to transport elders to and from service centers. Transportation is therefore a major issue for adult day care services.

Aged widowers are at risk for increased incidence of health problems and higher mortality rates following the death of a spouse. Mens' abilities for dealing with loss have been questioned, but little research has been done with this group. Surviving widowers provided significant insights illuminating the experience of resilience and providing relevant information regarding this population. Utilizing interpretive phenomenology, a methodology advocated by van Manen, in-depth interviews were conducted researching the lived experience of resilience among nineteen elderly widowers between 71 years and 100 years of age. Academic nursing practice holds great promise for the future of the nursing discipline. The successful and intentional integration of the tripartite mission of research, education, and clinical practice can facilitate both the evolution of the science and implementation of evidence-based practice, while imbuing practitioners in the making with the world of the possible. The demographics of the population of the United States are inexorably changing: The baby boomers, who have defined much of the culture of the past half century, will continue to do so over the next 50 years. Just as the population is aging, so are the members of the nursing profession. Working with elders as our patients and as our peers is a reasonable expectation.
