Abstract

Industrial workers exposed to the organochlorine pesticide, chlordecone (Kepone), had signs of toxicity in several organs. The extent of toxicity was proportional to the levels of this chemical in the tissues. In 22 patients, chlordecone was eliminated slowly from blood (half time of 165±27 days — mean ± S.E.M.) and fat (half time of 125 days, with a range of 97 to 177), chiefly in the stool. Output of chlordecone in bile was 10 to 20 times greater than in stool, suggesting that chlordecone is reabsorbed in the intestine. The purpose of this study was to determine the influence of isotonic and isokinetic weight training on vertical jumping proficiency as measured by a modified vertical jump. The subjects were college students enrolled in the Fitness and Lifetime Activities Program at South Dakota State University in Brookings, South Dakota. Isotonic and isokinetic training were assigned to the two experimental groups with the third group serving as controls. Training for the experimental groups consisted of isotonic circuit training on a Universal weight machine and isokinetic training on a Leaper machine. Aqueous microbiological and cytological studies were performed on 134 anterior chamber taps done on 118 patients both with and without uveitis. Cell wall defective bacterial forms (CWDF) were recovered from about 40% of these aqueous specimens and classical bacteria were recovered from 12%. A statistically significant difference could not be found between the incidence of bacterial cell wall defective forms in the various anatomical types of uveitis. There were hints from this work that the presence of CWDF In the aqueous was more frequent in younger patients with uveitis, early in the uveitis attack, if there were multiple foci of …

Effect Of Steroids On Patients With Postoperative Bowel Anastomotic Dysfunction, Luis H. Toledo-Pereya, Ignacio Guzman, Earl G. Yonehiro

Ten patients who developed anastomotic edema following large or small intestinal anastomosis had a rapid recovery after treatment with dexamethasone. The clinical and radiologic picture of adynamic ileus disappeared rapidly. There were no Immediate or long-term (more than two years) complications or side effects associated with the use of dexamethasone. This preliminary report indicates that dexamethasone may be a beneficial agent In the control of anastomotic edema.

Legionnaires' Disease: Description Of A Sporadic Case, Louis D. Saravolatz, Gene Tilchen

This first case of Legionnaires' disease seen at Henry Ford Hospital is described. A previously healthy 72-year-old man developed progressive pneumonia in September, 1977; it was unresponsive to treatment with cephalothin. However, he rapidly recovered when the treatment was changed to oral erythromycin. Diagnosis was established by serologic studies done at the Center for Disease Control. This pneumonia occurred in a sporadic form with no evidence of secondary cases.

Long-Term Survival After Onset Of Blast Crisis In Chronic Granulocytic Leukemia: Case Report And Therapeutic Considerations, Ellis J. Van Slyck, Ahmad Samhouri

Current analysis indicates a median survival of 3.6 months in 356 cases of chronic granulocytic leukemia after onset of blast transformation. Thirty-five (10.0%) complete remissions (CR), all of short duration, were observed. Closer scrutiny of the reported clinical experience offers clues for improving this poor outlook. A 76% CR rate can be extracted from this total experience by identifying cases treated with cytoslne arabinoside combined with one or more other agents. In isolated reports, weekly vincristine and prednisone have yielded good remission rates. Two general morphologic types of blasts can be distinguished by cytochemical stains and other features: lymphoid and …

Liver cell adenoma and focal nodular hyperplasia have been recognized as rare benign tumors. Their relationship with the use of oral contraceptives has been suggested since 1973. At Henry Ford Hospital, we have had four patients, ages 19, 23, 25,and 30, on oral contraceptives who presented with different clinical pictures suggesting acute abdomen. A hepatic tumor was suspected preoperatively in one patient, was found at laparotomy for biliary tract disease in two other cases, while the fourth patient presented with acute intra-abdominal hemorrhage. The prevalence of heart disease in the population over age 65 has contributed to the increasing number of aged patients hospitalized in critical care units (Gsell, 1977). Changes which occur in the process of aging and in cardiac disease should be considered when providing nursing care to these patients. A disturbance in the sleep wakefulness cycle resulting in a decrease of restorative sleep is a major change occurring at age 65 and after (Feinberg, 1967). The cardiac patient finds that it takes longer to go to sleep, and then awakens often during the night (Karacan et al., 1969).

The Rear-End Collision — Soma Or Psyché?, Hugh A. Clarke

Rear-end collisions represent 25% of all vehicular accidents. On occasion, the patients become more disabled by the diagnosis of "whiplash" than by the injury. During the past three years, 100 patients were referred for neurological examination and disposition following rear-end collisions. Appropriate neurological testing, included the Minnesota Multiphasic Personality Inventory Test (MMPI). A minority of these patients required hospitalization. The incidence of disc rupture requiring surgical decompression was less than 1%. The psychological tests were of limited aid in helping the physician to treat this group of patients. Cryptoccal Granuloma Of The Choroid Plexus In The Temporal Horn, Yohsuke Fukami

A man developed sudden symptoms of increased intracranial pressure following a prolonged course of cryptoccal meningitis which appeared to be responding to treatment. A choroid plexus granuloma was found in the dilated and blocked lateral ventricle. The poor prognosis of this type of complication of the cryptococcal meningitis is discussed.

The water-soluble contrast medium lumbar myelogram is a very helpful tool in the diagnosis of herniated nucleus pulposus (EfNP) in patients who suffer unilateral root pain of mechanical course. One can often make the differential diagnosis between a ruptured disc and a protruding disc. Unless the disc fragment lies way out in the intervertebral foramen, the myelogram will show some kind of anomaly. Therefore, fewer exploratory operations will be performed on patients having normal myelograms. Metrisamide, a newcomer among the water-soluble contrast media, is nontoxic for the spinal cord. Therefore, if the usual myelogram is normal and one fears a …
