Hemoglobin A₂ is often elevated in β-thalassemia and decreased in α-thalassemia. This might be due to hemoglobin subunit-subunit affinity variation. It has been inferred from the study of abnormal hemoglobins that the a subunits have higher affinity for β subunits than for δ subunits. However, only in one study has the affinity of α, β, and δ subunits for each other been measured. In this work we have attempted to measure the hemoglobin subunit-subunit affinity with somewhat different approach, i.e., hybridization of hemoglobin A and A₂. The Role Of The Insulin Receptor Tyrosine Kinase In Two Models Of Insulin Resistance, Nancy Barchus Block

Activation of the insulin receptor tyrosine kinase in vitro and in vivo was studied in two rat models of insulin resistance: hypercortisolemia and streptozotocin-induced diabetes. In control rats, intravenous insulin administration resulted in dose-dependent in vivo activation of the skeletal muscle insulin receptor kinase. Chronic cortisone treatment did not affect the kinase activity of receptors solubilized from muscle after activation with insulin in vivo or in vitro. In contrast, diabetes depressed skeletal muscle insulin receptor kinase activation in vivo by ~50%. Diabetes impaired the in vitro insulin stimulation of receptor kinase activity from skeletal muscle and liver towards the substrate …

Animal bites have become alarmingly common and may represent a quiet epidemic. It is estimated that between 1 and 3.5 million animal bites occur annually in the United States. The highest incidence has consistently been in 5-to-14-year-old schoolchildren, who have greater contact with animals, especially house pets, on a daily basis. This article discusses the epidemiology, microbiology, clinical findings and management of animal bites infections.

Over two years, 9892 mid-stream urine samples from patients attending the Aga Khan University Hospital, Karachi were cultured. Significant bacterial growth was seen in 23.5% samples. Further iden­tification of these organisms Eevealed 40% of E.coli, 16% Pseudomonas aeruginosa, 11% Klebsiella aerogenes, 5.0% Enterobacter sp., 13% Protdus sp., 4.0% Serratia liquifaciens, 1.0% Acinetobacter sp., 3.0% Citrobacter sp., 4.0% Enterococci, 0.5% Staphylococcus aureus. Results of sensitivity tests performed with antibiotics Ampicillin, Cotrimoxa.zole, Nitrofurantoin, Nalidixic acid, Gentamicin, Amikacin, Pipemedic acid, Cefotaxime, Azactain and Carbenicillin did not reveal any distinct pattern

Local Anaesthetic Cream For The Alleviation Of Pain During Venepuncture In Tanzanian Schoolchidren, U. Hellgren, C.M. Kihamia, Zul Premji, K. Danielson

The analgesic effect and the usefulness of EMLA cream 5% in connection with venous blood-sampling was investigated in 42 Tanzanian schoolchildren. Approximately 2.5 g EMLA was applied to the right cubital fossa for a minimum of 120 min. The analgesic effect was pronounced - 93% of the venepunctures were pain-free and no child experienced severe pain. No adverse reactions were observed and the children could continue normal school work during the application time.

D rugs may cause thrombocytopenia by three principal mechanisms: suppression of platelet production, direct platelet toxicity, or antibody-mediated platelet destmction (1). With the advent of assays specifically capable of detecting and quantifying IgG on the platelet surface (2), it has become increasingly possible to recognize cases of drug-induced immune thrombocytopenia based on the detection of elevated levels of platelet surface bound IgG in association with ingestion of causative dmgs. Consequendy, dmg-induced immune thrombocytopenia is no longer a disease diagnosed primarily by exclusion. Ten patients with idiopathic thrombocytopenic purpura (ITP) were studied before and following a rise in circulating platelets subsequent to infusions of intravenous gammaglobulin (400 mg/kg/day x 5 days). We quantitated the amount of circulating IgG capable of binding to normal donor platelets in vitro using an 125I-monoclonal anti-human IgG assay, as well as the amount of IgG associated with the patients' platelets before and following therapy. We found no evidence for a decrease in platelet-specific IgG antibodies in these patients undergoing an acute response to therapy. With increasing automation in Clinical Laboratories, the requirements for quality control material have greatly increased in order to monitor performance. The constant use of commercial control material is not economically feasible for many countries because of non-availability or the high cost of these materials. Here we describe a simple technique to prepare Home Made Quality Control serum using blood from polycythaemic patients. This preparation is stable for about six months without any alterations in the concentration of any of the chemical constituents. Congenital hypothyroidism is a preventable cause of mental retardation. Since clinical signs of congenital hypothyroidism do not generally become obvious before three months of age, screening programmes have been introduced in North America and Europe, which consist of T4 or TSH screening on newborn infants on the third day of life. The screening for congenital hypothyroidism was initiated in Pakistan by the Aga Khan University Hospital (AKUH) in March 1987. By April 1988, 5000 neonates were screened and five cases of congenital hypothyroidism were diagnosed. The study revealed the incidence of hypothyroidism to be one case per 1000 newborns which …

Among the many challenges of providing care for children with multiple physical and developmental handicaps is the proper management of their respiratory difficulties. This study was designed to evaluate the prevalence of respiratory symptoms in children attending a multidisciplinary clinic for handicapped children. We also used several methods to estimate the prevalence of allergic sensitivity in children with respiratory complaints. Questionnaires concerning respiratory symptoms were completed by the parents of 104 patients. The most frequent complaint reported was snoring in 41% of the children. Of the 39% who had multiple respiratory symptoms, 85% had detectable allergen- specific IgE to cat, …

We report the clinicopathologic features of an infant who died of a rare form of perinatal cirrhosis associated with idiopathic hepatic and extrahepatic parenchymal siderosis. The infant appeared normal at birth but soon became severely ill, following a progressively downhill course associated with hypoglycemia, metabolic acidosis, bleeding diathesis, jaundice, and shock. The infant died at 7 days of age. The manifestations were those of hepatic failure but mimicked sepsis and disseminated intravascular coagulation. Cirrhosis, giant cell transformation, and parenchymal iron deposition characteristic of perinatal idiopathic hemochromatosis, a recently emerging clinicopathologic entity of unknown etiology, were present in this infant.
