In the brain, mitochondrial metabolism has been largely associated with energy production, and its dysfunction is linked to neuronal cell loss. However, the functional role of mitochondria in glial cells has been poorly studied. Recent reports have demonstrated unequivocally that astrocytes do not require mitochondria to meet their bioenergetics demands. Then, the question remaining is, what is the functional role of mitochondria in astrocytes? In this work, we review current evidence demonstrating that mitochondrial central carbon metabolism in astrocytes regulates overall brain bioenergetics, neurotransmitter homeostasis and redox balance. Tuberculosis (TB) is the global health concern because of the rising incidence and mortality. TB is caused by Mycobacterium tuberculosis complex (MTBC) and transmitted via inhalation of infectious droplet nuclei circulating in the air. The World Health Organization (WHO) recommends the use of Acid-fast (AFB) smear microscopy, MTBC culture, and molecular detection of MTBC DNA for diagnosis of TB. The response to treatment should be acquired as soon as possible to reduce disease transmission and drug resistance. Background: P-cresyl sulfate and indoxyl sulfate are strongly associated with cardiovascular events, and all-cause mortality in chronic kidney disease (CKD). This study was conducted to compare the effects between sevelamer and calcium carbonate on protein-bound uremic toxins in pre-dialysis CKD patients with hyperphosphatemia. Methods: After 2 weeks of the run-in period, 40 pre-dialysis CKD patients with persistent hyperphosphatemia were randomly assigned to receive either daily 2,400 mg of sevelamer or 1,500 mg of calcium carbonate for 24 weeks. Classification of multicategory survival-outcome is important for precision oncology. Machine learning (ML) algorithms have been used to accurately classify multi-category survival-outcome of some cancer-types, but not yet that of lung adenocarcinoma. Therefore, we compared the performances of 3 ML models (random forests, support vector machine [SVM], multilayer perceptron) and multinomial logistic regression (Mlogit) models for classifying 4-category survival-outcome of lung adenocarcinoma using the TCGA. Mlogit model overall performed similar to SVM and multilayer perceptron models (micro-average area under curve=0.82), while random forests model was inferior. Studies indicate that many American older adults are not meeting their nutrition needs and often suffer from food insecurity. Meals on Wings (MOW) is a food recovery-meal delivery program that attempts to decrease the influence of food insecurity among older adults. This study aims to explore the self-perceived impact of a food recovery-meal delivery program on homebound seniors’ nutrition health, food security, and well-being.

Immune dysregulation is a hallmark of various chronic lung diseases such as Chronic Obstructive Pulmonary Diseases (COPD) and Idiopathic Pulmonary Fibrosis (IPF). Previous studies in our lab have shown evidences of autoimmunity such as increases in autoantibodies, abnormal T cell profile, immune complex deposition and the presence of ectopic lymphoid structures in the lung of these patients. This thesis consolidates two separate projects about various aberrant immune responses in these diseases. The first project identified Glucose-Regulated-Protein 78 (GRP78) as the common autoantigen in COPD and characterized how GRP78 autoantibodies may increase COPD mortality via its atherogenic effects. Glioblastoma multiforme (GBM) is a highly invasive, highly vascularized, and heterogeneous malignant tumor of the brain. Due to the highly infiltrative phenotype of GBM, surgery often leaves behind residual tumor cells. In many cases, recurrence occurs close to the surgical margin suggesting the role of these remaining cells in promoting tumor aggressiveness. Circadian rhythms are biological processes that cycle every ~24 hours and have allowed life to adapt for optimal function across the day. These rhythms are present in all tissues in the body and are coordinated to the light/dark cycle by the suprachiasmatic nucleus (SCN) in the hypothalamus. Along with active-rest cycles, cognitive performance fluctuates across the course of the day, peaking during the active phase. Objective: We sought to examine differences in aerobic activity (AA) and muscle-strengthening activity (MSA) by diabetes risk status (DRS) among pregnant women in the United States.

Background: Pregnant women without complications are advised to engage in physical activity (PA) to mitigate adverse outcomes. Differences may exist among pregnant women of diverging diabetes status in meeting national PA recommendations.

Purpose Examine the relationships between high-sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6), and soluble tumor necrosis factor-α receptor-1 (sTNF-R1) and the cumulative risk of heart failure with reduced (HFrEF) and preserved (HFpEF) ejection fractions in a diverse, population-based sample. Methods Study sample included 6,814 adult (45-84 years of age) men and women who participated in the Multi-Ethnic Study of Atherosclerosis and were free of cardiovascular disease at baseline. Obesity is regarded as a major risk factor for developing renal cell carcinoma (RCC). Advanced-stage RCC exhibits chemotherapeutic resistance, but is responsive to immunotherapies such as the immune checkpoint inhibitor anti-programmed cell death-1 (anti-PD-1). Despite some clinical successes, response rates remain low for anti-PD-1 monotherapy (20-30%). Preclinical evaluations of immunotherapeutic strategies typically use lean mice and do not account for patient comorbidities. This may explain the underwhelming success rates following clinical translation of new cancer treatments. Currently, only two drugs, benznidazole and nifurtimox, are available for treatment of CD and they are highly toxic and less effective in the chronic stage of the disease. ความเป็นมา และวัตถุประสงค์: การรักษามะเร็งปอดชนิดเซลล์ไม่เล็กระยะแพร่กระจาย ได้มีการพัฒนาการรักษามาอย่างต่อเนื่องและมีประสิทธิภาพ จนกระทั่งการรักษาด้วยยาภูมิคุ้มกันบำบัด ที่ออกฤทธิ์ต้าน programmed cell death ligand 1 (Anti PD-L1) และ/หรือยาที่ออกฤทธิ์ต้าน programmed cell death (Anti PD-1) แต่อย่างไรก็ตามยังมีข้อจำกัดถึงข้อมูลทางชีวภาพในการบ่งบอกถึงประสิทธิภาพของการรักษาด้วยยาภูมิคุ้มกันบำบัดในผู้ป่วยแต่ละราย จากข้อมูลการศึกษาก่อนหน้านี้พบว่าดัชนีมวลกายนั้นมีความสัมพันธ์กับการตอบสนองต่อการรักษาด้วยยาภูมิคุ้มกันบำบัด ดังนั้นจึงทำการศึกษาเพื่อหาความสัมพันธ์ของผลกระทบต่อสถานะทางโภชนาการโดยการประเมินด้วยแบบฟอร์ม patient generated subjected Global Assessment (PG-SGA) และปัจจัยหรือข้อกำหนดอื่นทางโภชนาการต่อผลลัพธ์ของการรักษาด้วยยาภูมิคุ้มกันบำบัดในผู้ป่วยมะเร็งปอดชนิดเซลล์ไม่เล็กระยะแพร่กระจาย วิธีการศึกษา: การศึกษาแบบไปข้างหน้าในผู้ป่วยโรคมะเร็งปอดชนิดเซลล์ไม่เล็กระยะแพร่กระจายที่ได้รับการรักษาด้วยภูมิคุ้มกันบำบัด โดยประเมินภาวะทางโภชนาการด้วยแบบฟอร์ม PG-SGA ในวันที่เริ่มการรักษาและสัปดาห์ที่ 12 หลังเริ่มการรักษา ตรวจวัดองค์ประกอบของร่างกายและเก็บตัวอย่างเลือดเพื่อวัดระดับพารามิเตอร์ทางโภชนาการ บันทึกประวัติการรับประทานอาหารด้วยสมุดบันทึกและวิเคราะห์ข้อมูลด้วยโปรแกรมอินมูแคล (INMUCAL-N) เพื่อศึกษาความสัมพันธ์ของภาวะโภชนการและปัจจัยที่เกี่ยวข้องต่อผลลัพธ์ของการรักษาด้วยยาภูมิคุ้มกันบำบัดในผู้ป่วยโรคมะเร็งปอดชนิดเซลล์ไม่เล็กระยะแพร่กระจาย ผลการศึกษา: ผู้ป่วยทั้งหมด 20 ราย พบว่าผู้ป่วย 18 รายมีภาวะทุพโภชนการเมื่อประเมินด้วยแบบฟอร์ม PG-SGA ระยะเวลาเฉลี่ยของการติดตามการรักษา 7 เดือน โดยในผู้ป่วยที่ภาวะทางโภชนาการปกติ (PG-SGA A) มีระยะเวลาการรอดชีวิตที่ปราศจากความก้าวหน้าที่ยาวนานกว่าผู้ป่วยที่มีภาวะทุพโภชนาการ (PG-SGA B/C) และปัจจัยอื่นได้แก่ ดัชนีมวลกาย (BMI), ระดับ NLR, pre-albumin, hs-CRP ในเลือด มีความสัมพันธ์ต่อผลลัพธ์ของระยะเวลาการรอดชีวิตของผู้ป่วยที่ยาวนานขึ้น สรุปผลการวิจัย: ภาวะทางโภชนาการที่ประเมินด้วย PG-SGA อาจจะมีความสัมพันธ์ต่อผลลัพธ์ของการรักษาด้วยยาภูมิคุ้มกันบำบัดในมะเร็งปอดชนิดเซลล์ไม่เล็กระยะแพร่กระจาย โดยภาวะทุพโภชนาการมีผลลัพธ์ต่อระยะเวลาการรอดชีวิตที่ปราศจากความก้าวหน้าที่แย่กว่า

วัตถุประสงค์ เพื่อศึกษาอัตราการการลดลงของสารยูรีมิกมวลโมเลกุลขนาดกลาง ß2-microglobulin ในผู้ป่วยไตวายเรื้อรังระยะท้ายที่ทำการฟอกเลือดแบบดั้งเดิมโดยใช้ตัวกรองรูใหญ่ปานกลาง และการฟอกเลือดเทคนิคมิกซ์ไดลูชั่นฮีโมไดอะฟิวเตรชั่นโดยใช้ตัวกรองมาตรฐาน วิธีการวิจัย การวิจัยเชิงทดลองแบบสุ่มไขว้กลุ่มแบบไปข้างหน้า ในผู้ป่วยไตวายเรื้อรังระยะท้ายที่ทำการฟอกเลือดด้วยเครื่องไตเทียม 3 ครั้งต่อสัปดาห์ที่โรงพยาบาลจุฬาลงกรณ์จำนวนทั้งหมด 14 ราย โดยแบ่งเป็นกลุ่มที่ได้รับการฟอกเลือดด้วยเทคนิคมิกซ์ไดลูชั่นฮีโมไดอะฟิวเตรชั่นโดยใช้ตัวกรองมาตรฐาน และกลุ่มที่ได้รับการฟอกเลือดแบบดั้งเดิมโดยใช้ตัวกรองรูใหญ่ปานกลาง ทั้งสองกลุ่มได้รับการเก็บเลือดส่งตรวจสารยูรีมิกในทุกวันที่มาฟอกกลางสัปดาห์ทุกสัปดาห์ จนครบ 8 สัปดาห์ก่อนฟอก และหลังฟอก และนำมาหาค่าเฉลี่ยของอัตราการลดลงของสารของแต่ละเทคนิค รวมทั้งยังเก็บน้ำยาไตเทียมที่ได้จากการฟอกเพื่อนำมาหาค่าเฉลี่ยของอัลบูมินที่สูญเสีย ผลการศึกษา พบว่าการฟอกเลือดด้วยเทคนิคมิกซ์ไดลูชั่นฮีโมไดอะฟิวเตรชั่นสามารถกำจัดสารยูรีมิกมวลโมเลกุลขนาดปานกลาง ß2-microglobulin ได้มากกว่าค่าที่ส่งผลดีต่ออัตราการรอดชีวิตคือร้อยละ 80 และยังมากกว่าการฟอกเลือดแบบดั้งเดิมโดยใช้ตัวกรองรูใหญ่ปานกลาง โดยค่าเฉลี่ยของอัตราการลดลงของ ß2-microglobulin และค่าเบี่ยงเบนมัธยฐานในเทคนิคมิกซ์ไดลูชั่นฮีโมไดอะฟิวเตรชั่น และการฟอกเลือดแบบดั้งเดิมโดยใช้ตัวกรองรูใหญ่ปานกลาง เท่ากับ 85.12 ± 3.87 และ 82.57 ± 5.34 ตามลำดับ ซึ่งมีค่าเฉลี่ยของความแตกต่างระหว่างกลุ่มเท่ากับ 2.56 ซึ่งมีนัยสำคัญทางสถิติ (p=0.001) สำหรับสารยูรีมิกมวลโมเลกุลขนาดเล็ก ขนาดกลาง และสารยูรีมิกที่จับกับโปรตีน ได้แก่ ยูเรีย, Ƙ-free light chain และอินดอกซิลซัลเฟต ไม่พบความแตกต่างของความสามารถในการกำจัดระหว่าง 2 กลุ่ม อย่างไรก็ตามสารยูรีมิกมวลโมเลกุลขนาดปานกลางที่มีขนาดใหญ่ขึ้น เช่น ɑ1-microglobulin และ λ-free light chain พบว่าการฟอกเลือดแบบดั้งเดิมโดยใช้ตัวกรองรูใหญ่ปานกลางสามารถกำจัดออกได้มากกว่าการฟอกเลือดด้วยเทคนิคมิกซ์ไดลูชั่นฮีโมไดอะฟิวเตรชั่น อย่างมีนัยสำคัญทางสถิติ (p <0.001) โดยค่าเฉลี่ยของอัตราการลดลงของ ɑ1-microglobulin เท่ากับ 41.49 ± 11.46 และ 30.13 ± 15.90 ตามลำดับ และ λ-free light chain เท่ากับ 50.81 ± 13.18 และ 40.85 ± 13.92 ตามลำดับ ในแง่ของการสูญเสียอัลบูมินทางน้ำยาไตเทียมพบว่าการฟอกเลือดแบบดั้งเดิมโดยใช้ตัวกรองรูใหญ่ปานกลางสูญเสียอัลบูมินมากกว่าการฟอกเลือดด้วยเทคนิคมิกซ์ไดลูชั่นฮีโมไดอะฟิวเตรชั่น เท่ากับ 3.51กรัมต่อครั้ง และ0.58 กรัมต่อครั้งตามลำดับซึ่งมีนัยสำคัญทางสถิติ (p=0.025) แต่เมื่อพิจารณาแง่ของระดับอัลบูมินในเลือดพบว่าทั้ง 2 กลุ่มไม่แตกต่างกัน สรุป การศึกษานี้แสดงให้เห็นว่าการฟอกเลือดทั้ง 2 เทคนิคสามารถกำจัดสารยูรีมิกได้ในระดับที่เหมาะสม ดังนั้นในกรณีที่เครื่องมือฮีโมไดอะฟิวเตรชั่นไม่พร้อมใช้สามารถนำการฟอกเลือดแบบดั้งเดิมโดยใช้ตัวกรองรูใหญ่ปานกลางมาใช้ทดแทนกันได้

ที่มาและวัตถุประสงค์ ปัจจุบันอุบัติการณ์การเกิดกรดไหลย้อน (Gastroesophageal reflux disease) และ ท้องผูก (Constipation) พบร่วมกันได้บ่อย แต่ยังไม่มีแนวทางการรักษาผู้ป่วยกลุ่มนี้อย่างชัดเจน การที่พบสองภาวะนี้ร่วมกันนั้นยังไม่ทราบว่าแน่ชัดว่ามีความสัมพันธ์ หรือกลไกที่เกี่ยวข้องกันระหว่างภาวะกรดไหลย้อน และภาวะท้องผูกหรือไม่ วิธีการวิจัย การศึกษาวิจัยทดลองแบบไขว้และสุ่ม ในผู้ป่วยโรคกรดไหลย้อน คือ มีอาการแสบร้อนกลางอก หรือเรอเปรี้ยว อย่างน้อย 3 วัน/สัปดาห์ ร่วมกับท้องผูกเรื้อรัง โดยท้องผูกมีอาการถ่ายอุจจาระ ≤ 2 ครั้ง หรือ ถ่ายลักษณะอุจจาระก้อนเล็กแข็งเป็นกระสุน รูปทรงยาวผิวตะปุ่มตะป่ำ หรือรูปทรงยาวผิวแตก (BSFS 1-3) ในช่วง 7 วัน จำนวนทั้งหมด 12 ราย อายุ 18-80 ปี โดยแบ่งกลุ่มการศึกษาเป็น 2 กลุ่ม และทำการทดลองแบบไขว้กัน โดยช่วงเวลาที่ผู้ป่วยจะเข้าร่วมแต่ละกลุ่มนั้นต้องห่างกันอย่างน้อย 2 สัปดาห์ ผู้ป่วยที่เข้าการศึกษากลุ่มอุจจาระค้างในลำไส้ใหญ่ ผู้ป่วยจะได้รับการรับประทานแคปซูลที่บรรจุสารทึบรังสี จำนวน 20 ชิ้น ในวันที่ 1 ต่อมาในเช้าวันที่ 4 ของการเข้าร่วมการศึกษา จะได้รับทำการถ่ายภาพรังสีบริเวณท้อง (x-ray abdomen) หากพบว่ามีสารทึบรังสีคั่งค้าง marker ≥ 90% (≥18/20) จะยืนยันเข้าศึกษาในกลุ่มอุจจาระค้างในลำไส้ใหญ่ และอีกกลุ่มผู้ป่วยจะได้รับการรับประทานแคปซูลที่บรรจุสารทึบรังสี จำนวน 20 ชิ้น ในวันที่ 1 เหมือนกันและสวนอุจจาระด้วย Unison enema วันละครั้งจำนวน 4 ครั้ง ต่อมาในเช้าวันที่ 4 หากถ่ายภาพรังสีบริเวณท้องพบว่ามีสารทึบรังสีคั่งค้าง marker < 90% (<18/20) จะยืนยันเข้าศึกษาในกลุ่มไม่มีอุจจาระค้างในลำไส้ใหญ่ ในการศึกษาแต่ละกลุ่มอาสาสมัครจะต้องงดน้ำและอาหารอย่างน้อย 6 ชั่วโมงก่อนรับการตรวจ โดยจะได้รับการตรวจการขยายตัวของกระเพาะอาหาร (Gastric accommodation) ด้วยวิธี Satiety nutrition drink test หลังจากตรวจสิ้นสุด 4 ชั่วโมงต่อมาจะได้การตรวจวัดการย้อนของกรดและน้ำย่อยในหลอดอาหาร (esophageal impedance pH monitoring) โดยให้อาสาสมัครทานอาหารควบคุม 520 กิโลแคลอรี เก็บข้อมูลการไหลย้อนเข้ามาในหลอดอาหารโดยดูการเปลี่ยนแปลงความต้านทานในหลอดอาหาร (esophageal impedance) นาน 2 ชั่วโมง และตอบแบบสอบถามประเมินอาการทางเดินอาหาร ความรุนแรงของอาการระบบทางเดินอาหารส่วนต้นก่อนรับการตรวจช่วงที่งดน้ำและอาหาร และหลังจากเสร็จสิ้นการตรวจการขยายตัวของกระเพาะอาหารส่วนต้น นาน 30 นาที และเป่าลมหายใจเพื่อส่งตรวจระดับ ไฮโดรเจน และมีเทน ในช่วงงดน้ำและอาหาร เพื่อเปรียบเทียบระหว่างสองกลุ่มการศึกษา ผลการศึกษา ผู้ป่วยโรคกรดไหลย้อนร่วมกับท้องผูกเรื้อรัง ในภาวะที่มีการค้างของอุจจาระในลำไส้ใหญ่พบว่ามีอาการทางเดินอาหารรบกวนโดยรวมที่มากกว่าโดยมีค่ามัธยฐาน 7 (3.3-8) เทียบกับกลุ่มที่ไม่มีอุจจาระค้างในลำไส้ใหญ่จำนวนค่ามัธยฐาน 4.5 (2.3-6) ซึ่งมีนัยสำคัญทางสถิติ (p=0.04)และอาการท้องอืดเหมือนมีลมในท้องในระดับที่มากกว่าโดยมีค่ามัธยฐาน 5.5 (4-8) เทียบกับภาวะที่ไม่มีอุจจาระค้างในลำไส้ใหญ่มีค่ามัธยฐาน 3 (2-5) อย่างมีนัยสำคัญทางสถิติ (p=0.02) โดยในช่วงอดอาหารและก่อนทานอาหาร ไม่พบว่ามีความแตกต่างของ ระดับไฮโดรเจน และมีเทนในลมหายใจ ในกลุ่มอุจจาระค้างในลำไส้ใหญ่ พบว่ามีจำนวนการเกิดการไหลย้อนของของเหลวสู่หลอดอาหารในระยะเวลา 2 ชั่วโมงหลังมื้ออาหาร จำนวนค่าเฉลี่ย 10.6 (4.8) จำนวนมากกว่ากลุ่มที่ไม่มีอุจจาระค้างในลำไส้ใหญ่จำนวนค่าเฉลี่ย 6.3 (4.1) ซึ่งมีนัยสำคัญทางสถิติ (p=0.003) ในกลุ่มอุจจาระค้างในลำไส้ใหญ่ หลังการทานมีอาการแสบร้อนกลางอกที่รบกวน มีค่ามัธยฐาน 2 (0-7.5) มากกว่ากลุ่มที่ไม่มีอุจจาระค้างในลำไส้ใหญ่จำนวนค่ามัธยฐาน 0 (0-0) ซึ่งมีนัยสำคัญทางสถิติ (p=0.04) และ ในกลุ่มอุจจาระค้างในลำไส้ใหญ่มีการขยายตัวของกระเพาะอาหารในปริมาตรค่าเฉลี่ย 591.7 (202.1) มิลลิลิตร ซึ่งมากกว่าเมื่อเปรียบเทียบกับกลุ่มที่ไม่มีอุจจาระค้างในลำไส้ใหญ่ ปริมาตรค่าเฉลี่ย 516.7 (158.6) มิลลิลิตร แต่ไม่มีนัยสำคัญทางสถิติ (p=0.07) สรุป ภาวะที่มีอุจจาระค้างในลำไส้ใหญ่มีความสัมพันธ์กับจำนวนการเกิดการไหลย้อนของของเหลวสู่หลอดอาหารที่มากขึ้น มีผลต่ออาการแสบร้อนกลางอกที่รุนแรงมากขึ้น และมีการขยายตัวของกระเพาะอาหารในปริมาตรที่มากขึ้นเช่นกัน จากผลการศึกษาภาวะที่มีการค้างของอุจจาระในลำไส้ใหญ่ ในผู้ป่วยโรคกรดไหลย้อนร่วมกับท้องผูกเรื้อรัง น่าจะเป็นเหตุปัจจัยที่ทำให้เกิดการเกิดไหลย้อนของของเหลวสู่หลอดอาหารที่มากขึ้นในผู้ป่วยกลุ่มนี้ ซึ่งอาจมีความสัมพันธ์กับการขยายขนาดของกระเพาะอาหารที่มากขึ้นหลังการรับประทาน

Some chronic rhinosinusitis with nasal polyps patients undergoing endoscopic sinus surgery (ESS) have unfavorable results despite proper postoperative treatments including oral and topical steroids. Steroid-impregnated absorbable nasal dressing has been shown to improve outcomes of the surgery. In some clinical practices, budesonide-impregnated nasal dressing is used together with perioperative oral steroid but the additional benefits of it are still unknown.
