Mechanical loading is essential for maintaining bone tissue. Reduced mechanical loading has been shown to have a negative effect on bone, and can result in the development of disuse osteoporosis. Disuse models of muscle inactivity and immobilization, like the Botox model used in this study, result in changes in the bone microarchitecture, the mechanisms behind which are not fully understood. In a previous four-week Botox disuse study, skeletally mature 20- week-old rats experienced degradation of intracortical bone, increased vascular porosity, and decreased osteocyte lacunar density in the tibiae. Background: Little is known regarding the clinical impact of treatment and treatment duration of probiotic VSL#3 on gut and microbiome function in irritable bowel syndrome (IBS). As part of a safety trial, we assessed the effect of VSL#3 treatment duration on abdominal pain, stooling, gut permeability, microbiome composition and function.
Methods: Adults with IBS were randomized into an open label trial to receive the probiotic VSL#3 for 4 or 8 weeks. Adverse events, abdominal pain, and stooling patterns were recorded daily. Gut permeability, fecal bile acid levels, and microbiome composition were profiled at baseline and after treatment.

Objectives: To determine how baseline weight status contributes to differences in postmenopausal weight gain among non-Hispanic Blacks (NHBs) and non-Hispanic Whites (NHWs).
Methods: Data were included from 70,750 NHW and NHB postmenopausal women from the Women's Health Initiative Observational Study (WHI OS). Body Mass Index (BMI) at baseline was used to classify women as having normal weight, overweight, obese class I, obese class II or obese class III. Cox proportional hazards was used to estimate the hazard of a 10% or more increase in weight from baseline.

Background: Gastrointestinal problems affect the health and quality of life of individuals with Rett syndrome (RTT) and pose a medical hardship for their caregivers. We hypothesized that the variability in the RTT phenotype contributes to the dysbiosis of the gut microbiome and metabolome in RTT, predisposing these individuals to gastrointestinal dysfunction.
Objectives: We characterized the gut bacterial microbiome and metabolome in girls and young women with RTT (n = 44) and unaffected controls (n = 21), and examined the relation between the composition of the microbiome and variations in the RTT phenotype.

Gut microbiota has been implicated as a modifier of childhood growth. Here, 16S rRNA sequencing-based fecal microbiota profiles of 18-24 month old Indian children were evaluated (n = 41), in relation to their anthropometric parameters, intestinal permeability, body composition and total energy expenditure. Pathway analyses were conducted to assess microbial functions related to stunting, underweight and wasting. The fecal microbiota was enriched in Prevotella 9, Bifidobacterium and Escherichia-Shigella. Weight, weight-for-age Z-scores (WAZ) and weight-for-length Z-scores (WLZ), along with age, acted as covariates of microbiota variation specifically in boys (n = 23). A variety of eating behaviors among children have been associated with obesity risk and are thought to broadly reflect child appetite self-regulation (ASR). While ASR is thought to occur on cognitive, emotional, motivational, biological, and behavioral levels, the inter-relatedness of ASR constructs as assessed by different methods/measures is not well-characterized. This narrative review describes the correspondence between different methods/measures of child ASR constructs as assessed by self-report questionnaires and/or observational tasks and their relationship to child standardized body mass index (BMIz). Obesity is often associated with metabolic dysregulation and oxidative stress with the latter serving as a possible unifying link between obesity and cardiovascular complications. Glutaredoxins (Grxs) comprise one of the major antioxidant systems in the heart. Although Grx3 has been shown to act as an endogenous negative regulator of cardiac hypertrophy and heart failure, its metabolic impact on cardiac function in diet-induced obese (DIO) mice remains largely unknown. Background: The use of physical activity tracker devices has increased within the general population. However, there is limited medical literature studying the efficacy of such devices in adolescents with obesity. In this study, we explored the feasibility of using wearable activity tracking devices as an adjunct intervention on adolescents with obesity.

Malnutrition, including both undernutrition and obesity, is a significant problem in low- and middle-income countries (LMICs). In order to study malnutrition and develop effective intervention strategies, it is crucial to evaluate nutritional status in LMICs at the individual, household, and community levels. In a multinational research project supported by the Bill & Melinda Gates Foundation, we have been using a wearable technology to conduct objective dietary assessment in sub-Saharan Africa. Skeletal muscle quantity and quality decrease with older age, which is partly attributed to ectopic fat infiltration and has negative metabolic consequences. To inform efforts to preserve skeletal muscle with aging, a better understanding of biologic correlates of quantity and quality of muscle and intermuscular adipose tissue (IMAT) is needed. An increasing number of investigations have established alternative polyadenylation (APA) as a key mechanism of gene regulation through altering the length of 3' untranslated region (UTR) and generating distinct mRNA termini. Further, appreciation for the significance of APA in disease contexts propelled the development of several 3' sequencing techniques. While these RNA sequencing technologies have advanced APA analysis, the intrinsic limitation of 3' read coverage and lack of appropriate computational tools constrain precise mapping and quantification of polyadenylation sites. AbstractCentral 5-hydroxytryptamine (5-HT), which is primarily synthesized by tryptophan hydroxylase 2 (TPH2) in the dorsal Raphe nuclei (DRN), plays a pivotal role in the regulation of food intake and body weight. However, the physiological functions of TPH2 on energy balance have not been consistently demonstrated. Here we systematically investigated the effects of TPH2 on energy homeostasis in adult male and female mice. We found that the DRN harbors a similar amount of TPH2+ cells in control male and female mice. Background/aim: Renal function of patients with hypothyroidism increases after reaching euthyroid state. There is no data regarding geriatric age group. The aim of the study was determined to investigate whether renal function of geriatric patients with hypothyroidism increases after they become euthyroid. Materials and methods: Patients who were sixty-five years or older were retrospectively screened in two centers. This study aimed to evaluate the role of frailty in predicting readmission, length of stay, and quality of life in the hospitalized older adults. Hepatoblastoma is the most common primary liver tumor in children. Despite an increasing incidence and major advances in care for other pediatric solid tumors, therapy for hepatoblastoma has remained virtually unchanged for the last 20 years, and children with metastatic or relapsed disease continue to suffer from a dismal prognosis. Metabolic disease encompasses various disorders, including obesity and diabetes, that negatively impact glucose and lipid homeostasis and increase the risk of co-morbidities. Adipose tissue, which regulates whole-body energy balance and acts as a specialized endocrine tissue, is negatively affected by obesity and diabetes. Brown adipose tissue (BAT) functions to dissipate excess energy as heat and therefore is an attractive target against metabolic disease. Many people living with HIV (PLWH) have cognitive impairment. Details of cognitive impairment subtypes are lacking. Unsupervised machine learning (ML) can reveal hidden subgroups within heterogeneous data. The study aimed to determine clusters of aging Thai PLWH with borderline cognitive impairment using unsupervised ML. HIV-NAT 207 study enrolled Thai PLWH aged ≥50 years. Cognitive performance was evaluated by the Thai-validated Montreal Cognitive Assessment (MoCA). This study included participants who scored between 23 and 27. Here, we investigated the prevalence of colistin resistant A. baumannii (CoR-AB) clinical isolates and their resistance mechanisms, and examined the in vitro synergistic activities of the antibiotic combinations against CoR-AB. ที่มา: การตรวจไขกระดูกถือเป็นสิ่งสำคัญที่ใช้ในการวินิจฉัยโรคทางโลหิตวิทยา การวิเคราะห์แปลผลไขกระดูกโดยแพทย์ผู้เชี่ยวชาญทางโลหิตวิทยาหรือพยาธิแพทย์ถือเป็นมาตรฐานในปัจจุบัน อย่างไรก็ตามการวิเคราะห์สเมียร์ไขกระดูกถือเป็นภาระงานที่ใช้เวลาค่อนข้างมาก ต้องอาศัยผู้มีความชำนาญมาก วัตถุประสงค์: เพื่อศึกษาประสิทธิภาพของเครื่องวิเคราะห์สเมียร์ไขกระดูกอัตโนมัติในการช่วยวินิจฉัยแยกโรคในกลุ่มมะเร็งเม็ดเลือดขาวเฉียบพลันชนิดมัยอีลอยด์และกลุ่มอาการไขกระดูกเสื่อมชนิดเอ็มดีเอสโดยดูจากปริมาณของบลาสต์ในไขกระดูกเป็นหลัก และเปรียบเทียบการจำแนกสัดส่วนเซลล์แต่ละประเภทในสเมียร์ไขกระดูกของทั้งสองวิธี ระเบียบวิธีวิจัย: การวิจัยนี้ศึกษาในตัวอย่างสเมียร์ไขกระดูกที่ทำในโรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย ในช่วงปี พ.ศ.2561-พ.ศ.2563 โดยสนใจศึกษาในสเมียร์ไขกระดูก 140 ตัวอย่าง แบ่งเป็น 4 กลุ่มเท่าๆกัน โดยตัวอย่างสเมียร์ไขกระดูกทั้งหมดในการวิจัยนี้จะถูกนำมาวิเคราะห์ 2 ครั้ง คือโดยเครื่อง Vision Hema® 8Pro และโดยแพทย์ผู้เชี่ยวชาญทางโลหิตวิทยา เพื่อนำผลการวิเคราะห์ของทั้งสองวิธีมาเปรียบเทียบตามวัตถุประสงค์ต่อไป ผลการศึกษา: จากการศึกษาในสเมียร์ไขกระดูกทั้งหมด 131 ตัวอย่าง ประกอบไปด้วย ไขกระดูกปกติ 31 ตัวอย่าง, ไขกระดูกเสื่อมชนิดเอ็มดีเอสชนิดบลาสต์น้อย 33 ตัวอย่าง, ไขกระดูกเสื่อมชนิดเอ็มดีเอสชนิดบลาสต์มาก 33 ตัวอย่าง และไขกระดูกโรคมะเร็งเม็ดเลือดขาวเฉียบพลันชนิดมัยอีลอยด์ 34 ตัวอย่าง พบว่าการวิเคราะห์สเมียร์ไขกระดูกเพื่อวินิจฉัยแยกกลุ่มโรคด้วยทั้งสองวิธีมีความสอดคล้องกันในระดับปานกลางโดยมีค่าสัมประสิทธิ์แคปปาของโคเฮน เท่ากับ 0.50 และ ค่า p < 0.001 สำหรับการจำแนกสัดส่วนเซลล์แต่ละประเภทในสเมียร์ไขกระดูกด้วยทั้งสองวีธี พบว่ามีความสอดคล้องค่อนข้างดีสำหรับ เซลล์ต้นกำเนิดของเม็ดเลือดแดงและเซลล์บลาสต์ โดยมีค่าสัมประสิทธิ์สหสัมพันธ์แบบเพียร์สัน เท่ากับ 0.72 (ค่า p < 0.001) และ 0.71 (ค่า p < 0.001) ตามลำดับ สรุปผล: จากการศึกษานำร่องนี้ แสดงให้เห็นว่าเครื่อง Vision Hema® 8Pro ยังไม่สามารถทดแทนแพทย์ผู้เชี่ยวชาญในการจำแนกโรคมะเร็งเม็ดเลือดขาวเฉียบพลันชนิดมัยอีลอยด์และกลุ่มอาการไขกระดูกเสื่อมชนิดเอ็มดีเอสโดยดูจากปริมาณบลาสต์ การพัฒนาระบบการวิเคราะห์ของเครื่องและการศึกษาเพิ่มเติมในกลุ่มโรคอื่นๆยังเป็นสิ่งจำเป็น

ที่มาของงานวิจัย ภาวะไตวายเฉียบพลัน (AKI) และภาวะหายใจล้มเหลวเฉียบพลัน (ARF) เป็นสองภาวะที่พบได้บ่อยและส่งผลต่ออัตราการเสียชีวิตในผู้ป่วยวิกฤต จุดมุ่งหมายของงานวิจัยคือการรายงานอัตราการเสียชีวิตในโรงพยาบาลของผู้ป่วยที่มีความสัมพันธ์ของทั้งสองภาวะนี้ในรูปแบบต่าง ๆ ระเบียบวิธีการวิจัย ผู้วิจัยได้เก็บข้อมูลผู้ป่วยจากการศึกษา SEA-AKI ซึ่งเป็นการศึกษานานาชาติที่เก็บข้อมูลของหอผู้ป่วยวิกฤตจากประเทศ ไทย ลาว และ อินโดนีเซีย และได้ทำการให้คำนิยามภาวะไตวายเฉียบพลัน ตามคำนิยามมาตรฐาน KDIGO ขั้นที่ 2-3 และให้คำนิยามภาวะหายใจล้มเหลวเฉียบพลันโดยการดูการใช้เครื่องช่วยหายใจ จากนั้นแบ่งกลุ่มผู้ป่วยออกเป็นทั้งหมด 6 กลุ่ม ตามลำดับการเกิดภาวะไตวายเฉียบพลันและภาวะหายใจล้มเหลวเฉียบพลันดังนี้ “no AKI/ARF”, “ARF alone”, “AKI alone”, “ARF first”, “AKI first”, “Concurrent AKI-ARF” และมีผลลัพธ์ปฐมภูมิของการวิจัยคืออัตราการเสียชีวิตในโรงพยาบาล ผลการศึกษา การศึกษาวิจัยได้รวบรวมข้อมูลผู้ป่วยมาทั้งหมด 5,468 ราย โดยพบว่าอัตราการเสียชีวิตในโรงพยาบาลขของกลุ่ม "Concurrent AKI-ARF" สูงที่สุดที่ 69.6% ในขณะที่ "AKI first" 54.4%, "ARF first" 53% , "AKI alone" 14.6%, "ARF alone" 31.5% และในกลุ่ม "no AKI/ARF" 12.4% สรุป ผู้ป่วยวิกฤตที่มีภาวะไตวายเฉียบพลันและภาวะหายใจล้มเหลวเฉียบพลันร่วมกันจะมีความเสี่ยงในการเสียชีวิตในโรงพยาบาลสูงขึ้น

ความสำคัญและที่มา: ภาวะรีบเร่งเข้าห้องน้ำเมื่อปวดถ่ายอุจจาระสามารถพบได้ในประชากรทั่วไปแต่จะมีอุบัติการณ์เพิ่มมากขึ้นในผู้ป่วยที่มีการขับถ่ายอุจจาระที่ผิดปกติไม่ว่าจะเป็นภาวะกลั้นอุจจาระไม่ได้ หรือ ภาวะท้องผูก แต่อย่างไรก็ตามยังไม่มีการศึกษาที่ครอบคลุมกลไกในการเกิดภาวะรีบเร่งเข้าห้องน้ำเมื่อปวดถ่ายอุจจาระ วัตถุประสงค์: เพื่อศึกษาระดับการรับความรู้สึกของไส้ตรงในผู้ป่วยที่มีความผิดปกติในการขับถ่ายร่วมกับมีภาวะรีบเร่งเข้าห้องน้ำเปรียบเทียบกับผู้ป่วยที่มีความผิดปกติในการขับถ่ายแต่ไม่มีภาวะรีบเร่งเข้าห้องน้ำโดยใช้เครื่องบาโรสแตท ระเบียบวิธีการวิจัย: งานวิจัยนี้มีการเปรียบเทียบข้อมูลพื้นฐาน คือ เพศ อายุ ดัชนีมวลกาย ซึ่งไม่แตกต่างระหว่างอาสาสมัครที่ไม่มีความผิดปกติในการขับถ่าย และผู้ป่วยที่มีความผิดปกติในการขับถ่ายร่วมกับมีและไม่มีภาวะรีบเร่งเข้าห้องน้ำ โดยนิยามของภาวะรีบเร่งเข้าห้องน้ำคือ มีภาวะรีบเร่งเข้าห้องน้ำ อย่างน้อย 1 ครั้ง/สัปดาห์ ในช่วง 3 เดือนที่ผ่านมา ระดับความรู้สึกของไส้ตรงแบ่งออกเป็น 8 ระดับ คือ 0 ไม่มีความรู้สึกในไส้ตรง 1 รู้สึกภายในไส้ตรงแต่ไม่ใช่ลมและอุจจาระ 2 รู้สึกว่ามีลมในไส้ตรง 3 รู้สึกอยากผายลม 4 รู้สึกว่ามีอุจจาระในไส้ตรงแต่ไม่ปวดอยากถ่าย 5 รู้สึกปวดอยากถ่ายอุจจาระ 6 รู้สึกต้องรีบเข้าห้องน้ำแต่สามารถกลั้นอุจจาระได้ 7 รู้สึกปวดอยากถ่ายมากไม่สามารถกลั้นอุจจาระได้ โดยใช้เครื่องบาโรสแตทในการตรวจวัดระดับการรับความรู้สึกของไส้ตรงและค่าความยืดหยุ่นของผนังไส้ตรง แบ่งออกเป็น 2 ขั้นตอน คือ การตรวจโดยเพิ่มความดันครั้งละ 4 มม.ปรอท เริ่มต้นที่ 4 มม.ปรอท คงไว้ที่ 1 นาที จากนั้นปล่อยออก มีระยะเวลาห่างกัน 1 นาที และเพิ่มความดันขึ้นเรื่อย ๆ จนกระทั่งถึง 50 มม.ปรอท และขั้นที่ 2 ตรวจโดยการเพิ่มความดันขึ้นเรื่อย ๆ จาก 4 มม.ปรอท ทุก 1 นาที โดยไม่มีการปล่อยลมออกจนกระทั่ง 50 มม.ปรอท ผลการวิจัย: ผู้ป่วยที่มีภาวะรีบเร่งเข้าห้องน้ำมีจำนวนการขับถ่ายต่อสัปดาห์มากกว่าผู้ป่วยที่ไม่มีภาวะรีบเร่งเข้าห้องน้ำเฉลี่ย 7 (4-14) vs 5(2-7) นอกจากนี้ผู้ป่วยที่มีภาวะรีบเร่งเข้าห้องน้ำมีลักษณะของอุจจาระที่เหลวมากกว่า พบว่ามีโรคประจำตัวคือลำไส้แปรปรวน ชนิดท้องเสีย และกลั้นอุจจาระไม่ได้มากกว่าผู้ป่วยที่มีไม่มีภาวะรีบเร่งเข้าห้องน้ำ ผู้ป่วยที่มีภาวะรีบเร่งเข้าห้องน้ำจำนวน 8 ราย (57.14%) ไม่มีความรู้สึกปวดอยากถ่ายอุจจาระ แต่จะมีความรู้สึกต้องรีบเข้าห้องน้ำทันทีหลังจากที่รู้สึกว่ามีอุจจาระในไส้ตรงซึ่งแตกต่างจากผู้ป่วยที่ไม่มีภาวะรีบเร่งเข้าห้องน้ำซึ่งพบเพียง 2 ราย (14.25%) และ อาสาสมัครที่ไม่มีความผิดปกติในการขับถ่ายซึ่งพบเพียง …
