วัตถุประสงค์: ภาวะตับแข็งระยะท้ายเป็นภาวะที่มีการอักเสบเพิ่มขึ้นในร่างกายเป็นปริมาณมากนำไปสู่การล้มเหลวของอวัยวะสำคัญหลายอย่าง ข้อมูลในปัจจุบันพบว่าอัลบูมินซึ่งเป็นโปรตีนที่สำคัญของร่างกาย นอกจากจะมีคุณสมบัติในการคงสารน้ำไว้ในหลอดเลือดแล้ว ยังมีคุณสมบัติในการลดการอักเสบในร่างกาย การศึกษาหลายการศึกษาก่อนหน้าพบว่า การให้อัลบูมินขนาดสูงในผู้ป่วยตับแข็งระยะท้ายสามารถทำให้สารไซโตไคน์ที่เกี่ยวข้องกับกระบวนการอักเสบลดลงได้ แสดงให้เห็นถึงความสัมพันธ์ระหว่างการให้อัลบูมินกับกระบวนการอักเสบในร่างกาย งานวิจัยนี้ต้องการศึกษาว่าผลในการลดการอักเสบของอัลบูมินนั้นมีความเกี่ยวข้องกับการเปลี่ยนแปลงของวิตามินดีซึ่งมีผลในการกระตุ้นระบบภูมิคุ้มกันหรือไม่ วิธีการวิจัย: งานวิจัยชิ้นนี้เป็นการศึกษาแบบไปข้างหน้า รวบรวมคนไข้จากคลินิกโรคตับของโรงพยาบาลจุฬาลงกรณ์ โดยมุ่งเน้นไปที่ผู้ป่วยตับแข็งระยะท้ายที่มีน้ำในช่องท้อง ผู้ป่วยที่ได้เข้าร่วมการศึกษาจะได้รับอัลบูมินทางหลอดเลือดดำในระดับสูง (80 กรัมของอัลบูมิน) ในระยะเวลา 1 สัปดาห์ โดยมีเป้าหมายดูการเปลี่ยนแปลงของระดับวิตามินดีทั้งในรูปออกฤทธิ์คือ 1,25 (OH)2 วิตามินดี และ 25 (OH) วิตามินดีก่อนและหลังได้รับอัลบูมิน รวมถึงดูการเปลี่ยนแปลงของระดับสารไซโตไคน์ที่เกี่ยวข้องกับการอักเสบก่อนและหลังได้รับอัลบูมินด้วยเช่นกัน ผลการศึกษา: ผู้ป่วยตับแข็งระยะท้ายที่มีน้ำในช่องท้องทั้งสิ้น 17 รายถูกรวบรวมเข้าสู่การศึกษานี้ สาเหตุส่วนใหญ่ที่ทำให้ผู้ป่วยเกิดตับแข็งคือภาวะตับแข็งจากแอลกอฮอล์ ตามด้วยภาวะตับอักเสบจากไขมันพอกตับ ไวรัสตับอักเสบบีเรื้อรัง และสาเหตุอื่นๆ ผู้ป่วย 10 รายมีความรุนแรงของตับแข็งในระดับ B ตามการประเมินโดยระบบ Child-Pugh score ส่วนผู้ป่วยที่เหลืออีก 7 รายเป็นตับแข็งระยะ Child-Pugh score C ค่าเฉลี่ยของ Model End-Stage Liver Disease-sodium (MELD-Na) อยู่ที่ 15.7 ± 5.11 สำหรับผลของระดับวิตามินดี พบว่าค่าของ 1,25 (OH)2 วิตามินดี ลดลงอย่างมีนัยยะสำคัญทางสถิติหลังได้รับอัลบูมิน (p = 0.011) ในขณะที่ 25 (OH) วิตามินดีก็ลดลงเช่นกันแต่ไม่มีนัยยะสำคัญทางสถิติ (p = 0.101) ส่วนระดับสารไซโตไคน์ที่เกี่ยวข้องกับการอักเสบที่สำคัญอย่างอินเตอร์ลิวคีน 1 เบต้า อินเตอร์ลิวคิน 6 ทูเมอร์เนโครสิสแฟคเตอร์อัลฟ่า และอินเตอร์เฟอร์รอนแกมม่าก็มีการลดลงอย่างมีนัยยะสำคัญทางสถิติหลังได้รับอัลบูมิน (p = 0.043, 0.037, 0.005 และ 0.02 ตามลำดับ) อย่างไรก็ตามเมื่อนำมาวิเคราะห์แบบสหสัมพันธ์พบว่าไม่พบความสัมพันธ์อย่างมีนัยยะสำคัญทางสถิติระหว่างการเปลี่ยนแปลงของระดับอัลบูมินและการเปลี่ยนแปลงของระดับวิตามินดีทั้ง 1,25 (OH)2 วิตามินดี และ 25 (OH) วิตามินดีรวมไปถึงการเปลี่ยนแปลงของระดับสารอักเสบไซโตไคน์ สรุป: การศึกษานี้แสดงให้เห็นถึงการลดลงอย่างมีนัยยะสำคัญทางสถิติของวิตามินดีในรูปออกฤทธิ์คือ …

Introduction: Falls are the leading cause of injury in older adults, resulting in worsening comorbidities, bone fractures, post-fall syndrome, and soft tissue injuries. This retrospective study aims to investigate the association between fall risk and the Montreal Cognitive Assessment (MoCA) scores, as well as other risk factors, among older adults in Thailand. การศึกษานี้มีวัตถุประสงค์เพื่อพัฒนาและประเมินคุณภาพแพลตฟอร์มประเมินการเกิดหลอดเลือดดำส่วนปลายอักเสบด้วยระบบปรึกษาการแพทย์ทางไกล รวมถึงศึกษาผลของการใช้เทปทรานสพอร์และแผ่นฟิล์มใสปิดแผลต่ออุบัติการณ์การเกิดหลอดเลือดดำส่วนปลายอักเสบ การวิจัยครั้งนี้แบ่งการศึกษาออกเป็น 3 ระยะ ได้แก่ ระยะที่ 1 การพัฒนาแพลตฟอร์ม กลุ่มตัวอย่างที่ใช้ในการวิจัยแบ่งออกเป็น 3 กลุ่ม ได้แก่ 1) พยาบาลผู้ให้สารน้ำจำนวน 30 คน 2) พยาบาลผู้เชี่ยวชาญที่ด้านการประเมินการเกิดหลอดเลือดดำส่วนปลายอักเสบจำนวน 4 คน และ 3) ผู้ป่วยที่ได้รับ การสอดใส่สายสวนหลอดเลือดดำส่วนปลายจำนวน 246 คน ระยะที่ 2 การประเมินคุณภาพแพลตฟอร์มประเมินการเกิดหลอดเลือดดำส่วนปลายอักเสบด้วยระบบปรึกษาการแพทย์ทางไกล เก็บข้อมูลด้วยการสนทนากลุ่ม กลุ่มตัวอย่างแบ่งออกเป็น 2 กลุ่ม ประกอบด้วย 1) พยาบาลวิชาชีพที่ได้จากการเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจงจำนวน 8 คน 2) พยาบาลผู้เชี่ยวชาญที่ได้จากการเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจงจำนวน 4 คน วิเคราะห์ข้อมูลเชิงคุณภาพโดยใช้เทคนิคการวิเคราะห์เนื้อหา และระยะที่ 3 การศึกษาผลของการใช้เทป ทรานสพอร์และแผ่นฟิล์มใสปิดแผลต่ออุบัติการณ์การเกิดหลอดเลือดดำส่วนปลายอักเสบ เป็นการวิจัยเชิงทดลองแบบสุ่ม กลุ่มตัวอย่างทั้งสิ้น 308 คน สรุปผลวิจัยได้ดังนี้ 1. แพลตฟอร์มประเมินการเกิดหลอดเลือดดำส่วนปลายอักเสบด้วยระบบปรึกษาการแพทย์ทางไกลประยุกต์เทคโนโลยีดิจิทัลเพื่อขอคำปรึกษาโดยการส่งภาพถ่ายที่สงสัยว่าจะเกิดหลอดเลือดดำส่วนปลายอักเสบและข้อมูลอาการและอาการแสดงของผู้ป่วย เพื่อให้ผู้เชี่ยวชาญเป็นผู้ให้คำปรึกษาเพื่อการวินิจฉัยและการรักษา โดยการออกแบบแผนผังการใช้งาน รูปแบบหน้าจอและการแสดงผลที่มีความสะดวกในการใช้งานทางคลินิก 2. แพลตฟอร์มประเมินการเกิดหลอดเลือดดำส่วนปลายอักเสบด้วยระบบปรึกษาการแพทย์ทางไกลที่พัฒนาขึ้นสามารถใช้ประเมินและการวินิจฉัยการเกิดหลอดเลือดดำส่วนปลายอักเสบได้ถูกต้อง มีความเป็นไปได้และเหมาะสมที่จะนำไปใช้งานจริงในคลินิก 3. อุบัติการณ์การเกิดหลอดเลือดดำส่วนปลายอักเสบในผู้ป่วยที่ใช้เทปทรานสพอร์ 77.92% ของผู้ป่วยที่ใช้แผ่นฟิล์มใสปิดแผล 66.88% มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (x2 = 4.696, p = 0.030 กลุ่มตัวอย่างที่ใช้เทปทรานสพอร์มีความเสี่ยงต่อการเกิดหลอดเลือดดำส่วนปลายอักเสบเป็น 1.16 เท่า เมื่อเทียบกับกลุ่มที่ใช้แผ่นฟิล์มใสปิดแผล (RR = 1.16, 95% CI = 0.116-0.452) การวิเคราะห์ทางสถิติแสดงให้เห็นถึงความแตกต่างที่มีนัยสำคัญทางสถิติเกี่ยวกับระยะเวลาการถอดสายสวนระหว่างทั้งสองกลุ่ม

การออกกำลังกายกล้ามเนื้อแกนกลางลำตัวเพื่อฟื้นฟูหลังผ่าตัดกระดูกสันหลังแบบแผลเล็กเป็นหนึ่งในวิธีที่ได้รับความนิยมสำหรับการลดอาการไม่พึงประสงค์ แต่จำเป็นต้องทำหลายครั้งและต่อเนื่อง ปัจจัยอื่น ๆ รบกวนจนไม่สามารถทำได้ครบจำนวน จึงมีการใช้ระบบการภาพบำบัดทางไกลเข้ามาทดแทน แต่ยังขาดการศึกษาวิจัยเกี่ยวกับรูปแบบการให้บริการ การศึกษาวิจัยนี้จึงมีวัตถุประสงค์ เพื่อศึกษาผลด้านระดับคุณภาพชีวิต ระดับความปวด ระดับความเชื่อในการหลีกเลี่ยงความกลัว ความสามารถในการปฏิบัติกิจวัตรประจำวัน และความแข็งแรงของกล้ามเนื้อแกนกลางลำตัว จากการออกกำลังกายฟื้นฟูกล้ามเนื้อแกนกลางลำตัวของผู้ป่วยหลังผ่าตัดกระดูกสันหลังแบบแผลเล็กผ่านระบบกายภาพบำบัดทางไกล เปรียบเทียบกับการฟื้นฟูที่สถานพยาบาล ศึกษาวิจัยในผู้ป่วยหลังผ่าตัดกระดูกสันหลังแบบแผลเล็กที่พักฟื้นมาแล้วอย่างน้อยสี่สัปดาห์ อายุสี่สิบถึงเจ็ดสิบปี จำนวนสามสิบแปดคน โดยแบ่งออกเป็นสองกลุ่มจำนวนเท่ากัน กลุ่มละสิบเก้าคน ได้แก่กลุ่มที่ออกกำลังกายฟื้นฟูผ่านระบบกายภาพบำบัดทางไกลหรือกลุ่มทดลอง และกลุ่มออกกำลังฟื้นฟูที่สถานพยาบาลหรือกลุ่มควบคุม ทั้งสองกลุ่มการศึกษาวิจัย จะได้รับการสอนออกกำลังกายเพื่อฟื้นฟูกล้ามเนื้อแกนกลางลำตัว สองครั้งต่อสัปดาห์ เป็นเวลาหกสัปดาห์ ผลการศึกษาวิจัยพบว่า ผู้เข้าร่วมวิจัยออกกำลังกายฟื้นฟูกล้ามเนื้อแกนกลางลำตัวครบสิบสองครั้ง ทั้งกลุ่มใช้ระบบกายภาพบำบัดทางไกลและกลุ่มออกกำลังกายฟื้นฟูที่สถานพยาบาล ผลจากรายงานผู้เข้าร่วมวิจัยพบว่า ทั้งสองกลุ่มการศึกษาไม่แตกต่างกัน ในด้านระดับคุณภาพชีวิต ระดับความปวด ความสามารถในการปฏิบัติกิจวัตรประจำวัน และความแข็งแรงของกล้ามเนื้อแกนกลางลำตัว สรุปผลการศึกษาวิจัยคือ การออกกำลังกายฟื้นฟูกล้ามเนื้อแกนกลางผ่านระบบกายภาพบำบัดทางไกล มีผลไม่ด้อยไปกว่าการออกกำลังกายฟื้นฟูที่สถานพยาบาล ในผู้ป่วยหลังผ่าตัดกระดูกสันหลังแบบแผลเล็ก

Background and Objectives: Peripheral neuropathy often precedes central degeneration and motor decline in Parkinson’s disease (PD). Here, we sought to evaluate corneal nerve parameters as markers of peripheral neuropathy comparing patients with prodromal PD, PD, and healthy controls using confocal microscopy. Methods: We conducted a cross-sectional study involving 53 participants (17 prodromal PD, 18 PD, 18 healthy controls). Background: Women’s soccer leads the National Collegiate Athletic Association for having the highest number of concussions in female sports. Common symptoms of concussion can include light headedness, headaches, and dizziness, but these symptoms are not exclusive to concussions. Female athletes may experience nutritional deficits that can lead to a wide range of symptoms mimicking those commonly experienced with a concussion such as headaches, dizziness, and fatigue. Background/aim: Fecal incontinence (FI) generally occurs with anal sphincter damage caused by vaginal delivery in women, obvious FI can develop in the postmenopausal stage. OBJECTIVE: Lipid emulsion contributes to parenteral nutrition associated cholestasis (PNAC). For decades, soybean oil-based intravenous lipid emulsion (SO-ILE) was the predominant product. Recently, a multicomponent lipid emulsion containing soybean oil, medium-chain triglycerides, olive oil and fish oil (SMOF-ILE) has been used off-label in neonatal care. This study evaluates the incidence of PNAC in neonates who received SMOF-ILE or SO-ILE.

Background and study aims Concomitant hiatal hernia (HH) repair with transoral incisionless fundoplication (TIF) is a therapeutic option for patients with HH > 2 cm and gastroesophageal reflux disease (GERD). Data comparing this approach with laparoscopic Nissen fundoplication (LNF) are lacking. In osteosarcoma patients, metastasis of the primary cancer is the leading cause of death. At present, management options to prevent metastasis are limited and non-curative. In this study, we review the current state of knowledge on the molecular mechanisms of metastasis and discuss promising new therapies to combat osteosarcoma metastasis. Genomic and epigenomic changes, metabolic reprogramming, transcription factors, dysregulation of physiologic pathways, and alterations to the tumor microenvironment are some of the changes reportedly involved in the regulation of osteosarcoma metastasis. INTRODUCTION: The type of vascular access (central or peripheral) in pediatric critical care depends on several factors, including the duration of treatment, the properties of the medication (osmolarity or vesicant), and the need for central pressure monitoring. The utilization of peripheral intravascular catheters (PIVCs) has shown a notable increase in the number of patients being treated. Extended dwell or midline catheters are another peripheral access option in addition to PIVCs. However, there are currently no established guidelines on their placement.

Purpose/objective: Postoperative toxicity for esophageal cancer impacts patient quality of life and potentially overall survival (OS). We studied whether patient and toxicity parameters post-chemoradiation therapy predict for post-surgical cardiopulmonary total toxicity burden (CPTTB) and whether CPTTB was associated with short and long-term outcomes.
Materials/methods: Patients had biopsy-proven esophageal cancer treated with neoadjuvant chemoradiation and esophagectomy. CPTTB was derived from total perioperative toxicity burden (Lin et al. JCO 2020). To develop a CPTTB risk score predictive for major CPTTB, recursive partitioning analysis was used.

Despite the beneficial effects of osteopathic manipulative techniques (OMT), there is a lack of in vitro models to understand the molecular mechanisms associated with these time-tested therapies. The Myofascial Release (MFR) technique is a non-invasive approach that involves passive stretching, hold and release, of the soft tissue to achieve myofascial homeostasis. Chronic restraint stress is known to cause significant alterations of mitochondrial biology. However, its effects on effort-based behavior and the sensitivity of these effects to treatments that restore mitochondrial function have not been assessed. Cancer survivors undergone treatment face an increased risk of developing atherosclerotic cardiovascular disease (CVD), yet the underlying mechanisms remain elusive. Recent studies have revealed that chemotherapy can drive senescent cancer cells to acquire a proliferative phenotype known as senescence-associated stemness (SAS). These SAS cells exhibit enhanced growth and resistance to cancer treatment, thereby contributing to disease progression. Endothelial cell (EC) senescence has been implicated in atherosclerosis and cancer, including among cancer survivors. BACKGROUND: A growing body of evidence has shown the involvement of the kynurenine pathway (KP), the primary route of tryptophan (TRP) catabolism, in the pathophysiology of neuropsychiatric disorders.
OBJECTIVE: The study aims to provide a comprehensive and critical overview of the clinical evidence on the KP involvement in the pathophysiology of Alzheimer's disease (AD) and Parkinson's disease (PD), discussing therapeutic opportunities.
