Circadian disruption caused by aging, genetic, environmental, and lifestyle factors can lead to compromised cardiac and skeletal muscle function and potentially accelerate the progression of metabolic diseases. Time-restricted feeding/eating (TRF/TRE, referred to as TRF hereafter), a dietary intervention that confines food intake within a consistent window of the day, has pleiotropic benefits from flies and mice to humans. However, the effectiveness of TRF on heart and muscle in the context of various metabolic challenges such as circadian disruption and obesity is largely unknown. The neurodegenerative disease field has enjoyed extremely limited success in the development of effective therapeutics. One potential reason is the lack of disease models that yield accurate predictions and optimal therapeutic targets. Standard clinical trials have pre-determined a single treatment modality, which may be unrelated to the primary drivers of neurodegeneration. Recent proof-of-concept clinical trials using a precision medicine approach suggest a new model of Alzheimer’s disease (AD) as a chronic innate encephalitis that creates a network insufficiency. Identifying and addressing the multiple potential contributors to cognitive decline for each patient may represent a more effective strategy. Here we review …

Background: Sleep and rest play an influential role in promoting recovery and healing in humans. Hospitalized patients are at risk for altered sleep from hospitalization, illness, and stimulation from a hospital environment. Non-pharmacologic interventions preformed by nurses can help to improve sleep and sleep environment for patients.
Local problem: There was no protocol or available information regarding patient sleep promotion for nurses to references when caring for patients.

PURPOSE: Delivering cancer care by high-functioning multidisciplinary teams promises to address care fragmentation, which threatens care quality, affects patient outcomes, and strains the oncology workforce. We assessed whether the 4R Oncology model for team-based interdependent care delivery and patient self-management affected team functioning in a large community-based health system.
METHODS: 4R was deployed at four locations in breast and lung cancers and assessed along four characteristics of high-functioning teams: recognition as a team internally and externally; commitment to an explicit shared goal; enablement of interdependent work to achieve the goal; and engagement in regular reflection to adapt objectives and processes. …

Background: New-onset diabetes (NOD) has been suggested as an early indicator of pancreatic cancer. However, the definition of NOD by the American Diabetes Association requires 2 simultaneous or consecutive elevated glycemic measures. We aimed to apply a machine-learning approach using electronic health records to predict the risk in patients with recent-onset hyperglycemia.
Materials and methods: In this retrospective cohort study, health plan enrollees 50 to 84 years of age who had an elevated (6.5%+) glycated hemoglobin (HbA1c) tested in January 2010 to September 2018 with recent-onset hyperglycemia were identified. Space travel has been associated with musculoskeletal pain, yet little is known about the nociceptive changes and pain experience during spaceflight. This preliminary study aims to investigate the pain experience and sensory alterations in astronauts following a 17-day mission to the International Space Station (ISS) on Axiom Space's AX-1 commercial space flight. Two participants were enrolled, and data were collected pre-flight, in-flight, post-flight, and three-month post-flight. Validated pain questionnaires assessed anxiety, catastrophizing, impact on physical and mental health, disability, and overall pain experience. Qualitative interviews were conducted post-landing and conditioned pain modulation (CPM) and quantitative sensory testing (QST) were performed. …

Laboratory and animal research support a protective role for vitamin D in breast carcinogenesis, but epidemiologic studies have been inconclusive. To examine comprehensively the relationship of circulating 25-hydroxyvitamin D [25(OH)D] to subsequent breast cancer incidence, we harmonized and pooled participant-level data from 10 U.S. and 7 European prospective cohorts. Included were 10,484 invasive breast cancer cases and 12,953 matched controls. Median age (interdecile range) was 57 (42-68) years at blood collection and 63 (49-75) years at breast cancer diagnosis. Although targeting oxidative phosphorylation (OXPHOS) is a rational anticancer strategy, clinical benefit with OXPHOS inhibitors has yet to be achieved. Here we advanced IACS-010759, a highly potent and selective small-molecule complex I inhibitor, into two dose-escalation phase I trials in patients with relapsed/refractory acute myeloid leukemia (NCT02882321, n = 17) and advanced solid tumors (NCT03291938, n = 23). The primary endpoints were safety, tolerability, maximum tolerated dose and recommended phase 2 dose (RP2D) of IACS-010759. The PK, PD, and preliminary antitumor activities of IACS-010759 in patients were also evaluated as secondary endpoints in both clinical trials. IACS-010759 had a narrow …

Purpose: Primary liver cancer (PLC) is a common cancer of the digestive system. Patients with PLC often experience a heavy symptom burden and along with a significant levels of anxiety and depression after liver resection. High levels of symptom burden can lead to increased anxiety and depression, whereas high levels of resilience can alleviate these conditions. Therefore, we aimed to explore the relationships among symptom burden, resilience, and anxiety/depression in Chinese patients with PLC after liver resection and to determine whether resilience mediates the relationship between symptom burden and anxiety/depression.

Background/objectives: Current treatments for chronic pancreatitis focus on symptom management and therapeutics targeting disease reversal are lacking. Given the role of the cyclooxygenase-2 (COX-2) enzyme in producing prostaglandin E2 (PGE2), a key component in the inflammatory pathway of chronic pancreatitis, this study evaluates the physiologic effect of oral indomethacin, a COX-2 inhibitor, on PGE2 levels in pancreatic fluid.

OBJECTIVES: To investigate whether and how inflammatory disease in the intestine influences the development of arthritis, considering that organ-to-organ communication is associated with many physiological and pathological events.
METHODS: First, mice were given drinking water containing dextran sodium sulfate (DSS) and then subjected to inflammatory arthritis. We compared the phenotypic symptoms between the cohoused and separately-housed mice. Next, donor mice were divided into DSS-treated and untreated groups and then cohoused with recipient mice. Sickness behavior was conceptualized initially as the behavioral counterpart of the fever response to infectious pathogens. It helps to raise body temperature to its higher setpoint and to maintain it at this new level and it has the additional benefit of enabling a weakened organism to protect itself from other dangers. The discovery of the behavioral effects of proinflammatory cytokines produced by activated immune cells provided a cellular and molecular basis to this phenomenon. Urinary stone disease is mostly formed in the kidney and is highly recurrent. The most common type of urinary stones is calcium oxalate (CaOx). The development of urinary screening tests for estimating risk of CaOx stone formation is still challenging. Recently, we developed a urinary test, called indole-reacted calcium oxalate crystallization index (iCOCI) test, that provided a high diagnostic accuracy for CaOx urolithiasis. Human corneal endothelial cells (hCECs) are vital for maintaining corneal transparency. However, their limited capacity for proliferation can result in vision loss, requiring corneal transplantation. The Leucine-rich repeat-containing G protein-coupled receptor 5 (LGR5) plays a critical role in maintaining various fetal and adult stem cell types by promoting the Wnt/β-catenin signaling. Although LGR5 is present in corneal endothelial progenitors, its expression decreases as these cells mature, coinciding with the loss of replicative properties. Background: Prolonged exercise can result in splanchnic hypoperfusion and elevated core body temperature (Tcore), which can contribute to gastrointestinal injury and endotoxemia. The effects of fluid temperature and carbohydrate consumption during exercise on these variables are less clear. การศึกษาวิจัยในครั้งนี้เป็นการศึกษารูปแบบตามรุ่นย้อนหลัง (retrospective cohort study) มีวัตถุประสงค์เพื่อศึกษาความสัมพันธ์ระหว่างการทำงานกะทั้งสถานะการทำงาน ความถี่และระยะเวลาการทำงานกะกับอัตราอุบัติการณ์ของเอนไซม์ตับผิดปกติและโรคตับคั่งไขมันที่ไม่ได้เกิดจากแอลกอฮอล์ ในบุคลากร 2 องค์กรขนาดใหญ่ในกรุงเทพมหานคร กลุ่มตัวอย่างที่เป็นบุคลากรในสังกัดสภากาชาดไทยและจุฬาลงกรณ์มหาวิทยาลัย มีผลการตรวจสุขภาพประจำปี และมีข้อมูลส่วนสูง น้ำหนัก เส้นรอบเอว ค่าเอนไซม์ตับและระดับไขมันไตรกลีเซอไรด์ในเลือดอย่างน้อย 2 ครั้งระหว่างปีพ.ศ. 2552-2559 ผ่านเกณฑ์การคัดเข้าสู่การศึกษาทั้งหมด 6,459 คน และมีกลุ่มตัวอย่างบางส่วนถูกคัดออกเนื่องจากมีค่าตัวแปรตามที่สนใจ ได้แก่ ค่าเอนไซม์ตับและ/หรือคะแนนวินิจฉัยโรคตับคั่งไขมันผิดปกติ ณ จุดเริ่มต้นของการศึกษา ข้อมูลประเมินการสัมผัสกับงานกะใช้ข้อมูลจากฐานข้อมูลแบบสอบถามซึ่งรวบรวม ณ ปี พ.ศ. 2559 และใช้ผลการตรวจสุขภาพประจำปี ได้แก่ ค่าตัวชี้วัดทางร่างกาย (ดัชนีมวลกาย หรือเส้นรอบเอว) ค่าเอนไซม์ตับ และระดับไขมันไตรกลีเซอไรด์ในเลือดในการคำนวณคะแนนวินิจฉัยโรคตับคั่งไขมันที่ไมได้เกิดจากแอลกอฮอล์ (lipid accumulation product: LAP; hepatic steatosis index: HSI) ใช้สถิติ Cox’s proportional hazard ในการประมาณค่าความเสี่ยงต่อการเกิดความผิดปกติของตับ (Hazard ratios: HR) และ 95% CI การศึกษาพบว่า เมื่อควบคุมปัจจัยกวนทั้งหมด สถานะการทำงานกะไม่มีความสัมพันธ์ต่อการเกิดความผิดปกติของตับ ทั้งค่าเอนไซม์ตับและคะแนนวินิจฉัยโรคตับคั่งไขมันที่ไม่ได้เกิดจากแอลกอฮอล์ เมื่อพิจารณาในกลุ่มคนทำงานกะไม่พบความสัมพันธ์ระหว่างขนาดการสัมผัสของความถี่ในการทำงานกะและผลกระทบต่อความผิดปกติของตับ แต่เมื่อพิจารณาเวลาในการทำงานกะ พบว่าบุคลากรทางการแพทย์ที่ทำงานกะเป็นระยะเวลาไม่เกิน 16 ปี เพิ่มความเสี่ยงต่อการเกิดโรคตับคั่งไขมันที่ไม่ได้เกิดจากแอลกอฮอล์ซึ่งวินิจฉัยด้วยคะแนน LAP เป็น 2 เท่าของบุคลากรที่ไม่ได้ทำงานกะ (adjusted HR: 2.00, 95% CI: 1.34, 2.97) ส่วนบุคลากรทางการแพทย์ที่ทำงานกะมากกว่า 25 ปี ลดความเสี่ยงต่อการเกิดความผิดปกติดังกล่าว (adjusted HR: 0.60, 95% CI: 0.40, 0.89) การศึกษานี้แสดงให้เห็นว่าระยะเวลาการทำงานกะมีความสัมพันธ์กับการเกิดความผิดปกติโรคตับคั่งไขมันที่ไม่ได้เกิดจากแอลกอฮอล์โดยเฉพาะอย่างยิ่งในกลุ่มบุคลากรทางการแพทย์

โรคกระดูกสันหลังส่วนเอวเสื่อม (lumbar degenerative disease, LDD) เป็นสาเหตุสำคัญที่ทำให้ผู้สูงอายุไม่สามารถทำกิจกรรมต่าง ๆ ได้ตามปกติจึงส่งผลต่อการดำเนินชีวิตประจำวัน ในปัจจุบันการวินิจฉัย ตรวจติดตามและรักษาโรค อาศัยภาพถ่ายคลื่นแม่เหล็กไฟฟ้า (magnetic resonance imaging, MRI) เพื่อช่วยตรวจหมอนรองกระดูกสันหลังเสื่อม การรักษาประกอบด้วยการให้ยาไปจนถึงการผ่าตัด ซึ่งทำให้เกิดบาดแผลและใช้เวลาในการฟื้นฟู โรคกระดูกสันหลังเสื่อมมีส่วนเกี่ยวข้องกับวิตามินดีที่มีบทบาทเสริมสร้างความแข็งแรงให้กับกระดูก ดังนั้นในงานวิจัยนี้จึงมีความสนใจศึกษาความสัมพันธ์ของระดับวิตามินดี การแสดงออกยีนและโปรตีนตัวรับวิตามินดี (vitamin D receptor, VDR) และระดับการเติมหมู่เมทิลของ VDR (VDR promoter methylation) ในเซลล์เม็ดเลือดขาวและเนื้อเยื่อ nucleus pulposus (NP) จากในผู้ป่วย LDD จำนวน 35 ราย เทียบกับกลุ่มควบคุม จำนวน 15 ราย จากการวิจัยพบว่ากลุ่ม LDD มีภาวะการทำกิจกรรมถูกจำกัด (40.00 %) จากการประเมิน Oswestry disability index (ODI) มากกว่ากลุ่มควบคุม (13.33 %) อย่างมีนัยสำคัญ (P < 0.001) และกลุ่ม LDD มีระดับ serum vitamin D เท่ากับ 24.30 ng/mL สูงกว่ากลุ่มควบคุม (16.20 ng/mL) อย่างมีนัยสำคัญ (P = 0.039) ส่วนระดับการแสดงออกของยีน VDR ทั้งในเซลล์เม็ดเลือดขาวและเนื้อเยื่อ NP ของทั้งสองกลุ่มไม่แตกต่างกัน แต่ระดับของโปรตีน VDR ในเลือดนั้น พบว่ากลุ่มควบคุมมีโปรตีน VDR เท่ากับ 53.28 ng/mg protein ต่ำกว่ากลุ่ม LDD ที่มีระดับการแสดงออกเท่ากับ 64.60 ng/mg protein อย่างมีนัยสำคัญ (P = 0.02) นอกจากนี้กลุ่ม LDD มีระดับ VDR promoter methylation เท่ากับ 22.02 % ในขณะที่กลุ่มควบคุมมีเพียง 14.18 % ซึ่งกลุ่ม LDD มีการเติมหมู่เมทิลสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญ (P < 0.001) นอกจากนี้เมื่อศึกษาความสัมพันธ์ของระดับความรุนแรงของโรคที่แบ่งตาม Pfirrmann grade พบว่ามีความสัมพันธ์กันแปรผันกับระดับวิตามินดี (r = 0.588, P < 0.001) และมีความสัมพันธ์แปรผันกับ VDR promoter methylation (r = 0.377, P = 0.026) อีกทั้งพบความสัมพันธ์แปรผันของ VDR mRNA ในเซลล์เม็ดเลือดขาวกับระดับโปรตีน VDR ใน serum (r = 0.420, P = 0.012) นอกจากนี้ได้พบความสัมพันธ์แปรผกผันของระดับวิตามินดีกับ ODI ของผู้ป่วย LDD (r = -0.359, P = 0.034) และความสัมพันธ์แปรผันของ VDR promoter methylation กับดัชนีมวลกาย (r = 0.348, P = 0.041) ดังนั้นสรุปได้ว่าระดับ VDR promoter methylation อาจนำมาประยุกต์และพัฒนาเพื่อนำไปใช้เป็นตัวบ่งชี้ทางชีวภาพในการพยากรณ์ระดับความรุนแรงของโรคได้ในอนาคต

วัตถุประสงค์ เพื่อศึกษาความรู้ ทัศนคติและการปฏิบัติที่เกี่ยวข้องกับการเจ็บป่วยจากความร้อนในคนประจำเรือสินค้าและปัจจัยที่สัมพันธ์กับการปฏิบัติที่เกี่ยวข้องกับการเจ็บป่วยจากความร้อนในคนประจำเรือสินค้า วิธีการศึกษา การศึกษาเชิงพรรณนาภาคตัดขวาง ณ จุดใดเวลาหนึ่ง ในคนประจำเรือไทยที่ทำงานในบริษัทเดินเรือสินค้าในกรุงเทพมหานคร จำนวน 246 คน โดยส่งแบบสอบถามออนไลน์ชนิดตอบด้วยตนเองไปยังอีเมลล์ของคนประจำเรือไทย ซึ่งประกอบด้วยข้อมูลปัจจัยด้านบุคคล ปัจจัยด้านงาน ด้านความรู้ ทัศนคติ และการปฏิบัติที่เกี่ยวกับการเจ็บป่วยจากความร้อนในการทำงานบนเรือ ผลการศึกษา ความรู้และทัศนคติส่งผลต่อการปฏิบัติฯ โดยทัศนคติและความรู้ที่ดีจะมีการปฏิบัติฯที่ดี อย่างไรก็ตาม ทัศนคติมีความสัมพันธ์เชิงบวกกับการปฏิบัติฯ มากกว่าความรู้ และพบว่ามากกว่าครึ่งหนึ่งของกลุ่มตัวอย่างมีความรู้ ทัศนคติ และการปฏิบัติฯ อยู่ในระดับน้อยถึงปานกลาง โดยปัจจัยที่สัมพันธ์กับระดับการปฏิบัติฯ อย่างมีนัยสำคัญทางสถิติ ได้แก่ ระดับการศึกษา กลุ่มตำแหน่งหน้าที่ ประเภทเรือ ลักษณะเวลาการทำงาน และ การทำงานที่ต้องสัมผัสกับแหล่งกำเนิดความร้อนอื่น และยังพบว่า ระยะเวลาการทำงานที่ต้องสัมผัสแหล่งกำเนิดความร้อนอื่นที่มากขึ้น สัมพันธ์กับคะแนนการปฏิบัติฯที่ลดลง สรุป ดังนั้นจึงควรมีการพัฒนา ให้ความรู้ เสริมสร้างทัศนคติ เพื่อนำไปสู่การปฏิบัติฯที่ถูกต้อง และควรมีมาตรการในการป้องกันการเจ็บป่วยจากความร้อนในคนประจำเรือสินค้าไทยในสภาวะอุณหภูมิโลกที่สูงขึ้น

วัตถุประสงค์งานวิจัย: ศึกษาความสัมพันธ์ของ พฤติกรรมสุขภาพอนามัยส่วนบุคคลโดยการประเมินจากแบบสอบถามกับภาวะ peritonitis ที่มาของงานวิจัย: การติดเชื้อเยื่อบุช่องท้อง (peritonitis) ถือว่าเป็นภาวะแทรกซ้อนของการฟอกไตทางช่องท้องและส่งผลทำให้มีการเปลี่ยนการฟอกไตไปเป็นการฟอกไตทางเส้นเลือด (Hemodialysis transfer; HD transfer) การศึกษานี้จึงมีจุดประสงค์หลักเพื่อดูความสัมพันธ์ระหว่างพฤติกรรมสุขภาพอนามัยส่วนบุคคลและผลลัพธ์ของผู้ป่วยล้างไตทางช่องท้อง วิธีการศึกษา: พฤติกรรมส่วนบุคคลถูกประเมินจากสถานพยาบาล 22 แห่งในประเทศไทยจาก Thailand and Practice Patterns Study (PDOPPS) ระหว่างปี พ.ศ.2559 ถึง ปี พ.ศ.2564 การประเมินพฤติกรรมสุขภาพอนามัยถูกประเมินจากแบบสอบถามทั้งหมด 8 ข้อ โดยแบบคำตอบคือ ใช่ และ ไม่ใช่ ซึ่งการแปลผลคะแนนสุขภาพอนามัยระดับแย่คือคะแนนรวมมากกว่าหรือเท่ากับ 3 คะแนน โดยใช้ Cox proportional hazards model regression และ Poisson regression incidence rate ratio (IRR) ในการประเมินความสัมพันธ์ของพฤติกรรมสุขภาพอนามัยและผลลัพธ์ของการล้างไตทางช่องท้องอื่น ๆ เช่น peritonitis, ระยะเวลาเปลี่ยนไปเป็นทำ HD transfer(time to HD transfer), death ผลการศึกษา: จากผู้ป่วยล้างไตทางช่องท้องจากสถานพยาบาล 22 แห่งในประเทศไทยทั้งหมด 669 คนมีรายงานพฤติกรรมสุขภาพอนามัย มีรายงานการเกิดสุขภาพอนามัยระดับแย่ร้อยละ 6 ของในผู้ป่วยล้างไตทางท้องที่ตอบแบบสอบถามทั้งหมดซึ่งเป็นสัดส่วนที่มีจำนวนมากในผู้ป่วยเบาหวานและผู้ป่วยที่มีคนดูแล ผลลัพธ์คือแบบสอบถามสุขภาพอนามัยระดับแย่สัมพันธ์กับการเกิด peritonitisและระเวลาที่สั้นลงของการเปลี่ยนไปฟอกเลือด แต่ไม่สัมพันธ์กับการเสียชีวิต ซึ่งความสัมพันธ์ดังกล่าวได้มีการจัดการกับตัวแปรที่เกี่ยวข้องเช่น อายุ เพศ ระยะเวลาการล้างไตทางช่องท้อง โรคประจำตัว สถานที่วิจัย ระดับอัลบูมิน สรุปผลการศึกษา: พฤติกรรมสุขภาพอนามัยส่วนบุคคลมีความเกี่ยวข้องกับความเสี่ยงในการเกิด peritonitis, ระยะเวลาที่เปลี่ยนไปฟอกเลือด แต่ไม่สัมพันธ์กับการเสียชีวิตแต่อย่างไรก็ตามควรมีการค้นคว้าเพิ่มเติมเพื่อประเมินประสิทธิภาพของแบบสอบถามในการวิจัยนี้

ที่มา: โรคอ้วนส่วนหนึ่งเกิดจากความผิดปกติของยีนที่เกี่ยวข้องกับระบบควบคุมน้ำหนักโดยตรงซึ่งทำให้เกิดโรคอ้วนรุนแรงตั้งแต่เด็ก การศึกษาในต่างประเทศพบความชุกของการกลายพันธ์ุของยีน MC4R บ่อยที่สุด รองมาคือยีน POMC ปัจจุบันยังไม่มีการศึกษาถึงความชุกของการกลายพันธุ์ของยีน MC4R และ POMC ในผู้ป่วยโรคอ้วนรุนแรงชาวไทยที่ได้รับการผ่าตัดกระเพาะเพื่อลดน้ำหนัก วิธีการศึกษา: ผู้ป่วยโรคอ้วนรุนแรงชาวไทยอายุ 18-70 ปี ที่มีค่าดัชนีมวลกายมากกว่า 30 กิโลกรัมต่อตารางเมตร และได้รับการผ่าตัดกระเพาะเพื่อลดน้ำหนักในโรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย ตั้งแต่เดือนมิถุนายน ปี พ.ศ. 2554 ถึงเดือนมิถุนายน ปี พ.ศ. 2566 ได้รับการเจาะเก็บเลือดเพื่อตรวจหาการกลายพันธุ์ของยีน MC4R และ POMC ด้วยวิธี Sanger sequencing และติดตามต่อเนื่องหลังการผ่าตัดที่ 3, 6, 12, 18, 24 เดือน และหลังจากนั้นทุก 1 ปี วัตถุประสงค์หลักของการศึกษาคือการหาความชุกของการกลายพันธุ์ของยีน MC4R และ POMC ในผู้ป่วยโรคอ้วนรุนแรงชาวไทยที่ได้รับการผ่าตัดกระเพาะเพื่อลดน้ำหนัก วัตถุประสงค์รองคือความแตกต่างของการลดลงของน้ำหนักตัวและการหายของโรคเบาหวานหลังการผ่าตัดกระเพาะเพื่อลดน้ำหนักระหว่างผู้ที่มีการกลายพันธุ์ของยีนกับผู้ที่ไม่มีการกลายพันธุ์ของยีน การศึกษานี้ลงทะเบียนในงานวิจัยทางคลินิกของประเทศไทย หมายเลข TCTR20221109004 ผลการวิจัย: ผู้ป่วยโรคอ้วนรุนแรงชาวไทยจำนวน 220 ราย ตรวจพบการกลายพันธุ์ของยีน MC4R ชนิดใหม่ในผู้ป่วย 3 ราย (ร้อยละ 1.4) โดยผู้ป่วย 1 รายพบการกลายพันธุ์ชนิดใหม่แบบ missense mutation คือ c.904T>C และผู้ป่วย 2 รายพบการกลายพันธุ์ชนิดใหม่แบบ frameshift mutation คือ c.597_599del CAT ผู้ป่วย 9 ราย (ร้อยละ 4.1) พบ Single-nucleotide polymorphisms (SNPs) ที่ยังไม่มีรายงานว่ามีความสำคัญทางคลินิก ผู้ป่วย 9 ราย (ร้อยละ 4.1) พบการกลายพันธุ์ของยีน POMC ที่ยังไม่มีรายงานว่ามีความสำคัญทางคลินิก …

ที่มา: การใช้พลังงานขณะพักและขณะทำกิจกรรมมีความแตกต่างกันตามช่วงอายุและรูปแบบการทำกิจกรรม ผู้สูงอายุมีอัตราการใช้พลังงานต่างจากช่วงอายุอื่น และผู้ป่วยโรคปอดอุดกั้นเรื้อรังมีการใช้พลังงานในการหายใจเพิ่มขึ้น การประมาณอัตราการใช้พลังงานในกลุ่มผู้สูงอายุที่ป่วยเป็นโรคปอดอุดกั้นเรื้อรังจึงมีความซับซ้อนเพิ่มขึ้น โดยเฉพาะอัตราการใช้พลังงานในขณะทำกิจกรรม และยังไม่มีการศึกษาในประชากรกลุ่มนี้ วัตถุประสงค์: เพื่อศึกษาอัตราการใช้พลังงานในการทำกิจกรรมของผู้สูงอายุที่ป่วยเป็นโรคปอดอุดกั้นเรื้อรังในภาวะอาการคงที่เปรียบเทียบกับอัตราการใช้พลังงานในผู้สูงอายุสุขภาพดี วิธีการศึกษา: เป็นการศึกษาเพื่อประเมินอัตราการใช้พลังงานขณะพักและขณะทำกิจกรรมต่างๆ 6 กิจกรรมที่มีความหนักแตกต่างกันในผู้สูงอายุที่ป่วยเป็นโรคปอดอุดกั้นเรื้อรังในภาวะคงที่และผู้สูงอายุสุขภาพดี โดยคัดเลือกอาสาสมัครที่มีอายุมากกว่า 60 ปี วัดอัตราการใช้พลังงานโดยใช้ค่า oxygen uptake (VO2) ขณะนอนนิ่งเป็นระยะเวลา 10 นาที และขณะทำกิจกรรมเป็นระยะเวลา 5 นาที ได้แก่ ยืนแกว่งแขน กวาดพื้น ยืนพับผ้า ปั่นจักรยาน เดินสายพานความเร็ว 1.5 mph, 0% grade และความเร็ว 1.8 mph, 5% grade โดยเปรียบเทียบอัตราการใช้พลังงานในแต่ละกิจกรรมของอาสาสมัครทั้งสองกลุ่ม และเปรียบเทียบกับค่าอ้างอิงมาตรฐาน ผลการศึกษา: ผู้เข้าร่วมการศึกษาทั้งหมด 55 คน เป็นผู้สูงอายุที่ป่วยเป็นโรคปอดอุดกั้นเรื้อรัง 26 ราย (เพศชาย 25 ราย คิดเป็น 96.2%) และผู้สูงอายุสุขภาพดี 29 ราย (เพศชาย 15 ราย คิดเป็น 51.7 %) มีอายุเฉลี่ย±SD 69.2±5.6 ปี และ 64.1±4.5 ปีตามลำดับ โดยมีความรุนแรงของการอุดกั้นทางเดินหายใจระดับปานกลาง (FEV1 74.5%, IQR 53-82) คิดเป็น 50% พบว่าอัตราการใช้ออกซิเจนในการทำกิจกรรมแกว่งแขนของผู้สูงอายุที่ป่วยเป็นโรคปอดอุดกั้นเรื้อรังมีค่าเฉลี่ยของอัตราการใช้พลังงานมากกว่ากลุ่มผู้สูงอายุสุขภาพดี (VO2 7.9±1.4 เทียบกับ 6.8±1 mL/kg/min ตามลำดับ, P=0.001) แต่ไม่พบความแตกต่างอย่างมีนัยสำคัญทางสถิติในขณะทำกิจกรรมอื่น สรุปผล: ผู้สูงอายุที่ป่วยเป็นโรคปอดอุดกั้นเรื้อรังมีอัตราการใช้พลังงานในกิจกรรมแกว่งแขนสูงกว่าผู้สูงอายุสุขภาพดี แต่ไม่พบความแตกต่างของอัตราการใช้พลังงานระหว่างสองกลุ่มในการทำกิจกรรมอื่น

ความสำคัญและที่มาของปัญหา : ผู้ป่วยไตวายเรื้อรังที่ต้องได้รับการฟอกเลือด พบว่าจะมีภาวะความเจ็บป่วย และอัตราการเสียชิวิตในระยะยาวมากกว่าคนปกติ เนื่องจากแม้ว่าจะได้รับการฟอกเลือดแล้ว แต่ก็ยังมีสารพิษที่จับกับโปรตีน (protein-bound uremic toxins, PUBT) และสารพิษที่มีโมเลกุลขนาดใหญ่ (large molecular weight uremic toxins, LMWT) จากวิธีมาตรฐาน (hemodialysis,HD) มาเป็นวิธีฮีโมไดอะฟิลเตรชั่น (hemodiafiltration, HDF) ซึ่งต้องอาศัยงบประมาณที่มาก นอกจากนี้ยังมีการพัฒนาตัวกรอง จากตัวกรอง high-flux มาเป็นตัวกรองที่มีขนาดรูกรองใหญ่พิเศษ (super high-flux dialyzers, SHF) แต่พบว่าไม่มีความแตกต่างกันในการกำจัดสารพิษข้างต้น จึงเป็นที่มาของการศึกษานี้ว่าการนำตัวกรอง SHF มาประกอบกับการฟอกเลือดด้วยวิธี HDF สามารถกำจัดสารพิษเหล่านี้ได้มากขึ้นหรือไม่ เปรียบเทียบกับการฟอกเลือดด้วยวิธี HD ระเบียบวิธีวิจัย : การศึกษานี้เป็นการศึกษาแบบสุ่มและมีกลุ่มควบคุม ในผู้ป่วยไตวายระยะสุดท้ายที่ปัสสาวะน้อยกว่า 100 มิลลิลิตร จำนวน 14 ราย และได้รับการฟอกเลือดด้วยเครื่องไตเทียมโดยวิธี HDF และ HD โดยใช้ตัวกรองรูใหญ่พิเศษ ELISIO-21HX เป็นระยะเวลา 8 สัปดาห์ และได้รับการฟอกเลือดด้วยวิธีเดิมก่อนเข้าวิจัยเป็นระยะเวลา 4 สัปดาห์ จากนั้นสลับวิธีการฟอกเลือดเป็นระยะเวลา 8 สัปดาห์เช่นกัน เปรียบเทียบความสามารถในการกำจัด PBUT (free and total indoxyl sulfate, IS) and LMWT (β2-microglobulin, kappa and lambda free light chains) รวมถึงติดตามระดับอัลบูมินในเลือด และวัดการสูญเสียอัลบูมินในน้ำยาไตเทียม มีการติดตามภาวะโภชนาการ มวลกล้ามเนื้อ และสำรวจคุณภาพชีวิตระหว่างการศึกษาวิจัย ผลการศึกษา : การฟอกเลือดด้วยวิธี HD และ HDF ด้วยตัวกรอง SHF กำจัด free และ total IS ได้ไม่แตกต่างกัน …

Sleep apnea is prevalent in end-stage kidney disease (ESKD) patients undergoing peritoneal dialysis (PD). Fluid overload and upper airway edema are suspected contributors, but the impact of fluid balance control on sleep apnea remains unexplored. We conducted a pre- and post-intervention study on 14 stable PD patients with volume overload. All underwent overnight polysomnography (PSG) at baseline. A fluid balance control intervention was implemented. Successful intervention was defined as maintaining actual body weight within 1.5 kg of estimated dry weight and reducing overhydration fluid. การศึกษาในอดีตพบว่าการบริโภคคาร์โบไฮเดรตย่อยยากในผู้ป่วยโรคลำไส้แปรปรวนร่วมกับกรดไหลย้อนมีผลทำให้การคลายทำของหูรูดหลอดอาหารส่วนปลายมากขึ้นซึ่งเป็นกลไกหนึ่งของโรคกรดไหลย้อน และทำให้โรคกรดไหลย้อนมีอาการมากขึ้น ทางผู้วิจัยต้องการศึกษาประสิทธิภาพของการแนะนำการรับประทานอาหารคาร์โบไฮเดรตชนิดย่อยง่ายในผู้ป่วยโรคกรดไหลย้อน วิธีการดำเนินการวิจัย: ผู้ป่วยที่มีอาการจำเพาะต่อโรคกรดไหลย้อนอันได้แก่แสบร้อนหน้าอกหรือเรอเปรี้ยวที่มีความรุนแรงและความถี่ของตัวโรคผ่านการตอบแบบสอบถาม Reflux Disease Questionnaire (RDQ) มีคะแนนมากกว่าหรือเท่ากับ 3 หลังจากที่หยุดยาลดกรดชนิด Proton pump inhibitor เป็นเวลาอย่างน้อย 2 สัปดาห์ร่วมกับรับประทานอาหารคาร์โบไฮเดรตย่อยยากมากกกว่า 7หน่วยต่อสัปดาห์จะได้รับการแบ่งกลุ่มแบบสุ่มเป็น 2กลุ่ม ได้แก่ กลุ่มการแนะนำและช่วยปรับการบริโภคคาร์โบไฮเดรตชนิดย่อยง่ายตามแต่ละบุคคล(Structural Individual Low FODMAP Dietary Advice (SILFD+)) และ กลุ่มได้รับการแนะนำอาหารแบบดั้งเดิม Usual dietary advice (Usual advice) โดยจะคัดผู้ป่วยผู้ป่วยโรคลำไส้แปรปรวนร่วมออก ทั้งสองกลุ่มจะได้รับคำแนะนำตามมาตรฐานได้แก่ ลดน้ำหนักกรณีที่มีน้ำหนักเกิน หลีกเลี่ยงกับรับประทานอาหารมื้อดึก หลีกเลี่ยงการรับประทานอาหารที่กระตุ้นให้เกิดอาการ งดสูบบุหรี่ และนอนยกศีรษะสูงกรณีที่มีอาการช่วงเวลากลางคืน สำหรับในกลุ่ม SILFD+ จะได้รับคำแนะนำเพิ่มเติม ได้แก่ 1) ตรวจสอบรายการอาหารในช่วง 1สัปดาห์ก่อนเข้าร่วมการศึกษา 2) ตรวจสอบว่ามีรายการใดบ้างที่เป็นคาร์โบไฮเดรตชนิดย่อยยาก 3) แนะนำให้เปลี่ยนรายการคาร์โบไฮเดรตชนิดย่อยยากให้เป็นคาร์โบไฮเดรตชนิดย่อยง่าย จากนั้นติดตามการรักษาที่ 4สัปดาห์ เพื่อเปรียบเทียบทั้งสองกลุ่มโดยประเมินจาก ผู้ป่วยที่ตอบสนองต่อการรักษา (โดยกำหนดนิยามคือ มีความรุนแรงและความถี่ของตัวโรคผ่านการตอบแบบสอบถาม Reflux Disease Questionnaire (RDQ) มีคะแนนน้อยกว่า 3) คุณภาพชีวิตของผู้ป่วยผ่านแบบสอบถาม Gastrointestinal Quality of Life Index (GIQLI) และพารามิเตอร์จากการวัดความเป็นกรดและความต่างศักย์ที่หลอดอาหารส่วนปลายในช่วง 2ชั่วโมงหลังรับประทานอาหารเที่ยง ผลการศึกษา: ผู้ป่วยทั้งหมด 55ราย (ค่ามัธยฐานอายุ 52 (37-61) ปี เป็นผู้หญิง 41 ราย) เป็นกลุ่ม SILFD+ 29 ราย และกลุ่ม Usual advice 26 ราย ทั้งสองกลุ่มมีพื้นฐานความรุนแรงและความถี่ของโรคกรดไหลย้อน ปริมาณหน่วยคาร์โบไฮเดรตย่อยยากเท่ากัน หลังครบ 4สัปดาห์ผู้ป่วยทั้งสองกลุ่มมีอัตราส่วนของผู้ป่วยที่ตอบสนองต่อการรักษาของทั้งสองกลุ่มไม่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ(27.6% vs. 19.2%, p=0.47) …
