Objective:

Through geocoding the physical residential address included in the electronic medical record to the census tract level, we present a novel model for concomitant examination of individual patient-related and residential context-related factors that are associated with patient-reported experience scores.

We investigate how distinct residential environments uniquely influence chronic child disease. Aggregating over 200,000 pediatric geocoded medical records to the census tract of residence and linking them to neighborhood-level measures, we use multiple data analysis techniques to assess how heterogeneous exposures of social and environmental neighborhood conditions influence an index of child chronic disease (CCD) prevalence for the neighborhood. We find there is a graded relationship between degree of overall neighborhood disadvantage and children’s chronic disease such that the highest neighborhood CCD scores reside in communities with the highest concentrated disadvantage. A neighborhood’s normative climate is linked to, but conceptually distinct from, its structural characteristics such as poverty and racial/ethnic composition. Given the deleterious consequences of early sexual activity for adolescent health and well-being, it is important to assess normative influences on youth behaviors such as sexual debut, number of sex partners, and involvement in casual sexual experiences. The current study moves beyond prior research by constructing a measure of normative climate that more fully captures neighborhood norms, and analyzing the influence of normative climate on behavior in a longitudinal framework. We advance scholarship about how macroeconomic forces differentially manifest themselves across local spaces by developing a holistic conceptual framework and empirical analyses involving multilevel change modeling. Unlike prior work, we examine differential rates of change in neighborhood indicators. We illustrate our approach with Chicago data measuring the crime, housing, and economic domains of neighborhood quality- of-life over the 2000-2009 period. We find that the local dynamic manifestations of macroeconomic cycles were far more nuanced than have been previously observed. Neighborhood indicators moved along distinct trajectories, sometimes but not necessarily tracking each other or the overall business cycle, and they changed …
