Medicare provides health care coverage for approximately 93% of non-institutionalized older adults. Compared to uninsured adults, Medicare beneficiaries have greater access to needed healthcare including preventative care. However, disparities in accessing needed health care still exist among Medicare beneficiaries. Prior research has described barriers to accessing needed health care among older Medicare beneficiaries, such as transportation and health system characteristics, but little is known about prevalence, risks, or health consequences of older Medicare beneficiaries delaying, forgoing, or having trouble accessing needed health care. To determine predictors of cervical cancer survival by socioeconomic status (SES), urbanization, race/ethnicity, comorbid conditions, and treatment among elderly Medicare-eligible women whose conditions were diagnosed with cervical cancer in a multiethnic population.

Methods: A total of 538 women with cervical cancer aged 65 years or older were identified from 1999 to 2001 from the Texas Cancer Registry and were linked with the state Medicare data and Texas Vital Records to determine survival times. All women had similar access to care through Medicare fee-for-services insurance. A composite measure of SES was created using census tract-level data as was urbanization. Treatment and …

Political Economy, Moral Economy And The Medicare Modernization Act Of 2003, Judie Svihula

Through the lens of political and moral economy, I examined the dominant values and actors in the legislative process of the Medicare Modernization Act of 2003. In my content analysis of federal hearings, I found that witnesses from government agencies, Congress and think tanks had almost equal presence at the hearings. Witnesses who were invited by Congress to testify at the hearings expressed twice as much support for private interests than for the general Medicare population or low-income beneficiaries. Few expressed concern for the uninsured population. Witnesses offered almost four times as many expressions of support for market rationalism than …

We trace key policy changes that affected use of the Medicare home health benefit from the 1980s through the prospective payment system implemented in 2000, analyzing the impact on three measures of home care use: expenditures, users and visits. We demonstrate the impact of policies generated in the legislative, the judicial, and the executive branches of government and the gaming behavior of home health agencies in response to policy changes. Our analysis suggests that the policy itself and the implementation process are critical to understanding benefit use. The Omnibus Budget Reconciliation Act of 1990 (P.L. 101-158) was the federal legislation that included the Patient Self-Determination Act, now referred to as advanced directives. This historical and policy analysis as a qualitative method of research will examine the legislative policy that was passed by congress and signed into law by the president of the United States as well as the public regulations developed by the executive branch. This law requires that patients in health care facilities receiving federal funds (Medicare and Medicaid) receive information regarding advanced directives. This article reports on a study of interstate differences in the availability of nursing home beds, personal care home slots and public housing, and attempts to assess the impact of the adoption of Medicare and Medicaid six years before on the availability of these components of what was termed the "geriatric residential environments continuum" or GRE. The underlying idea is that components of long-term health care, personal care and housing/shelter are three common elements of a wide variety of public policy for the aged.
