The deminer was investigating the signal and excavating by sitting/squatting position and his prodder/bayonet detonated the PMN mine. The evidence on site shows that the excavation of the signal started from the first mark, and not 30cm back from the indication. The bayonet directly touched the mine causing a detonation. This indicates he was not conducting operations with the correct procedure.

On 10/12/2007 as the team was busy with normal demining activities, suddenly a dog of the villagers run harshly into the minefield and as the dog reached to 6m distance from a deminer's clearance lane, suddenly an explosion happened to the dog which perished it on the spot. As a result of the explosion [the Victim] deminer who was busy on marking of his cleared lane got slight injuries on his both hands and as he was dressed fully with the PPE, he did not receive any more injuries to other parts of his body.

After the routine break at 1140 – 1150 hrs, the deminer was advised by his Section Leader (SL) to mark his lane clearly so that the SL could conduct his own QC checks. On 01 November 2007 at 09:50 am an accident happened on [the Victim] deminer of section # 2 of the mentioned team. The victim deminer was walking around and wanted to cross the boundary of the MF using unclear area that suddenly he stepped on an AP mine and caused it to explode. The accident happened during rest time of the team and the deminer was without PPE, so as a result of the accident he lost his left leg, sustained right foot ankle joint fracture and some other minor injuries to other part of his body.

On 13/12/2007 at 11:05hrs a demining accident happened on a deminer named [the Victim] as he was working on a detected signal using a pick; suddenly his pick hit top of an YM1 Iranian AP mine and caused it to explode. On 9th Oct 2007 at 08:10 Hrs, the accident happened on [Victim No.1] the Asst. Team Leader of [Demining group] EOD-4 while he was shifting UXO to pit. There were three antipersonnel PMN-2 mines near to the UXO and the Asst. MF AF/0308/01634/MF163 is located at Jan Qadam village of Bagram province. This area is contaminated by anti personnel mines laid by Russians over the period from 1982-1985. These mines have been laid as belt to secure Bagram airbase from Mujahidin attacks. The density of mine contamination in this area is very high. On 23 September 2007 while [Demining group] DT#5 was working in this MF as usual, [the Victim] one of the deminers detected a signal at 7:40 hrs and started excavation on it. While removing the excavated soil, an explosion happened and as a result he lost two fingers of right hand. On 25 August 2007 at 0905 hrs while [the Victim] a deminer of Party-01, Section-1 of the mentioned team was working in his clearance lane on a detected signal his bayonet stroke on top of an anti personnel PMN-2 mine; as a result the mine exploded and since helmet was not on his head, the victim deminer lost one eye, sustained serious injuries to other eye and some injuries to his face and hand with active bleeding.

[The Victim] deminer was working in a bushy area with MIL-D1 mine detector and after he pinpointed a signal he put the detector away and started excavation of the signal by using a pick, and within a few minutes the pick stroke on the top of an AP mine and the explosion happened. On July 19 at 07:15 hours an accident happened to [the Victim] deminer while he was working in an area, which was prepared by [National demining agency] MDU-01.
