This dissertation is a study of the implications of racism in oral health care settings for dental care-related fear/anxiety, and dental care utilization. One in five adults in the US have experienced discrimination while receiving health care. Even though racism is the most reported type of discrimination in health care, little is known about its impact on dental outcomes. There is a paucity of prior studies measuring experiences of racism in dental settings. This study is an exploratory applied research project piloting a therapeutic assessment model and method for addressing racial trauma. Therapeutic Assessment for the Psychological Impact of Racial Encounters (TAPIRE) hopes to illuminate the impact of racial trauma, process racial identity and experience, and explore effective intervention techniques to address a client’s particular relationship with racial identity and distress. Four measures, in addition to a clinical interview, were used to assess different aspects of one’s identity structure, racist experiences, and self-treatment. Integrated primary care has been incorporated into a variety of healthcare settings. The benefits of these services are empirically supported by a plethora of studies, which highlight the integration of behavioral and physical healthcare to be beneficial for both patient and healthcare providers. Integrated care models are typically incorporated in Veterans Affairs hospitals, general primary care facilities, and community clinics. Community-based clinics, such as safety-net clinics, typically serve underrepresented populations, and research has shown several mental and physical health disparities to exist among minority populations.
