Assessing 4-H And Its Circle Of Courage In A Psychiatric Residential Treatment Facility: A Case Study, Elizabeth Steering

This 4-H case study takes place in a psychiatric residential treatment facility (PRTF) in Kentucky. The PRTF provides clinical services to youth that are not able to be safely maintained in their homes due to having demonstrated unsafe or harmful behaviors. Youth admitted to the PRTF stay for an average of three to six months while they receive intensive therapeutic and psychiatric care as well as medical treatment and public schooling. Approximately two-thirds of U.S. children and adolescents report having experienced a traumatic event. A posttraumatic stress disorder (PTSD) diagnosis requires that the youth has directly experienced a traumatic event, witnessed a traumatic event occur to others, or learned that a traumatic event has occurred to someone close. The clinician is in a diagnostic quandary, however, when the youth presents with the PTSD hallmarks of hyperarousal, avoidance, and emotional dysregulation, in absence of such life experiences. The answer to this quandary lies in an understanding of developmental trauma disorder (DTD). Clinical interventions for DTD are the focus of this capstone.

The current dissertation, based on three separate and yet closely related studies, tested the longitudinal dynamic relationships of maternal and paternal parenting factors with adolescent deviance, the direct links as well as underlying mechanisms. Study 1, based on longitudinal data collected from 570 early adolescents, part of the Brno Longitudinal Study of Youth (BLSY), used latent growth modeling and tested the development of three parenting measures, including parental solicitation, knowledge, and peer approval, and deviance during early adolescence, as well as the developmental (i.e., growth to growth) links of these three parenting measures with adolescent deviance over time. Gender presentation, appearing in a way that fits social expectations of one’s gender role, represents one of the most obvious ways in which one’s gender identity becomes salient to others. This quality is especially relevant to note given the continued controversy surrounding children’s gender role development when raised by non-heterosexual parents. Autism spectrum disorder (ASD) is a neurodevelopmental disorder characterized by social and communication deficits, as well as restricted, repetitive, and stereotyped behaviors, interests, and activities (American Psychiatric Association, 2013). Although important relationships have been shown and described among the issues of challenging behavior, parent stress, and parenting sense of competence for families of children with ASD, there is a shortage of intervention programs appropriately suited for families which target these issues. The current study examined the relation between parental empathy, parenting aggression and child maladjustment in a group of parents who perpetrated child abuse and neglect. Twenty parents who were court mandated to receive a parenting intervention program at the Nest Center for Women, Children, and Families participated in this research study. Information about parental dispositional empathy, parent-child specific empathy, parenting aggression and child internalizing and externalizing symptoms were collected through an interview with the parents. Parents in this study reported high levels of dispositional empathy, but exhibited low to moderate levels of empathy in a parent-child relationship rated by coders. …

Oppositional Defiant Disorder (ODD) is commonly conceptualized as a disorder of negative affect and low effortful control. Currently, it is unclear whether temperament and personality traits associated with negative affect and effortful control can be useful assessment tools for identifying ODD early during development. This study examined the relationship between temperament and personality traits and ODD in a clinical sample of preschoolers. Results suggest that, at this age, temperament and personality traits of negative affect and neuroticism and effortful control and conscientiousness/agreeableness are not associated with one another. To investigate whether there are different patterns of development for binge eating and purging behavior among pre-adolescent and early adolescent girls, I conducted trajectory analyses of those behaviors in 938 girls across eight waves of data from the spring of 5^th grade (the last year of elementary school) through the spring of 9^th grade (the first year of high school). Analyses revealed four separate developmental trajectories for binge eating behavior (labeled none, increasing, decreasing, and high steady) and three separate developmental trajectories for purging behavior (labeled none, dabble, and increasing). The timing of pubertal development has important mental and physical health consequences. Individuals who enter puberty off-time are at greater risk for psychological disorders, social difficulties, and physical morbidity. One variable associated with early pubertal development is marital conflict. Life History Theory proposes that marital conflict signals an unreliable environment and promotes advanced pubertal timing to enhance reproductive fitness. Such calibrations allow individuals to unconsciously invest more resources in reproduction, following a quantity over quality approach. Harm reduction strategies can mitigate against some of the deleterious effects of alcohol on families. These strategies are most feasible and cost-effective when they can be targeted at those who are most at risk. Previous studies examining the relation between parents’ alcohol use and their children’s psychological adjustment have failed to consider important contextual questions such as drinking motives.
