Treatment strategies for school age children with disorganized attachment are not well established. This population exhibits a range of difficulties in social relationships and self-regulation. Additionally, children with disorganized attachment generally rely on defense mechanisms and present with other comorbid conditions, adding to the complexity of treatment. This paper explores current research and theories about disorganized attachment and then, on the basis of evidence provided throughout this paper, proposes the concept of attunement needs in relation to working with children with disorganized attachment. Among the most traumatized youth are those placed in residential treatment centers. This population reports exposure to abuse and neglect at an alarming rate in comparison with the general population. These youth are often described as irritable and angry and characterized by antisocial, delinquent and problematic behaviors. As such, they are assigned diagnostic labels based on checklists of surface level symptoms while trauma histories and chronic stress are ignored or minimized. This study used a randomized control design with immediate treatment and wait list control conditions to evaluate the efficacy of the New Hope program, a home-based, parent-and-child therapy program that has been developed for very young children living in poverty who experienced one or more potentially traumatizing events. Within a three-phase model of treatment, the New Hope program is designed to establish safety, build the caregiver child relationship, create a nurturing environment, teach coping skills, address trauma-related thoughts and feelings, and develop prosocial skills. Cumulative childhood trauma has been associated with both symptoms of post-traumatic stress disorder and depression. However, few studies have examined these relations with normative young adult populations nor have they explored the relation between childhood adversities and cognitive distortions as an outcome variable. The current study aimed to: 1) replicate and extend research on the relations between cumulative adversity, using a broad measure of adverse childhood experiences (ACEs; Felitti et al., 1998), which assesses both maltreatment (e.g., physical, sexual, emotional abuse and neglect) and exposure to elements of household dysfunction (e.g., caregiver substance use, witnessing maternal abuse), and mental health …

The study aimed to develop a brief screening instrument to assess symptoms associated with potentially traumatic experiences (PTE) in very young children (under 6). Potential items for the Early Childhood Traumatic Stress Screen (ECTSS) were sampled from each of the major content areas implicated in trauma. The items underwent a principle component analysis, which produced a 34-item screening measure with four reliable factors and one sub-scale assessing response style. All subscales and the overall trauma composite score significantly correlated with pre-established measures of traumatic stress in very young children, and a receiver operating characteristics curve analysis identified a cut-score with …

Children in the child welfare system comprise a group characterized by their exposure to trauma via experiences of maltreatment, under circumstances presenting multiple risk factors for traumatic stress. High rates of posttraumatic stress have been observed in this population. However, there is currently no standard for the universal screening of children in child welfare for trauma exposure and traumatic stress. Extensive research suggests script-driven imagery procedures employed with traumatic event-exposed adults produce reliable reactions that map onto contemporary models of posttraumatic stress, including increased physiological (Carson et al., 2000; Orr et al., 1998; Ramón et al., 2006) and negative affective (Pitman et al., 1987) responses to trauma scripts. Therefore, such procedures can be utilized in a controlled, laboratory-based setting, supporting mechanism-oriented research designed to better understand the nature, correlates, and consequences of traumatization and posttraumatic stress symptoms (PTSS). This study examines manifestations of mental health concerns in response to Gender Based Violence (GBV) specifically rape in a Non-western post-conflict setting. The population is a sample of girls aged 13-18 years residing in an Internally Displaced Person’s camp in Northern Uganda. Through semi-structured interviews, the girls shared their experiences of Gender Based Violence. Using Interpretative Phenomenological Analysis, 30 transcripts were analyzed to explore what these girls’ experiences have been with regard to rape, mental health and cultural and contextual stressors. Findings show that the girls described experiencing symptoms similar to those outlined in the DSM. Additionally, the girls described …

Peer victimization has been shown to negatively impact youth functioning and may be especially damaging during adolescence, given the increased importance of peers. However, there is a dearth of longitudinal research examining trauma symptomatology as an outcome of peer victimization with low-income, ethnic minority adolescents. The present study investigated this relation in a predominantly African American sample of 684 students assessed at five time points between the fall of their sixth grade and seventh grade school years. Trauma experiences are, unfortunately, a common part of childhood in the United States and are connected to serious health-related concerns throughout childhood and adulthood. A primary symptom of trauma exposure and posttraumatic stress is re-experiencing, which often occurs in the form of nightmares. Though cognitive behavioral treatment (CBT) is currently the most well supported treatment model for trauma-exposure, it does not specifically address nightmares. Left untreated, trauma-related nightmares may become chronic, impairing quality and quantity of sleep, and exacerbating and perpetuating trauma symptoms. Posttraumatic stress disorder in maltreated youth is a young and burgeoning field. Lemos-Miller and Kearney (2006) were among the first to propose and test a theoretically and empirically-based model of PTSD in maltreated youth. The present study replicated and evaluated the Lemos-Miller and Kearney model within a larger and more diverse sample of maltreated adolescents. First, the Lemos-Miller and Kearney model was tested via structural equation modeling (SEM). Second, the model was evaluated across age, gender, and ethnicity. Third, trauma history, family environment (i.e., cohesion and conflict), dissociation, and trauma-related cognitions were evaluated as potential mediating variables within the model. …

This survey-based, comparative study investigated the percentage of students at a suburban Chicago therapeutic day school who meet criteria for clinically significant levels of PTSD as compared to students in a general education setting. The directional hypothesis was that students placed at therapeutic day schools have a higher prevalence of PTSD than a general population of students. The method used was a survey assessment called the Child PTSD Symptom Scale (CPSS) given to 16 students at a suburban Chicago therapeutic day school. Psychotherapists have been found to have higher rates of childhood developmental trauma when compared to non-clinicians, yet they do not report more distress. The current study added to the literature regarding the experiences of psychotherapists and explored a theoretical model integrating attachment and coping as mediators for the relationship between childhood developmental trauma and psychological distress among psychotherapists.
A total of 130 masters' level psychologists participated in this study. This study examined the symptoms of posttraumatic stress disorder (PTSD) in sexually abused children. Sixty-seven children volunteered to participate in this study. Of the 67 children, 33 (26 female and 7 male) were sexually abused and 34 (22 female and 12 male) were non-sexually abused. Several instruments were employed to assess the severity of the symptoms of PTSD including the Children's PTSD Inventory, the Child Behavior Checklist, and the Children's Depression Inventory, as well as others. The results of this study showed that sexually abused children have more symptoms of PTSD than non-sexually abused children. Early diagnosis and treatment of …
