The Massachusetts Patient Centered Medical Home Initiative (MA PCMHI) is a statewide, multi-payer demonstration that seeks to transform 46 primary care practices into PCMHs. Clinical care management of high-risk patients is a key element of the PCMH and a new service for most primary care practices. Training for practices in the MA PCMHI includes the clinical care manager (CCM) role, identification/tracking of highest-risk patients, care plan development/implementation, care coordination, and communications. Content is delivered through learning collaborative sessions, monthly webinars and practice-based transformation facilitation. Exploring Pregnant Women’S Perceptions And Decision Making Of Household Chemicals: Phthalates As A Model, Justin M. Ashley

Recent research has identified a group of plasticizers known as phthalates as potential anti-androgens and a risk to developing male fetuses. A constructivist grounded theory study was used to understand pregnant women and obstetrical care providers’ perceptions of phthalates. Twenty-three pregnant women and twelve obstetrical care providers (Obstetricians, Family Physicians, and Midwives) from Southwestern Ontario participated in semi-structured interviews. Pregnant women and clinicians had little knowledge regarding phthalates and the potential associated risks. Women felt that knowledge of these risks would be important to prenatal counselling while clinicians required more evidence. Two separate but related models emerged from the data …

Diabetes mellitus is a prevalent chronic health condition associated with significant morbidity and mortality. Those with diabetes must acquire self-efficacy in the tasks necessary for them to successfully manage their disease. In this study, a controlled pre- and post-design was used to determine the effect of an adult support and education group visit program embedded in an urban academic family medicine practice on weight and the achievement of treatment goals for hemoglobin A1C, low-density lipoprotein (LDL) blood concentration, and blood pressure (BP) several months after it was implemented. Participants: Formal and informal knowledge brokers are the targeted users of the strategies. Methods: Research studies and conceptual literature in knowledge transfer informed the development of brief-Knowledge Transfer (b-KT) principles. Principles are explained and a sample of how they informed the development of KIT-Tip Sheets is offered to promote ways to use principles in knowledge dissemination. Current literature demonstrates a significant investigation of chronic pain. The actual mechanisms that promote adaptation, accommodation, and change, however, are not explicated in the current literature on chronic pain. Our goal was to conduct a mini realist synthesis approach in current literature to identify mechanisms critical to promote return to work (RTW) for persons with chronic pain. Specifically, we sought to identify if the mechanisms of these programs are consistent with an occupation-based approach used in Occupational Therapy (OT).

Why does child malnutrition persist in India? Amongst the fastest growing economies over the last two decades, India has struggled to make progress in the health of its children. In this article the author argues that the reason malnutrition persists is not limited to poverty or inadequate access to food; but that a lack of nutritional knowledge amongst families plays a very important role.
