Diabetes mellitus is a chronic metabolic disease that affects over 34.2 million or 10.5% of the U.S. population and ranks as the seventh leading cause of death. Tennessee's diabetes prevalence rate is ranked 45th overall in the nation, with 14.9% of the population affected, and in the local area for this project, the rate is greater than 15%, creating a public health concern. Despite its prevalence, there is still a wide gap in knowledge among nurses on diabetes management. In 2010, the Institute of Medicine issued a report calling for the transformation of undergraduate nursing education to prepare new graduates for the complexities of practice. To improve preparation for practice, faculty responded by incorporating student-centered, active learning strategies (SCALS); however, the effectiveness of SCALS has not been examined. AbstractDiabetes mellitus is a complex chronic condition that requires continuous care and management. Approximately 34 million Americans are diagnosed with diabetes and around 90%–95% suffer from Type 2 diabetes. The problem identified in this project was the need to follow the American Diabetes Association guidelines in place at the study site clinic but are not adhered to by the staff. A rejected blood specimen can be very costly to a hospital as well as to the patient. Many errors occur in the preanalytical phase in which nurses and assistive personnel are often involved because they are usually not trained to do so. This staff education project was designed to decrease the number of laboratory rejections of blood specimens in a local hospital setting. Knowles’s adult learning theory was combined with the knowledge translation framework, proposed by Fredericks, Martorella, and Catallo, so that evidence-based changes in practice could occur more readily. Every year, thousands of congestive heart failure (CHF) patients are readmitted to the hospital within 30 days of discharge. There is a gap in practice in the care continuum of patients with CHF within the transition from hospital to home. One of the factors known to increase a patient's risk for readmission is the lack of patient engagement and self-efficacy regarding the treatment plan.
