A Novel Training Simulator For Portable Ultrasound Identification Of Incorrect Newborn Endotracheal Tube Placement – Observational Diagnostic Accuracy Study Protocol, Hasan S. Merali, Mark O. Tessaro, Khushboo Q. Ali, Shaun K. Morris, Sajid Bashir Soofi, Shabina Ariff

Background: Endotracheal tube (ETT) placement is a critical procedure for newborns that are unable to breathe. Inadvertent esophageal intubation can lead to oxygen deprivation and consequent permanent neurological impairment. Current standard-of-care methods to confirm ETT placement in neonates (auscultation, colorimetric capnography, and chest x-ray) are time consuming or unreliable, especially in the stressful resuscitation environment. Point-of-care ultrasound (POCUS) of the neck has recently emerged as a powerful tool for detecting esophageal ETTs. It is accurate and fast, and is also easy to learn and perform, especially on children.
Methods: This will be an observational diagnostic accuracy study consisting of two …

Methicillin Resistant Staphylococcus Aureus Outbreak In A Neonatal Intensive Care Unit, Seema Irfan, Imran Ahmed, Farida Lalani, Naureen Anjum, Nadia Mohammad, Maria Owais, Afia Zafar Oct 2019

Methicillin Resistant Staphylococcus Aureus Outbreak In A Neonatal Intensive Care Unit, Seema Irfan, Imran Ahmed, Farida Lalani, Naureen Anjum, Nadia Mohammad, Maria Owais, Afia Zafar

Department of Pathology and Laboratory Medicine

The global and national burden of communicable and noncommunicable diseases continues to rise, thus making access to Healthcare workers (HCWs) colonized with methicillin-resistant Staphylococcus aureus (MRSA) may pose transmission risk to vulnerable patients including neonates. This study reports an MRSA outbreak in a level-II neonatal intensive care unit (NICU) of a secondary care hospital in Pakistan. Once identified, an infection control team from the parent hospital visited the facility, risk factors were listed and infection control measures taken to control the outbreak. Screening cultures of NICU staff and environmental cultures from NICU were obtained for the presence of MRSA. Five …

Health Systems Changes After Decentralisation: Progress, Challenges And Dynamics In Pakistan, Shehla Zaidi, Maryam Bigdeli, Etienne V. Langlois, Atif Riaz, David W. Orr, Nasir Idrees, Jesse B. Bump Jan 2019

Decentralisation is widely practised but its scrutiny tends to focus on structural and authority changes or outcomes. Politics and process of devolution implementation needs to be better understood to evaluate how national governments use the enhanced decision space for bringing improvements in the health system and the underlying challenges faced. We use the example of Pakistan's radical, politically driven provincial devolution to analyse how national structures use decentralisation opportunities for improved health planning, spending and carrying out transformations to the health system. Our narrative draws on secondary data sources from the PRIMASYS study, supplemented with policy roundtable notes from Pakistan. …

Evaluation Of The Cost-Effectiveness Of The Treatment Of Uncomplicated Severe Acute Malnutrition By Lady Health Workers As Compared To An Outpatient Therapeutic Feeding Programme In Sindh Province, Pakistan, Eleanor Rogers, Saul Guerrero, Deepak Kumar, Sajid Bashir Soofi, Shahid Fazal, Karen Martínez, Jose Luis Alvarez Morán, Chloe Puett Jan 2019

Evaluation Of The Cost-Effectiveness Of The Treatment Of Uncomplicated Severe Acute Malnutrition By Lady Health Workers As Compared To An Outpatient Therapeutic Feeding Programme In Sindh Province, Pakistan, Eleanor Rogers, Saul Guerrero, Deepak Kumar, Sajid Bashir Soofi, Shahid Fazal, Karen Martínez, Jose Luis Alvarez Morán, Chloe Puett

Background: Due to the limited evidence of the cost-effectiveness of Community Health Workers (CHW) delivering treatment for severe acute malnutrition (SAM), there is a need to better understand the costs incurred by both implementing institutions and beneficiary households. This study assessed the costs and cost-effectiveness of treatment for cases of SAM without complications delivered by government-employed Lady Health Workers (LHWs) and complemented with non-governmental organisation (NGO) delivered outpatient facility-based care compared with NGO delivered outpatient facility-based care only alongside a two-arm randomised controlled trial conducted in Sindh Province, Pakistan.
Methods: An activity-based cost model was used, employing a societal perspective …
