Background: Assessing children's postoperative pain is complicated by the presence of a powerful emotional component. The purpose of the study: To match the safety and duration of postoperative pain relief using caudal Magnesium Sulfate and Dexmedetomidine as adjuvants with Bupivacaine 0.25percent in pediatric patients undergoing infra-umbilical surgeries.

Patients and methods: Sixty children cases with (ASA) Classes I & II, weighing up to 20 kg, were enrolled in this randomized controlled prospective trial at ALAzhar University Hospitals (Assiut). Each of the three groups consisted of twenty patients: Group A (Dexmedetomidine), Group B (Magnesium), and Group C (Control). Results: Our study showed that onset …

Soluble Anti- Erythropoietin Level As A Predictor Of Erythropoietin Resistance In Patients Under Regular Hemodialysis, Safwat Farrag Ahmed, Mohamed Farouk Ibrahim Mosa, Mahmoud Ahmed Abdelaziz Said Jan 2023

Soluble Anti- Erythropoietin Level As A Predictor Of Erythropoietin Resistance In Patients Under Regular Hemodialysis, Safwat Farrag Ahmed, Mohamed Farouk Ibrahim Mosa, Mahmoud Ahmed Abdelaziz Said

Al-Azhar International Medical Journal

Background: Anemia is multifactorial in ESRD cases. It has been shown to be primarily caused by insufficient erythropoietin (EPO) synthesis combined with erythropoietin resistance and an increased erythropoietin demand. EPO stimulates the production of red blood cells in the bone marrow. Therefore, recombinant human EPO therapy (rHuEPO) is the primary medication for ESRD-associated anemia. Objective: Determine the predictive value of anti-EPO levels in the monitoring of erythropoietin resistance in those receiving hemodialysis. Patients and methods: This Cross-Sectional Study was conducted in the hemodialysis facilities of the Department of Internal Medicine at Al-Azhar University Hospital for Boys in Cairo. During a …

Impact And Outcome Of Primary Prophylaxis Of Variceal Bleeding Among Egyptian Hepatocellular Carcinoma Patients With Portal Vein Thrombosis Treated With Sorafenib, Anwar Abdelaleem Mohamed, Amal Mohamed Mohamed Mahmoud, Mohamed Ashraf Eljaky, Rasha Abdelhafiz Aly, Mohamed Akl Rady Jan 2023

Impact And Outcome Of Primary Prophylaxis Of Variceal Bleeding Among Egyptian Hepatocellular Carcinoma Patients With Portal Vein Thrombosis Treated With Sorafenib, Anwar Abdelaleem Mohamed, Amal Mohamed Mohamed Mahmoud, Mohamed Ashraf Eljaky, Rasha Abdelhafiz Aly, Mohamed Akl Rady

Al-Azhar International Medical Journal

Background: Patients with cirrhosis and hepatocellular carcinoma (HCC) experience portal hypertension, causing esophageal varices (OV). Some HCC patients taking Sorafenib experience bleeding, while others ignore this effect and believe that sorafenib might minimize bleeding. A comparison was made between carvedilol therapy and primary preventive band ligations in HCC patients with a large, risky OV and portal vein thrombosis (PVT) taking sorafenib. Survival of these patients is also analyzed.

Methods: Study enrolled 120 HCC patients with PVT and large OV. They were divided into four groups: 1) Sorafenib with prophylactic band ligation, 2) Sorafenib with prophylactic carvedilol, 3) prophylactic band ligation, …

Comparative Study Between Transversus Abdominis Plane Block And Intravenous Patient Controlled Analgesia After Spinal Anesthesia In Elective Cesarean Sections, Abdallah Mohamed Ahmed, Essam Shafiq Mohammad Abd El Whab, Islam Ashraf Abdelaziz Mohyeldin Jan 2023

Comparative Study Between Transversus Abdominis Plane Block And Intravenous Patient Controlled Analgesia After Spinal Anesthesia In Elective Cesarean Sections, Abdallah Mohamed Ahmed, Essam Shafiq Mohammad Abd El Whab, Islam Ashraf Abdelaziz Mohyeldin

Al-Azhar International Medical Journal

Background: One of the most popular surgical procedures worldwide is the caesarean section (CS). Both the mother and the newborn experience post-operative discomfort, particularly in the first 48 hours after birth. Objective: To assess the pain-relieving effectiveness of patient-controlled analgesia (PCA) with ultrasound-guided transversus abdominis plane (TAP) block following cesarean delivery. Patients and Methods: After getting ethical permission, 50 cases were finally enrolled, and they were allocated randomly to one of two groups. In group I,cases undergo spinal anaesthetic with 2.2 ml of 0.5% bupivacaine + 25 µg fentanyl, followed by TAP block when the procedure is complete. Patients in …

Study Of Salivary Alpha-2 Macroglobulin In Type 2 Diabetic Egyptian Patients, Mahmoud Haddad Hemida, Hossam Aladl Aladl, Amr Ahmed Rezk, Ibrahim Mohamed Sami Darwish

Al-Azhar International Medical Journal

Background: Evaluating glycemic control in saliva samples in a noninvasive method is necessary. Alpha-2 Macroglobulin (A2MG) might be present in saliva; therefore, it has the potential to be employed as a noninvasive approach for tracking T2DM individuals' glycemic control. Aim and Objectives: evaluating the efficacy of salivary A2MG as a biomarker for glycemic control in Egyptians with T2DM. Patients and methods: The Internal Medicine Outpatient Clinic at Al-Azhar University Hospital in Cairo enrolled 100 participants for a Cross-Sectional Observational Research throughout September 2021 to March 2022. Results: Age, family history, hypertension, morbidities, Post-Hoc test for multiple contrasts among examined groups …
