A 38-year-old male presented to the emergency department with complaints of a painful penile ulcer for four weeks. Six months prior to presentation, the patient was diagnosed with secondary syphilis and clinically treated with Bicillin; however, RPR titers posttreatment were not ordered, as is standard of care. One week before presentation, the patient visited his primary care physician and had a urine PCR screening for sexually transmitted infection, which provided the diagnosis of Chlamydia. Over 90% of cases of cryptococcal meningoencephalitis present in immunocompromised patients, with the majority of those being in patients with AIDS. However, this infection can also occur in other immunocompromised states, such as steroid use, malignancy, rheumatologic diseases, and use of immunosuppressive medications. Delay in diagnosis can often lead to rapid neurological deterioration and mortality. We present a case of a young, otherwise immunocompetent patient, with history of Chiari I malformation and recent COVID-19 infection, found to have cryptococcal meningitis. A 35-year-old African American male with no significant past medical history presented with generalized weakness, fatigue, cough, progressive 50-pound weight loss in the last 2 months, frontal headache, neck stiffness, and blurry vision. Workup revealed fever, sinus tachycardia, positive HIV screening, and multiple electrolyte abnormalities. There were negative/insignificant results on chest, lumbar spine, and head imaging. HIV viral load was 784,000; CD4 count was 4. Blood cultures grew yeast. Lumbar puncture revealed an opening pressure of 50 mmHg, WBC 2, protein 18, glucose 22, and a CSF panel positive for cryptococcus. The patient was initiated on induction therapy with AmBisome …

Objectives: Interspinous process decompression (IPD) is a procedure that restricts lumbar spine extension through implantation of a spacer between adjacent spinous processes to reduce neurogenic claudication symptoms associated with lumbar spinal stenosis (LSS). Our study aims to determine whether a minimally invasive IPD provides relief to United States Military Veterans who have undergone the procedure over the past year. Background: Frailty is a geriatric syndrome typically marked by increased vulnerability to environmental stressors generally attributed to a diminished physiological reserve. People living with HIV (PLH) experience a precocious onset of frailty. Several biological and sociobehavioral factors have been associated with frailty in PLH. For example, we have recently reported that lifetime alcohol use increases the prevalence of frailty in PLH. The objective of this analysis was to identify potential predictors of frailty progression in PLH using a preliminary cross-sectional analysis. Introduction: Group B Streptococcus is a gram-positive beta hemolytic streptococcus species commonly colonizing the genitourinary tract of females. Although it contributes to a variety of infections, group B strep is increasing in frequency in its rare manifestation of infective endocarditis in many non-pregnant women. Our case presents an uncommon complication of a necrotic uterine leiomyoma with a concurrent rare presentation of group B Streptococcus endocarditis in a previously asymptomatic 39-year-old nulliparous female.
