Micronutrient Deficiencies In Beaumont Integrative Medicine Fatigue Patients, Jessica Dorschner, Maureen Anderson

Fatigue is a common patient concern at primary care visits and can persist after addressing etiologies such as anemia, infection, malignancy, depression and cardiopulmonary disorders. Micronutrient deficiencies have also been investigated in fatigue patients, particularly individual vitamins such as C, D, B12, E and the mineral zinc. Fewer studies have investigated broad panels of micronutrients in fatigue patients. The Beaumont Integrative Medicine Clinic evaluates many patients with fatigue and assesses micronutrient levels using SpectraCell testing which measures 31 vitamin, mineral and metabolite levels concurrently.

• Numbers have increased, with >350 asylum-seeking families (1500 individuals) in 2022 in Portland alone
• Maine Medical Center (MMC) provides obstetrical care for more newly arrived, non-English speaking patients
• Total deliveries in 2020, 2021 (6192) broke records at MMC
• New Mainers at Increased risk for health disparities due to: language barriers, low socioeconomic status and stressors from a history of trauma

The prevalence for hepatitis C during 2013-2016 was about ~2.4 million. Screening helps to prevent chronic hepatitis C or start early curative treatment. USPSTF recently changed their guidelines in 2021 for Hepatitis C screening to now include all asymptomatic patients from ages from 18-79 years old without liver disease. Screening usually includes the Hepatitis C Antibody test. Further testing includes Hepatitis C mRNA PCR if screening test is positive. High risk patients should have periodic screening. Since the change, this project was done to ensure the appropriate screening and testing thereafter was done, as well as appropriate communication and documentation. …

Introduction: Over 264,000 cases of breast cancer are diagnosed annually within the United States, with 99% of them being in women. Regarding prevention of this terrible disease, early detection of such cancers through screening mammography has up to a 20% reduction in breast cancer mortality. Therefore, the USPSTF recommends screening of women who are 50-74 years of age via mammography at average risk of breast cancer every two years. The goal of this study was to measure, track, and increase compliance with these screening recommendations within the Medical City resident clinics.

INTRODUCTION: The Patient Health Questionnaire-9 (PHQ-9) is a tool used to help diagnose depression and assess a patient’s response to treatment. Quality metric (QM) 371 is a measure of PHQ-9 utilization in patients with depression, and Medical City Family Medicine tracks QM 371 in order to improve the quality of care provided to our patients. This project sought to assess utilization of the PHQ-9 in our clinical practice. At the time of this QI project’s inception, QM 371 was reporting at 0%.

This is a unique case of a 38 year old Caucasian female with no significant medical history who presented to the hospital with complaint of 3 days of abdominal pain, nausea/vomiting, dark red formed stool, and fever (TMax 102F), as well as dizziness, body aches and throat pain. She reported eating meatloaf with her daughter two days before symptom onset, and daughter was reported to be ill as well. Upon initial exam, patient was found to have temp 100F, was hypotensive and tachycardic with lactate of 4.2. FOBT was positive. Neonatal opioid withdrawal syndrome (NOWS) is a significant public health concern, affecting infants born to mothers who use opioids during pregnancy. Those exposed in-utero exhibit numerous signs of withdrawal after delivery including tremors, gastrointestinal distress, inconsolability, and poor feeding. Typical treatment for infants suffering from NOWS included direct admission to the neonatal intensive care unit (NICU) for close observation and pharmacological interventions. However, NICU stays can interfere with bonding and breastfeeding. They can also lead to increased lengths of stay, higher costs, and more invasive pharmacological treatments. These infants typically do not have any other concerning health issues besides NOWS …

Introduction: This is the case of a patient presenting with phlegmasia cerulea dolens after recent travel with concurrent extensive pulmonary emboli. Patient’s symptoms progressed quickly, atypically, and he did not have known common risk factor for the extent of his disease.
Case Description: The patient is a 78-year-old Caucasian male who presented to the emergency department complaining of left leg swelling and mild pain that began 1 day prior. Continuity of care is an integral part of patient care and is important for reducing morbidity and mortality following hospitalizations. Medical errors are common during transitions of care, and are most frequently caused by lack of suitable discharge planning and poor communication between surgical teams, patients, and primary care physicians. This project aims to create a new standardized note template for surgery teams to use when discharging patients, with the goal of augmenting communication and efficiency during transitions of care within and outside the UVM Health Network.

Exercise and physical therapy are concerns among patients experiencing menopause and patients who near the age of menopause. Newtown Primary Care serves a population, whose census indicates an aging population. An interdisciplinary pamphlet on weight-bearing exercises and community resources for the peri- and postmenopausal population was developed with input from physical therapists and doctors.

A scoping review of COVID-19's impact on healthcare delivery and health outcomes found disruptions in care delivery and a shift towards telemedicine(1).
