Introduction: Acne is a common skin disorder with multifactorial causes that can persist beyond adolescence or appear for the first time in adulthood. Treatments of varying potency exist, ranging from over-the-counter face washes and creams to prescription topical and oral treatments. Current treatment guidelines target sebum production, exfoliation of excess skin cells, and the bacteria Propionibacterium acnes. Research as early as 1995 suggests dysfunctions of the skin barrier, such as abnormal ceramide levels and increased water loss, play a role in the pathophysiology of acne and are not addressed by current treatments. Background: Insulin resistance and high carbohydrate diets are currently considered to be influential in acne aetiology. Insulin is a hormone that does not only regulate the concentration of blood glucose but also affects the production of sebum and through the Insulin Growth Factor-1 (IGF-1) receptor stimulates androgen synthesis which will increase the proliferation of keratinocytes duct and the production of sebum in acne of the pilosebaceous.

Background: Demodicosis is a disease, caused by parasitisation of the opportunistic parasites from the acariasis group – Demodex mites. This article presents a comparative study of two methods (light microscopy of skin scrapings and confocal laser scanning in vivo microscopy) for identification of Demodex mites on the facial skin in acne and rosacea patients. The use of confocal laser scanning in vivo microscopy in dermatology today is considered as one of the most promising methods.

OBJECTIVE: The objective of this selective EBM review is to determine whether or not a once daily topical application of fixed-dose combination gel containing adapalene 0.1%-benzoyl peroxide (BPO) 2.5% (adapalene-BPO) is effective at treating mild to moderate acne.

STUDY DESIGN: Three multi-center double blind randomized control studies published in the English language between 2007-2013.

STUDY SOURCES: Three peer-reviewed RCTS were found using Pubmed. These studies compared the same fixed-dose combination gel adapalene-BPO against placebo vehicles. Two studies also compared to additional groups receiving monotherapies.

STUDY DESIGN: A review of three English language studies published in 2007. Includes two randomized, double-blind, controlled trials and one open label, noncomparative study. DATA SOURCES: Randomized, controlled trials comparing dapsone gel to vehicle gel control were found using PubMed and Cochrane database.
