Background: Studies examining HIV prevalence and risk behaviors within trans subgroups have identified them as high risk. Yet few studies have addressed how discrimination impacts this prevalence. Minority stress theory suggests that there is a relationship between minority stress and HIV-related risk behaviour. We hypothesize that multiple minority statuses may result in discriminatory experiences, specifically self-reported transphobia and racism in synchrony with other attributes, that interact to alter past-year HIV-related risk behaviour.

The development of screening approaches to identify novel affinity ligands has paved the way for a new generation of molecular targeted nanomedicines. To identify novel targeting ligands, several studies have demonstrated the advantages in screening one-bead-one-compound (OBOC) libraries. Conventional methods typically bias the display of the target protein to ligands during the screening process. We have developed an unbiased multiplex ‘beads on a bead’ strategy to isolate, characterize, and validate high affinity ligands from OBOC libraries. Breast cancer is a leading cause of death in women, due primarily to ineffective treatment of metastatic disease. Despite being a lethal process, metastasis is surprisingly inefficient, with less than 0.5% of cancer cells able to successfully navigate the metastatic cascade, indicating that only a small proportion of cancer cells possess the necessary characteristics to establish metastases. This thesis aimed to determine the psychometric properties and applications of sensory threshold tools and outcome measures in patients with Carpal Tunnel Syndrome (CTS). The first chapter is a psychometric study that defines clinically important difference (CID), construct validity and responsiveness of touch and vibration threshold tools and in the Symptoms Severity Scale (SSS). The study found the CID for the PSSD and for the SSS was 0.15g/mm2 and 0.50 respectively. Group 1 metabotropic glutamate receptors (mGluR1 and mGluR5) are G-protein coupled receptors (GPCRs) activated by glutamate. mGluR1/5 couples to Gαq/11 and releases Ca2+ from the endoplasmic reticulum. Ca2+/calmodulin-dependent protein kinase II alpha (CaMKIIα) can be activated by Gαq/11-mediated Ca2+ release through binding of Ca2+/calmodulin. Results from a proteomic screen identified CaMKII as a novel mGluR-interacting protein. Therefore, we hypothesized that CaMKIIα associates with group 1 mGluRs and this association alters mGluR1/5 signalling and internalization. Children with epilepsy are less physically and socially active than their peers. The objective is to explore whether parents represent a barrier to children’s activity, by examining associations between child and family factors and parents’ perceptions of epilepsy-related activity restrictions. Data were from the Health Related Quality of Life in Children with Epilepsy Study, a longitudinal study of children 4-12 years old with new-onset epilepsy. The vast majority of studies on attention-deficit/hyperactivity disorder (ADHD) are based on samples with inherent age, sex, and referral biases. Osteopontin (OPN) is a phosphorylated acidic glycoprotein that has been shown to play a vital role in tumourigenesis and metastases. The objective of this study was to determine the expression of OPN and its receptors (CD44 and integrin αvβ3) in several salivary gland cancers, and assess its clinical correlations.

Immunohistochemical techniques were used to semi-quantify the level of OPN expression and its receptors in normal salivary glands and different types of salivary gland tumour.

Enhancing maternal and child health comprise key Millennium Development Goals (MDGs), driving an analysis of whether efforts to improve maternal health through quality antenatal care (ANC) is associated with the recommended health-related care of children in Sub-Saharan Africa, where under-five mortality is the highest.

This study examines ANC as a potential determinant of immunization and appropriate care for fever and diarrhea in Ghanaian children. Data were pooled from the 1998, 2003, and 2008 Ghana Demographic Health Surveys, and Andersen’s Behavioural Model provided the conceptual framework.

Using the C2C12 mouse myoblast cell line, I investigated how prolonged hypoxia affected components of the insulin signalling and FAO/PGC-1α/PPARs pathways, as they might impact insulin sensitivity in skeletal muscle cells.

Hypoxia resulted in lower p-Akt (Thr 308) and higher total cellular GLUT4 protein levels after 7 days of differentiation. This coincided with higher triglyceride content and alterations of the FAO/PGC-1α/PPARs components, both of which could contribute to the changes observed in the components of the insulin signalling pathway. The role of planar cell polarity (PCP) signalling in breast cancer is unclear, as evidence shows that activation of this pathway with WNT5A can either promote or inhibit progression. Using the 21T cells, which represent distinct stages of breast cancer progression when grown in a mouse xenograft model (21PT, atypical ductal hyperplasia; 21NT ductal carcinoma in situ; 21MT-1, invasive mammary carcinoma), I hypothesized that WNT5A will promote progression only when VANGL1, another component of PCP signalling, is elevated.

Older adults have high rates of suicide, necessitating enhanced models of suicide risk in later life. Previous cross-sectional findings suggest that suicide ideation is positively associated with psychological risk factors and negatively associated with resilience factors, including recognition of meaning in life (MIL) and perceptions of social support (PSS). In the research setting, instrumented treadmills are often used to study prolonged periods of walking. This thesis examines the effects of in-shoe foot orthoses on walking gait during prolonged periods of treadmill walking. The two types of foot orthoses investigated were: 1) a pedorthist hand-made orthotic with medial longitudinal arch (MLA) support and 2) a proprioceptive feedback-type orthotic designed to stimulate the intrinsic foot muscles of the MLA. This study assessed associations between suicide ideation and personality characteristics in a sample of community-residing adults 65 years of age and older. Measures of personality, suicide ideation, and depression were administered at two time points 6-12 months apart. Multiple linear regression analyses were employed to investigate cross-sectional and longitudinal associations between personality characteristics and suicide ideation. A total of 106 older adults (mean age=74.2 years, SD=5.8, including 78 women) completed both phases of the study. The effect of the interaction between gender and posterior cruciate ligament (PCL) prosthesis design on patient-reported outcomes is an understudied area of research. We evaluated 1613 patients, from the Ontario Joint Replacement Registry (2001-2006), who underwent primary total knee replacement. This study investigated the impact of the gender-PCL design interaction on Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) change scores and patient satisfaction, by performing linear regression analysis, using full-adjusted models that also included the gender-PCL prosthesis design interaction variable. Renal ischemia-reperfusion injury (IRI) occurs following reduced renal blood flow and is a major cause of acute injury in both native and transplanted kidneys. We have previously demonstrated that NK cells can mediate tubular cell death and kidney IRI. Natural killer receptor-protein 1B (NKR-P1B) has been shown to interact with C-type lectin-related protein B (Clr-b), resulting in the suppression of NK cell-mediated cytotoxicity.

Clr-b mRNA and protein expression in the kidney were up-regulated after renal IRI. Similar upregulation of Clr-b expression was seen in cytokine-challenged primary-cultured tubular epithelial cells (TEC). Furthermore, NK cytotoxicity assays demonstrated enhanced necrotic death in TEC …

A growing body of evidence suggests that stress-related factors may contribute to the initiation, development and progression of breast cancer. The disruption of myocardial extracellular matrix (ECM) has been implicated in myocardial dysfunction during sepsis. However, the underlying mechanism(s) are not clear. Interleukin-33 (IL-33) is a cytokine which regulates collagen synthesis in various cardiac pathologies. The purpose of the present study is to test whether IL-33 contributes to sepsis-induced myocardial dysfunction through regulation of matrix metalloproteinase-9 (MMP-9). The in vivo, feces-induced peritonitis (FIP) in mice and in vitro lipopolysaccharide (LPS) treatments to isolated cardiomyocytes were used. The study purpose was to explore clinical instructors’ perceptions of empowerment in academic nursing environments. Clinical instructors (CIs), often part-time faculty, facilitate learning in professional practice environments. However, they also need to function within the academic environment to learn about the curriculum and how students are to be evaluated.

The qualitative description method was used to obtain a broad understanding of CIs’ empowerment experiences and to interpret their perceptions with the framework of Kanter’s (1977, 1993) structural empowerment and Spreitzer’s (1995) psychological empowerment theories. Eight CIs from two nursing programs were interviewed.

This study examined the influence of parents’ and children’s perceptions of their built and social environments on children’s use of active transportation (AT) between home and school. For trans Ontarians with access to publicly insured health care, this study aimed to determine predictors of not having a family physician, as well as to identify factors that influence a trans patient’s comfort discussing trans status or trans-related health needs with their physician. Previously collected demographic and family physician access related data (n=433) were used. Multiple logistic predictive model showed that age, marital status, education, employment, income-to-needs ratio, and social support independently predicted not having a family physician. This study compared the effectiveness of diabetes self-management education (DSME) methods by examining changes in knowledge, attitude, and behaviour (KAB) after receiving education. Participants from a convenience sample were randomized into two groups, one receiving education through conversation maps and the other through traditional group education. Participants’ knowledge and attitude changes were measured by using a repeated measures pre-test/post-test design and changes in Hb A1c were observed. The present study sets out to better understand how falls prevention can be marketed and positioned in order to increase participation rates in seniors. Market segmentation was used to create unique marketing mixes for individual groups of seniors. The senior market was segmented into frequent falling males and females, and non-falling males and females. Frequent fallers were defined by having experienced 2 or more falls in the past year, where non-fallers had experienced 1 or less falls.
