Computer-based patient simulations have been used to enhance the dental curriculum since the 1980s. This article describes the development of CASE STUDIES for Dentistry (CSD), a patient case simulation building template, developed at Virginia Commonwealth University, with which authors who have no programming expertise can create realistic, effective, interactive multimedia patient simulations by entering their own information and images into a straightforward, fill in the blanks interface. This program was written with Authorware, by Macromedia Inc. Design considerations included emphasis on information collection and analysis, synthesis of collected information, hypothesis proposal and testing, diagnosis, and treatment planning. ABSTRACT

Peptide toxins with disulfide-stabilized structures have been used as molecular calipers to probe the outer vestibule structure of K channels. We want to apply this approach to the human ether-a-go-go-related gene (HERG) channel, whose outer vestibule is unique in structure and function among voltage-gated K channels. Our focus here is BeKm-1, a HERG-specific peptide toxin that can suppress HERG in the low nM concentration range. Although BeKm-1 shares the three-dimensional scaffold with the well-studied charybdotoxin, the two use different mechanisms in suppressing currents through their target K channels. BACKGROUND

In preclinical and phase 2 studies, adefovir dipivoxil demonstrated potent activity against hepatitis B virus (HBV), including lamivudine-resistant strains.

METHODS

We randomly assigned 515 patients with chronic hepatitis B who were positive for hepatitis B e antigen (HBeAg) to receive 10 mg of adefovir dipivoxil (172 patients), 30 mg of adefovir dipivoxil (173), or placebo (170) daily for 48 weeks. The primary end point was histologic improvement in the 10-mg group as compared with the placebo group.

RESULTS

The purpose of this study was to determine whether chlorhexidine gluconate (0.12%), used as an endodontic irrigating solution would affect the apical seal of three root canal cements. One hundred extracted human single-canal teeth were divided into 9 experimental groups of 10 teeth each, in addition to a positive and negative control group of 5 teeth each. The teeth were decoronated at the level of the cementoenamel junction, accessed, instrumented to a Master Apical File #50, irrigated with either sterile saline, 5.25% NaOCl or 0.12% chlorhexidine gluconate, and dried using paper points. Parent Perceptions Of Occupational Therapy In Early Childhood Special Education Programs Following Transition From Early Intervention Programs, Anita M. Wyrick

A qualitative approach was used to explore the perspectives of seven parents of preschool age children with disabilities in Virginia regarding occupational therapy and the transition from early intervention (EI) to early childhood special education (ECSE). Open-ended interviews were used to collect data from each family participant and were presented as individual case reports. A cross-case analysis revealed six common categories and four overarching themes. Results indicated that the transition was viewed positively when parents saw benefits of the change and when they were oriented toward the future. Purpose: The purpose of this study was to report the distribution of procedures provided to Virginia Medicaid children by three types of dental providers in rural and urban areas. Methods: Medicaid claims filed for dental patients younger than 21 were obtained and analyzed for fiscal years 1994 and 1995. Dental providers were categorized according to their practice type: general practice (GP), pediatric (PD) and public health (PH) dentists. Each type of practice was categorized as practicing in a Metropolitan, Urban, Rural or Completely Rural location and evaluated for percentages of preventive, diagnostic, and corrective services provided.Results: The number of procedures …

ABSTRACT

[Ca2+]i and electrical activity were compared in isolated β-cells and islets using standard techniques. In islets, raising glucose caused a decrease in [Ca2+]i followed by a plateau and then fast (2–3 min−1), slow (0.2–0.8 min−1), or a mixture of fast and slow [Ca2+]i oscillations. In β-cells, glucose transiently decreased and then increased [Ca2+]i, but no islet-like oscillations occurred. Simultaneous recordings of [Ca2+]i and electrical activity suggested that differences in [Ca2+]i signaling are due to …

Purpose: This study analyzed the percentage of Virginia practitioners utilizing vacuum form machines, types of appliances recommended, and types of patient information provided.Methods: Questionnaires were constructed and mailed to 2500 dentists.Results: 80% of dentists utilized vacuum form machines, 42.5% recommended mouthguards, 60.2% recommended home bleaching trays, 37.6% provided patient information on mouthguards, 37.1% provided patient information on home bleaching trays, 16.5% inquired on patient questionnaires about mouthguard protection during contact sports,and 17.3% inquired on patient questionnaires about tooth color satisfaction.Conclusions: Dentists use vacuum form machines for home bleaching trays more than mouthguards. Primary brain tumor affects some 18,000 adults in the United States each year (Silverberg et al., 1990; Merchant et al., 1997) and over 30% are high-grade anaplastic astrocytoma or glioblastoma multiforme (GBM) (Parney et al., 1997). According to Kruse et al., 1989, the treatment of patients with recurrent or persistent high-grade gliomas represents a major therapeutic challenge. The use of conventional therapy consisting of surgery, followed by radiation therapy and chemotherapy for gliomas, has been relatively ineffective (Jaecle et al., 1994) despite the fact that these therapies are cytoreductive in nature (Black, 1991). This study determined if any of six endodontic solutions would have a softening effect on resorcinol-formalin paste in extracted teeth, and if there were any differences in the solvent action between these solutions. Forty-nine single-rooted extracted teeth were decoronated two mm coronal to the CEJ, and the roots sectioned apically to a standard length of 15 mm. Canals were prepared to a 12mm WL and a uniform size with a #7 Parapost drill. Teeth were then mounted in a cylinder ring with acrylic. Orthodontic Faculty Recruitment And Retention: Goals And Perceptions, Sheldon L. Peck

There is a shortage of faculty in academic dentistry and orthodontics in particular. The purpose of this study was to quantify real and perceived differences in income, workload, and satisfaction between full-time academic and private practice orthodontists. Surveys were returned by 119 (70%) faculty, 79 (36%) private practitioners, and 160 (59%) orthodontic residents. Average annual income for faculty was less than half that of private practitioners. Faculty also reported working significantly more hours per week and taking less vacation time. Though both faculty and practitioners perceived their own jobs to be more satisfying overall, faculty felt academics was more stressful, …

Diabetes mellitus, a serious and costly disease, is a public health challenge. Diabetes is controllable, yet non-adherence to prescribed medications causes diabetesrelated complications resulting in hospital admissions and readmissions that may be prevented. A cross-sectional, descriptive-correlational study was conducted to investigate the relationship of metabolic control (A1C) to hardiness, self-efficacy, and perceived medication adherence. Health-Related Hardiness Scale (Pollock, 1990); Long-Term Medication Behavior Self-Efficacy Scale (De Geest et al., 1994); and Perceived Medication Adherence Scale (Allison, 2000) were administered to 215 participants. The metabolic syndrome, a clinical condition linked to diabetes and cardiovascular disease is a powerful predictor for overall mortality, and is present in more than 20% of the US. population (Ford, Giles, & Dietz, 2002). This study examines gender differences as well as other factors associated with metabolic syndrome as defined by the Adult Treatment Panel III of the National Cholesterol Education Program.

A sample of 10,134 adults between 20-64 years of age was selected from the Third National Health and Nutrition Survey. Metabolic syndrome was present in 19.6 % of this sample. An ecological model of health services was …

The purpose of this study was to determine the influence of sagittal mandibular position on preferred lip position in profile. Five androgynous silhouette profiles differing only in the degree of mandibular retro- or prognathism (-25º, -18º, -11º, -4º, & +3º facial contour angles) were created. Using a computer animation program, evaluators moved the lips independently into the position deemed to be the most esthetic for each profile. Evaluators included adolescent orthodontic patients, parents of patients, and orthodontists.
