Studies using cultured cells have shown that gamma interferon (IFN-gamma) induces the expression of Fc gamma RI (the type I Fc receptor for IgG) on human polymorphonuclear neutrophils (PMN) and greatly increases the number of these receptors on human monocytes. Administration of rIFN-gamma in vivo also causes enhanced Fc gamma RI expression on these cell populations. Because streptococcal antigens are potent inducers of IFN-gamma in vitro, we postulated that IFN-gamma would be produced endogenously in vivo in patients with streptococcal infections. Such production of IFN-gamma in vivo, even at low levels, might be expected to induce the expression of Fc …

Malignant External Otitis And Mastoiditis Associated With An Igg4 Subclass Deficiency In A Child., Ricardo Castro, Md, Norman Robinson, Md, Joel Klein, Md, William Geimeier, Md

We have presented the first child in Delaware with malignant external otitis associated with IgG4 deficiency. Our patient needed three courses of intravenous antibiotics and twice required mastoidectomy, but has recovered completely following the restoration of the natural barrier between the internal and external ear, using a fascial graft from the large temporalis muscle. Some hearing deficit remains.

3-Hydroxy-N-Methylpyrrolidone And Preparation Thereof, George A. Digenis, Walter J. Doll, Amale Hawi

The novel compound, 3-hydroxy-N-methylpyrrolidone, is effective as a transdermal enhancer for the absorption of drugs through the skin of humans and animals. The 3-hydroxy-N-methylpyrrolidone is prepared from delta-butyrolactone which is converted to methyl 2,4-dibromobutyrate, this intermediate is reacted with methylamine to form N-methyl-2,4-dibromobutyramide, this intermediate is converted to 3-bromo-N-methyl-2-pyrrolidone by ring closure, and the 3-bromo-N-methyl-2-pyrrolidone is reacted with an alkaline earth metal carbonate or alkaline metal carbonate to form 3-hydroxy-N-methyl-2-pyrrolidone.

Acquired Immune Deficiency Syndrome (AIDS) and impaired or threatened nutritional status seem to be closely related. It is now known that AIDS results in many nutritional disorders including anorexia, vomiting, protein-energy malnutrition (PEM), nutrient deficiencies, and gastrointestinal, renal, and hepatic dysfunction (1-7, 8).
Reversibly, nutritional status may also have an impact on the development of AIDS among HIV-infected people. Not all individuals who have tested antibody positive for the Human Immunodeficiency Virus (HIV) have developed AIDS or have even shown clinical symptoms (9, 10). A poor nutritional status, especially PEM, has a depressing effect on immunity which may predispose an …

This was particularly seen with Acquired Immunodeficiency Syndrome (AIDS) when it became known that Rock Hudson of movie and TV acclaim was dying of it. By the time the celebrated pianist and entertainer Liberace died of AIDS, the various public health agencies around the world had begun to focus on the problem, and enormous sums of money were being committed toward its control. The purpose of this project was to compare three different models of building and strengthening community-based systems of care for older adults. Models were distinguished by the type of organization that took the lead in developing the system of services: Area Agency on Aging, acute care hospital, and residential facility. Specific questions addressed in this project were: (1) What conditions in a local community give rise to a community-based system of care (CBSC) for older adults? The relations of systolic and diastolic blood pressures to alcohol intake and exercise tolerance levels in 15,612 men and 3,855 women were investigated. Alcohol intake was assessed by questionnaire and stratified into seven levels for men and six for women according to the ounces of ethanol consumed per week. Exercise tolerance was determined by maximal treadmill exercise testing and was categorized into six age-specific by sex-specific levels. Both systolic and diastolic blood pressure were significantly related to both alcohol intake and exercise tolerance levels in both men and women. Multiple factors affect the thrombin-catalyzed conversion of fibrinogen to fibrin, including: fibrinopeptide (FPA and FPB) release leading to exposure of two types of polymerization domains (“A” and “B,” respectively) in the central portion of the molecule, and exposure of a noncatalytic “secondary” thrombin-binding site in fibrin. Fibrinogen containing the FPA sequence but lacking the Bβ 1-42 sequence (“des-(Bβ 1-42)-fibrinogen”), was compared to native fibrinogen (containing both FPA and FPB) to investigate the role played by Bβ 1-42 in the polymerization of α-fibrin (i.e. fibrin lacking FPA), to compare reptilase and thrombin cleavage of FPA from fibrinogen, and …

Home Adaptations For Persons With Chronic Disabilities: An Educational Model., Ruth E. Levine, Laura N. Giltin

This paper offers a method by which to train students in the provision of culturally relevant, in-home environmental adaptations for persons with chronic disabilities. On the basis of a theoretical framework, the student therapist learns about the client's life-style and offers adaptations that evolve from a collaborative problem-solving process. The training process has been developed and refined over a 3-year period. A case study illustrates the student's application of theory and practice and the outcome for the client of this service provision approach.

It has been assumed that endogenous synthesis by the platelet precursor cell, the bone marrow megakaryocyte, is the major source of platelet a-granule protein. To test this hypothesis, we used mRNA phenotyping to detect in megakaryocytes the presence of mRNA transcripts specific for various proteins. Our results indicate that megakaryocytes synthesize platelet factor 4, a protein relatively specific for platelets, but do not express mRNA transcripts for the fibrinogen, albumin, or IgG found in a-granules. We have previously shown that mega- karyocytes endocytose circulating proteins, including fibrino- gen, albumin, and IgG, and incorporate them into a-granules. Thus, platelets appear to …

Effects Of Ethanol Extraction And Heat Treatment Of Soybean Flakes On Function And Morphology Of Pig Intestine, J. D. Hancock, E. R. Peo Jr., A. J. Lewis, Rodney A. Moxley

Digesta and tissue samples were collected from the intestinal tracts of 27 pigs to determine the relationship between intestinal morphology and the utilization of nutrients from soybean flakes. Soybean flake treatments were under-, intermediate- and overprocessed (Le., 5, 20 and 60 min of autoclaving) either without extraction or with heating before or after extraction with a 55% ethanol-water mixture. Final BW was greatest (P< .001) for pigs fed soybean flakes given 20 min of heat treatment. There was a trend (P < .09) for plasma lysine concentrations to be reduced when the unextracted soybean flakes were over-processed (60 min of heat). Differences in the flow rate of DM and N through the ileum and colon reflected differences in DM and N intake, rather than differences in intestinal function. The soybean flake treatments had no effect (P > .OS) on pH of the contents of the stomach, duodenum, ileum or colon. The ethanol extraction process increased (P < .001) N digestibility of the soybean flakes, especially when the soybean flakes were underprocessed (interaction, P < .02). Villus size (area, height and perimeter length) tended to be greater in pigs fed the soybean flakes heated after extraction and(or) exposed to the intermediate level of heat treatment. Indicators of villus shape (villus area/ villus height) and proliferative activity (crypt depth and villus heigh/crypt depth) were not affected by soybean flake treatment (P > .OS). A two-year study tested the hypothesis that growth rate interacts with bull exposure to influence age at puberty in heifers. Heifers exposed to sterile bulls and fed to achieve a high growth rate (1.8 lb/day) attained puberty 73 days earlier than heifers not exposed to bulls and fed to a moderate growth rate (1.3 lb/day). Heifers exposed to bulls and fed to achieve a moderate growth rate attained puberty at ages similar to heifers not exposed to bulls and fed to a high growth rate. Furthermore. heifers receiving these two treatments attained puberty 23 days earlier than heifers not exposed …
