Condylar Remodeling After Distraction Osteogenesis And Bilateral Sagittal Split Osteotomy, Robert Barker Hoffman

The present pilot study is a histological evaluation of mandibular condyles taken from minipigs that underwent either traditional mandibular advancement (bilateral sagittal split osteotomy), or a distraction osteogenesis procedure to advance the mandible. One pig received a bilateral sagittal split osteotomy with an immediate mandibular advancement of 14 mm, while another pig underwent distraction osteogenesis consisting of a gradual advancement of 2 mm per day to advance the mandible the same distance. The control animal received no surgery.

The present study is a histological evaluation of condyles taken from rabbits that underwent either a midline or paramedian distraction osteogenesis procedure on the mandibles. Fifteen rabbits were originally operated on and eight of these fifteen successfully made it through both the distraction osteogenesis procedure and the 1 month holding period that followed. There was one control animal.

Previous studies have evaluated the effects of various hemostatic agents on radicular tissues following apical surgery. The purpose of this study was to examine the early osseous responses to seven hemostatic agents under identical conditions in rabbits. Sixteen adult male New Zealand white rabbits were used for this study. Following anesthesia, utilizing a midline submandibular incision, standardized bony defects were prepared on the lateral-superior regions of the mandible. Test materials were placed using standardized volumes and exposure times. Distraction osteogenesis, a unique system of affecting rapid bone healing in fractures, realigning malformed bones, and lengthening congenitally short and traumatically shortened bones has become an increasingly utilized technique for the maxillofacial region. The use of distraction osteogenesis has been described for increasing the height of edentulous mandibular alveolar ridges while concurrently forming new bone within the ridge to allow for adequate reconstruction. Recent investigations have revealed that the tetracycline family of antibiotics can prevent bone loss. The purpose of this study was to evaluate the effects of the resorbable membrane Guidor™ and the osteogenic protein hOP-1 on hard tissue healing following periradicular surgery in cats.

Surgical endodontic therapy was performed on the maxillary canine teeth of twelve cats. After orthograde root canal therapy, flap reflection, osteotomy, resection of the root end, root end cavity preparation and the placement of a root end filling, eight of the surgical sites were treated with Guidor™ and eight with hOP-1. The remaining eight sites received no further treatment and served as controls.

This study evaluated candidate carriers for recombinant human bone morphogenic protein-2 (rhBMP-2) in periodontal reconstructive surgery.

Canine demineralized bone matrix (DBM), bovine inorganic crystallene bone matrix (Bio-Oss), bovine derived microfibrillar collagen matrix (Helistat), poly (D,L-lactide-coglycolide) micropaiticles (PLGA), and polylactic acid granules (Drilac) were used with rhBMP-2 (20 (µg/100 µl implant volume) in routine critical size canine supraalveolar periodontal defects. Contralateral defects in six beagle dogs were randomly assigned to receive: DBM/rhBMP-2, DBM-control, Bio-Oss/rhBMP-2, Helistat/rhBMP-2, PLGA/rhBMP-2, or Drilac/rhBMP-2, all with autologous blood except for Helistat and PLGA. Maxillary sinus osteotomy is sometimes indicated for transantral surgical approaches to palatal roots of maxillary molars. A search of the literature showed an absence of any report on the healing potential of such defects. The purpose of this study was to determine histologically the reparative process of the nasal sinus of dogs following small and large surgical antral perforations. This study was undertaken to test the hypothesis that a corticotomised tooth moves as a tooth-bone unit when under orthodontic force. A non-human primate model was designed to test the hypothesis. The non-human primate selected was Macaca fasicularis. Three adult males with erupted third molars were chosen so that extrapolation could be made to the adult human. Polyoxyinethylene (Delrin) is a polyacetyl homopolymer which has been used successfully in many orthopedic joint replacements. To determine histologic host response to Delrin as a temporomandibular joint condylar replacement condyles were removed from ten fully mature macaca fasicularis monkeys. The condyles were replaced with a total prosthetic Delrin condyle attached to a titanium mesh. Specimens obtained at one, two, and six months indicated total acceptance of the Delrin, remodeling of the glenoid fossa and formation of bone between the middle cranial fossa and the roof of the glenoid fossa. Sixty-four 35 day old Sprague Dawley Harlan rats were divided into four groups of 16 rats each and categorized as the control, orthopaedic expansion, surgery without graft and surgery with graft respectively. Expansion of the maxilla at the midpalatal suture has had much investigation and is a recognized option in orthodontic therapy. Lateral expansions of the mandible through surgical separation at the symphysis have been done in animal studies. A combination of these procedures has not yet been done together.

The purpose of this study was to expand both the maxilla and the mandible of a baboon simultaneously while maintaining occlusion. The amount of expansion and stability was studied using photographs, models, cephalograms, and tetracycline bone labels using U.V. microscopy.

Mechanical expansion of the maxilla, at the midline suture, is a widely accepted orthodontic procedure used to gain arch width in certain types of maxillary deficient arches.

In animal research on the mandible, techniques using surgical separation at the symphysis with immediate expansion up to 12 mm, fixation with a metal splint, and autogenous bone transplanted into the expanded area, have been reported to be successful.

For sometime, expansion of the maxillary arch, by means of surgical and mechanical procedures, have been acknowledged with minimal attention directed toward the mandible. In this pilot study a surgical technique was employed in separating the mandible, A-P, at the symphysis. The interosseous void, approximately 10 mm., was then filled with autogenous bone marrow collected from the greater trochanter and the crest of the ilium.

Preparations used in this study were obtained from an investigation which had undertaken the following:

Durapatite, a hydroxylapatite ceramic was implanted in 24 mature Beagle dogs to study its maintenance effect on the alveolar bone of the mandible.

After dividing the dogs into two groups, a fresh socket and a healed socket group, the posterior teeth were removed and alveolectomies were performed. Eight ceramic implants were then placed in the right mandible of each animal in each group. The healed group received the implants five weeks after tooth removal and the fresh socket group received implants immediately after extraction.

Utilizing an extra-oral approach and sterile technique, left mandibular ostectomy was performed on five miniature pigs. The segment removed was 3 cm long and included the inferior border, mandibular canal and, in some cases, apices of a molar tooth.

Bone and plaster grafts (one solid and one canalized of each) were used to fill these defects. The fifth was left with no graft.

The objective was to learn the effect of each of these procedures on regeneration of the mandibular nerve across the defect. It was hypothesized that canals through the grafts would facilitate regeneration.

The nature of epithelium and the remarkable facility with which it aggressively covers granulation tissue, suggests that removal of the sulcular epithelium surrounding the extraction wound of a tooth to be replanted could promote reattachment of the periodontal tissues at a more nearly ideal coronal level. To investigate this hypothesis twenty mature teeth in five mongrel dogs were experimentally replanted. One half of them were replanted in alveoli around which the crevicular epithelium had been removed, the remaining teeth without creviculoectomy acting as controls. This experiment was designed to evaluate an oral surgical dressing which may be used as an aid to healing tissue. The parameters of evaluation were directed toward wound healing enhancement, duration, and stability of the bandage. A histological evaluation of the surgical wounds protected with the bandage as compared to the control wounds was made.
