Many models of accident investigation have been created and have served as the basis for other tools and techniques. One of the most prominent techniques, Reason's Swiss Cheese Model (1990), is based on the idea of active and latent failures. Expanding on this idea, the Human Factors Analysis and Classification System (HFACS) was created with the idea to associate contributing factors and errors (Wiegmann & Shappell, 2003).
While HFACS has been frequently applied in the field of aviation in literature, other industry types are under-represented. Seventeen data sources encompassing various industry types were collected and included in this dissertation analysis. …

Computing a maximum a posteriori (MAP) assignment in graphical models is a crucial inference problem for many practical applications. Several provably convergent approaches have been successfully developed using linear programming (LP) relaxation of the MAP problem. We present an alternative approach, which transforms the MAP problem into that of inference in a finite mixture of simple Bayes nets. We then derive the Expectation Maximization (EM) algorithm for this mixture that also monotonically increases a lower bound on the MAP assignment until convergence. A temperature compensation scheme of detecting automobile exhaust gas based on fuzzy logic inference is presented in this paper. The principles of the infrared automobile exhaust gas analyzer and the influence of the environmental temperature on analyzer are discussed. A fuzzy inference system is designed to improve the measurement accuracy of the measurement equipment by reducing the measurement errors caused by environmental temperature. The case studies demonstrate the effectiveness of the proposed method. The fuzzy compensation scheme is promising as demonstrated by the simulation results in this paper.

This paper presents a structure first image inpainting method based on self-organizing map (SOM). SOM is employed to find the useful structure information of the damaged image. The useful structure information which includes relevant edges of the image is used to simulate the structure information of the lost or damaged area in the image. Assortment planning is the process in which a retailer selects a product line to offer to customers and is a key determinant of a retailer's profit. We consider the assortment planning problem using a locational choice model for customer product selection and allow for both horizontal and vertical product differentiation. When the distribution of customer preference is unimodal, the optimal solutions for this problem are unknown. We propose two solution philosophies for generating product assortments. This study attempted to estimate the damping properties of human tissue by using spring and damper system as a model. Data of impacting human tissue at the deltoid area was used to obtain a continuous, second order system to represent the mass-spring-damper system. A discrete ARMA(2,1) model was fitted using the data obtained from experiments in which the deceleration of a pendulum impacting human shoulders in the area of the deltoid muscle was measured. The data of the deceleration was integrated twice to obtain estimates of displacement. The integration was done until the maximum displacement occurred at zero velocity.

Force exertion is critical in grasping and holding activities at sub-maximal levels. Exertion misjudgments lead to musculoskeletal disorders (MSDs) impairing performance and productivity. Published literatures on grasping have addressed the force balance and endurance issues for non-prehensile movements of hand. However, little information is available on the force exerted in precision gripping employed in health care. Hybrid electric vehicle (HEV) uses both internal combustion engine (ICE) with an electric system. The combination of the electric power train with the ICE is intended to achieve both better fuel economies than the conventional vehicles and better performance. Quarter Billion dollars could be saved annually by double stacking pallets. A forklift storing 100 double stacked pallets saves 2.5 working hours versus single pallets. More than one billion pallets and cases handled between Wal-Mart distribution centers according to BEA White Paper (2006) [1], assume only 500 million were stackable pallets. This translates to 12.5 million working hours. If a forklift driver earns $20 per hour, then $250 million can be saved.

In this paper, a general regression neural network (GRNN) based controller is used to control the speed and thrust output of the linear induction motor drive. The field orientation principle is used to asymptotically decouple the motor speed from the secondary flux. Mechanical stresses experienced by packages in the distribution environment include shock and vibration amongst several others. The destructive effects of these hazards can typically be restricted by using cushioning materials to help protect fragile goods during distribution. ASTM D1596 is the conventional standard used to determine shock absorbing performance of a cushioning material for a given combination of static loading, thickness, and drop height. We consider the joint acquisition and pricing problem where the retailer sells multiple products with uncertain demands and the suppliers provide all unit quantity discounts.The problem is to determine the optimal acquisition quantities and selling prices so as to maximize the retailer’s expected profit, subject to a budget constraint. This is the first extension to consider supplier discounts in the constrained multi-product newsvendor pricing problem. Remanufacturing, recycling, and disposal recovery operations require the performance of disassembly activities. The disassembly line is the best choice for automated disassembly of returned products, however, finding the optimal balance is computationally intensive with exhaustive search quickly becoming prohibitively large. In this paper, a greedy algorithm is presented for obtaining optimal or near-optimal solutions to the disassembly line balancing problem. The greedy algorithm is a first-fit decreasing algorithm further enhanced to preserve precedence relationships. Variability in the inflow of end-of-life (EOL) products and fluctuating inventory levels often make the processing of EOL products an economically risky operation for product recovery facilities (PRFs). Choosing an appropriate pricing policy can enhance the performance of PRFs by methodically clearing their inventory and increasing profits. This work presents two pricing models to counter the prospect of product obsolescence that can happen either gradually or suddenly. We generate a disassembly PN (DPN) from a disassembly precedence matrix. The resulting DPN can be analyzed using the reachability tree method to generate all feasible disassembly process plans (DPPs), and cost functions can be used to determine the optimal DPP. Since generating the reachability tree is NP-complete, we develop a heuristic algorithm to limit the size of the reachability tree. The algorithm employs multihypothesis search to dynamically explore the v likeliest lowest cost branches of the tree, in order to identify near-optimal DPPs. We study the disassembly process of a moderately complex consumer product (cell phone) consisting of 25 components. The investigation involves a heuristically solved disassembly line balancing problem and the determination of an appropriate disassembly sequence influenced by sequence-dependent costs, which is based on a novel combination of exact and heuristic algorithms. The issue of generating various suboptimum solutions to cope with multiple optimization criteria is also discussed.
