Approximately 19% of breast cancer patients undergoing breast conserving surgery (BCS) must return for a re-excision surgery due to positive margin reported by pathology. This problem is mainly associated with lack of intraoperative cancer margin assessment tools. Our previous work demonstrated that lipid-weighted photoacoustic imaging (PAI) can be used intraoperatively to successfully differentiate between cancerous and healthy tissue with high sensitivity. However, the technology was cumbersome and significantly prolonged the procedure time. Conventional whole-gland therapies often result in overtreatment in such cases and debate still surrounds the optimal method of oncologic control. CT perfusion (CTP) imaging is a validated treatment decision support tool in acute ischemic stroke. Automated analysis of CTP cerebral blood flow (CBF) and Tmax maps produces estimates of ischemic core and penumbra volumes used to determine target mismatch profiles for treatment. Photoacoustic tomography (PAT) is a widely explored hybrid imaging modality combining advantages of ultrasound and optical imaging. However, on the acoustic detection side, limited-view angle coverage and limited-detector bandwidth are common key issues in PAT systems that result in unwanted artifacts. While analytical and simulation studies of limited-view artifacts are extensive, experimental setups capable of comparing limited-view to an ideal full-view case are lacking. Concussion is a traumatic brain injury usually caused by a direct or indirect blow to the head that affects brain function. The maximum mechanical impedance of the brain tissue occurs at 450±50 Hz and may be affected by the skull resonant frequencies. After an impact to the head, vibration resonance of the skull damages the underlying cortex. The skull deforms and vibrates, like a bell for 3 to 5 milliseconds, bruising the cortex. Oscillating gradient spin-echo (OGSE) is an implementation of diffusion MRI that enables shorter effective diffusion times than the conventional pulse gradient spin-echo (PGSE) by periodically modulating the diffusion gradient. Measurements of the diffusion kurtosis, which reflects the degree of restricted diffusion, have previously been prohibited with OGSE due to technical limitations of clinical gradient systems. This thesis presents a novel oscillating gradient waveform that enables the measurement of kurtosis using OGSE without requiring advanced gradient hardware. Decreases of kurtosis are observed in OGSE acquisitions of healthy human subjects relative to PGSE, demonstrating the dependence of the kurtosis on oscillation frequency. …

Fringe Projection Profilometry (FPP) is a popular method for non-contact optical surface measurements, including motion tracking. The technique derives 3D surface maps from phase maps estimated from the distortions of fringe patterns projected onto the surface of an object. Estimation of phase maps is commonly performed with spatial phase retrieval algorithms that use a series of complex data processing stages. Premature birth, defined as a gestational period less than 37 weeks, occurs in 8% of infants born in Canada. These births are associated with a higher risk of developing neurological complications. Infants born with very low birth weights (VLBW, < 1500 g) experience cognitive or behavioural deficits at a rate of 40-50%, while a further 5-10% develop major disorders such as cerebral palsy. The likelihood of injury increases with a shorter gestational period and/or a lower birthweight. Intraventricular hemorrhaging (IVH) occurs in 20-25% of VLBW infants, characterized by bleeding in the germinal matrix and surrounding white matter. This highly vascularized region is particularly susceptible to bleeds due to underdeveloped cerebrovascular structures. Severe IVH causes an inflammatory response and subsequent obstruction of cerebrospinal fluid (CSF) drainage, resulting in enlargement of the brain’s ventricles, referred to as post-hemorrhagic ventricular dilatation (PHVD). PHVD increases intracranial pressure and can result in compression/damage of brain tissue.

Diagnosis of IVH and PHVD is regularly performed using cranial ultrasound. Clinicians can visually assess and grade hemorrhaging/ventricle dilatation. Ultrasound, however, is limited in its ability to continuously monitor and only detects irreversible damage. Diffusion MRI is used to non-invasively characterize the microstructure of the brain. However, the accuracy of the characterization is difficult to verify because no other non-invasive imaging modality provides the same information. This thesis presents a novel 3D printed axon-mimetic (3AM) diffusion MRI phantom, a synthetic object designed to mimic the brain's microstructure.

The phantoms were characterized using microscopy, synchrotron micro-computed tomography, and diffusion MRI, and found to have sufficiently axon-mimetic properties to be useful as diffusion MRI phantoms. Vegetative state (VS) is a disorder of consciousness often referred to as “wakefulness without awareness”. Patients in this condition experience normal sleep-wake cycles, but lack all awareness of themselves and their surroundings. Clinically, assessing consciousness relies on behavioural tests to determine a patient’s ability to follow commands. This subjective approach often leads to a high rate of misdiagnosis (~40%) where patients who retain residual awareness are misdiagnosed as being in a VS. Although computational tools, such as finite element analysis, provide a powerful quantitative technique to evaluate and answer a variety of biomechanical and clinical questions, glenohumeral finite element models (FEMs) have not kept pace with improvements in modeling techniques or model validation methods seen in other anatomic locations. Cochlear implants provide the sensation of sound to deaf individuals. An accurate estimate of cochlear duct length (CDL) is required for pre-operative implant electrode selection and can be obtained from clinical computed tomography (CT) by measuring the “A-value”.

The objectives of this work were to estimate the accuracy and variability in manual A-value measurements, and to automate measurements.

Four specialists repeatedly measured the A-value on clinical CT images from which the inter- and intra-observer variability were calculated. To address the operator-dependent settings challenge, an automated method for wall filter cut-off selection, the wall filter selection curve (WFSC) method, was developed using flow-phantom images. This new high-contrast imaging can be employed to study the first hundred nanometers from the surface as it utilizes the evanescent field of a waveguide as the illumination source. Colloidal inorganic nanoparticles (NPs) have been attracting considerable interest in biomedicine, from drug and gene delivery to imaging, sensing and diagnostics. It is essential to modify the surface of nanoparticles to have enhanced biocompatibility and functionality for the in vitro and in vivo applications, especially in delivering locally and recognizing biomolecules. Herein, the goal of this research work is to develop advanced NPs with well-tailored surface functionalities and/or bio-functionality for the applications in cell tracking and analytes detection.

Endoscopic cameras are increasingly employed in image-guidance procedures, where the video images must be registered to data from other modalities. However, such cameras are susceptible to distortions, requiring calibration before images can be used for registration, tracking and 3D reconstruction. Camera calibration is learned in a laboratory setting, where configuring and adjusting the physical setup is tedious and not necessarily conducive to learning. A centralized resource that utilizes 3D interactive components needs to be available for training on camera calibration.
