Modern medicine has dramatically improved the lives of many. In orthopaedics, robotic surgery has given clinicians superior accuracy when performing interventions over conventional methods. Nevertheless, while these and many other methods are available to ensure treatments are performed successfully, far fewer methods exist to predict the proper treatment option for a given person. Clinicians are forced to categorize individuals, choosing the best treatment on “average.” However, many individuals differ significantly from the “average” person, for which many of these treatments are designed. This dissertation focused on modeling specimen-specific soft tissue structures in the context of joint replacement surgery. The research addressed four key aspects. The first study involved developing a workflow for creating finite element models of the hip capsule to replicate its torque-rotational response. Experimental data from ten cadaveric hips were used to calibrate the models, resulting in improved accuracy and relevance for surgical planning and implant design. The second study tackled the challenge of expediting the calibration of mechanical properties of the hip capsule to match patient-specific laxities. Biomechanical analysis is widely used to assess human movement sciences, specifically using three-dimensional motion capture modelling. There are unprecedented opportunities to increase quantitative knowledge of rehabilitation and recreation for disadvantaged population groups. Specifically, 3D models and movement profiles for human gait analysis were generated with emphasis on post-stroke patients, with direct model translation to analyze equivalent measurements while horseback riding in use of the alternative form of rehabilitation, equine assisted activities and therapies (EAAT) or hippotherapy (HPOT). Significant improvements in gait symmetry and velocity were found within an inpatient rehabilitation setting for patients following a stroke, and the developed movement …

Because of its extreme rarity, the genetic disease arthrogryposis multiplex congenita (AMC) and the needs of individuals with the diagnosis are often overlooked. AMC refers to the development of nonprogressive contractures in disparate areas of the body and is characterized by decreased flexibility in joints, muscle atrophy, and developmental delays. Colton Darst, a seven-year-old boy from Indianapolis, Indiana, was born with the disorder, and since then, he has undergone numerous surgical interventions and continues to receive orthopedic therapy to reduce his physical limitations. Necessary for effective ambulation, head stability affords optimal conditions for the perception of visual information during dynamic tasks. This maintenance of head-in-space equilibrium is achieved, in part, by the attenuation of the high frequency impact shock resulting from ground contact. While a great deal of experimentation has been done on the matter during steady state locomotion, little is known about how head stability or dynamic visual acuity is maintained during asymmetric walking.

This study examines several different kinetic variables in relation to pitch velocity and elbow varus torque in collegiate baseball pitchers using force plates, an inertial measurement unit, and a radar unit. The purpose of this study is to investigate the kinetic variables being measured and their relationship to pitch velocity and loads being placed on the medial elbow. Radial head hemiarthroplasty design has been extensively studied with a focus on utilizing the geometrical characteristics of the native radial dish as a guide of implant placement and design. Although implant design has been studied, optimal radial head implant fixation technique remains unknown. This thesis focused on the effect of stem fit on radiocapitellar contact mechanics, using both finite element and experimental bench-top approaches. Bone mineral density (BMD), among other factors, largely effect the initial stability of the cementless tibial tray component in a total knee replacement (TKR), where increased motion at the tray-bone interface hinders bony ingrowth. With a lack of bony ingrowth, the cementless implant will not experience long-term success. Understanding which factors influence initial stability yields insight into surgical technique considerations and help inform a surgeon’s implant choice. Transtibial amputees are at increased risk of contralateral hip and knee joint osteoarthritis, likely due to abnormal biomechanics. Biomechanical challenges exist for transtibial amputees in gait and cycling; particularly, asymmetry in ground/pedal reaction forces and joint kinetics is well documented and state-of-the-art passive and powered prostheses do not fully restore natural biomechanics. This thesis proposes an algorithm that determine the geometry of 3D-printed, custom-designed spring element bands made of thermoplastic elastomer (TPE) for use in a wearable orthotic device to aid in the physical therapy of a human hand exhibiting spasticity after stroke. Each finger of the hand is modeled as a mechanical system consisting of a triple-rod pendulum with nonlinear stiffness at each joint and forces applied at the attachment point of each flexor muscle. Based on the principles of cutting edge bone remodeling research, a unique therapeutic exercise device was designed specifically to improve bone quality at the most critical location of the proximal femur prone to fracture: the superior-lateral femoral neck where the fracture first initiates during a fall. The exercise/device is intended to work by inducing enough strain in the bone to stimulate the body’s natural bone remodeling mechanisms to increase bone density in the proximal femur and consequently prevent a fracture from arising if a fall to the side does occur.

Rowing machines are used for performance improvements through concentric exercises, however a combination of concentric and eccentric actions is known to improve the effectiveness of training. Many studies aim to understand the fundamentals of kicking commonly displayed by soccer players [4,6,10,16,17,18,24,25,28,29,30,34,36,38,40]. Of those studies, most are limited to a two-dimensional (2D) analysis using high-speed cameras for position tracking or utilizing electromyography to observe the activity of select muscles [4,6,18,25,29,36]. The few studies that investigate kicking using a three-dimensional (3D) model are limited in their position tracking capabilities and focus mainly on joint flexion potentials and foot speed.

The anterior cruciate ligament (ACL) provides stability to the knee joint while performing activities such as a side step cut. Neuromuscular fatigue, a reduction in muscle force producing capabilities, alters lower extremity mechanics while performing a side step cut and may increase the risk of ACL injury, particularly in females. Muscle mechanical behavior potentially plays an important role in some of the most common bladder disorders. These include overactive bladder, which can involve involuntary contractions during bladder filling, and impaired contractility or underactive bladder, which may involve weak or incomplete contractions during voiding. Actin-myosin cross-bridges in detrusor smooth muscle (DSM) are responsible for contracting and emptying the bladder. The total tension produced by muscle is the sum of its preload and active tensions. In vitro biomechanical investigations can help to identify changes in subaxial cervical spine (C3-C7) stability following injury, and determine the efficacy of surgical treatments through controlled joint simulation experiments and kinematic analyses. However, with the large spectrum of cervical spine trauma, a large fraction of the potential injuries have not been examined biomechanically. This includes a lack of studies investigating prevalent flexion-distraction injuries. Therefore, the overall objective of this thesis was to investigate the changes in subaxial cervical spine kinematic stability with simulated flexion-distraction injuries and current surgical instrumentation approaches using both established and novel biomechanical techniques.

The knee joint plays an essential role in the human musculoskeletal system. It has evolved to withstand extreme loading conditions, while providing almost frictionless joint movement. However, its performance may be disrupted by disease, anatomical deformities, soft tissue imbalance or injury. Knee disorders are often puzzling, and accurate diagnosis may be challenging. Current evaluation approach is usually limited to a detailed interview with the patient, careful physical examination and radiographic imaging. Neuroprostheses can be used to restore movement of the upper limb in individuals with high-level spinal cord injury. Development and evaluation of command and control schemes for such devices typically require real-time, ldquopatient-in-the-looprdquo experimentation. A real-time, 3-D, musculoskeletal model of the upper limb has been developed for use in a simulation environment to allow such testing to be carried out noninvasively. The model provides real-time feedback of human arm dynamics that can be displayed to the user in a virtual reality environment.
