Upper extremity (UE) pain affects approximately 30.7% of adults in the United States and is associated with decreased function and participation. Despite evidence supporting non-pharmacological approaches, pharmacological interventions remain commonly used. This capstone project implemented and evaluated an occupation-based educational program to promote non-pharmacological pain management among adults receiving outpatient occupational therapy for UE conditions. This 14-week project included 10 participants and utilized a mixed-methods design with pre-, mid-, and post-surveys. The program consisted of an introduction and three modules addressing therapeutic movement, physical agent modalities, and sensory regulation. This doctoral capstone experience (DCE) was completed at Corewell Health Grand Rapids Hospitals Hand Therapy Clinic and focused on two ACOTE concentration areas including advanced clinical practice skills and education. The experience combined hands-on clinical practice in a specialized outpatient hand therapy setting with the development of high-quality client education materials and home exercise programs (HEPs). Objectives were created to address identified needs through the creation of client education handouts, development of diagnosis-specific Medbridge HEP templates, and creation of training resources to improve therapist efficiency with Medbridge use. Non-adherence to prescribed home exercise programs significantly undermines the efficacy of physical therapy (PT), with research indicating only approximately 35% of patients fully adhere to regimens. This issue contributes to suboptimal recovery and places a staggering financial burden on the healthcare system, estimated at $100 billion to $300 billion annually. The Physical Therapy Home Exercise Adherence Trial (PT-HEAT) evaluates the effectiveness of a 14-week behavioral education intervention designed to improve patient adherence outside the clinical setting.

This project aims to address a major problem in the physical therapy industry: the “home adherence gap” which is the routine failure to follow through with prescribed exercise at home. Research shows that only about 35% of patients routinely follow through with exercises prescribed for them to do on their own. By focusing on behavior modification and education rather than just physical instruction, this intervention aims to improve long-term recovery, and reduce need for procedures like injections and surgeries. This project aimed to address educational and psychosocial needs through two targeted initiatives grounded in health promotion, client-centered care, and community engagement. The first initiative focused on improving awareness of pelvic floor health in men affected by cancer. Individuals with intellectual and developmental disabilities (IDDs) experience disproportionately low employment rates despite expressing a strong desire for community integrated employment. This study examined whether discrepancies exist between the current work capacity of adolescents and young adults with IDDs and the physical exertion demands of their desired careers, and explored the role of physical therapy (PT) in addressing these gaps. Forty-five participants (ages 14–24) in the Nebraska EmployABILITY project identified their top career choice. Diagnostic Reasoning and Differential Diagnosis (DD) are increasingly vital in occupational therapy (OT) practice as direct access legislation expands evaluative responsibilities for occupational therapy practitioners (OTPs). Despite this need, little research has explored how OTPs understand and apply diagnostic reasoning in clinical contexts. This study examined processes, strategies, and perceived barriers and facilitators influencing OTPs’ engagement in diagnostic reasoning and DD across diverse settings. A national online survey of licensed OTPs collected quantitative and qualitative data on knowledge, confidence, training, interprofessional collaboration, and clinical behaviors. Driver rehabilitation is an underrepresented specialization within occupational therapy despite its critical role in supporting individuals with physical, cognitive, or visual impairments to drive safely and maintain community mobility. The literature reveals limited inclusion of driver rehabilitation in OT curricula, minimal fieldwork exposure, and scarce driver rehabilitation programs, resulting in fewer certified specialists. Barriers to program development include insufficient funding, lack of institutional support, and limited equipment access. Cardiac rehabilitation (CR) is a cost-effective, evidence-based approach for patients with cardiac conditions to improve quality of life and health outcomes. Despite the benefits, it remains a nationally underutilized service, and extensive research has been conducted to identify interventions that most effectively increase referrals and participation in CR programs. Two of the more successful interventions include strengthening provider recommendations and using a standardized referral system (Ades et al., 2022).
