Abstract

Approximately 40% of all healthcare workers have reported episodes of violence by patients over the last 2 years. This Veteran’s Health Agency reported 824 disruptive events reported in the last 5 years, of which 56 occurred in the emergency department. Research shows that interventions are not always timely, resulting in the clinical staff requesting the Veteran’s Health Agency Police to intervene when a veteran’s level of stress is escalating. This study presents research about the nature and prevalence of adverse childhood experiences (ACEs), the mental and physical impacts to individuals exposed to ACEs, and incorporating mindfulness practices into therapeutic approaches to treating them, particularly Mindfulness-Based Narrative Therapy (MBNT). It is important because ACEs have become a global crisis, with the potential for interfering with psychological and physical development, early mortality, and other harmful life outcomes. This thesis provides critical information for mindfulness teachers and practitioners, therapists, educators, regulators, parents, and individuals subjected to ACEs.

This study investigated the lived experiences of individuals living with HIV (PLWH) in the Dallas-Fort Worth (DFW) area of Texas during the COVID-19 pandemic, using an Interpretative Phenomenological Analysis (IPA) framework. The research examined the intersection of health, social dynamics, and psychological resilience among PLWH, who faced compounded vulnerabilities due to systemic inequities and the dual public health crises of HIV and COVID-19. Group therapy is an essential part of inpatient psychiatric treatment, with documented positive effects including better medication compliance, lower readmission rate, and improved self- esteem. Patients who are admitted for an inpatient psychiatric stay have a small window of time in which behavioral health staff can work with them. Background: Perinatal mental health is a growing concern, with under identification and lack of access to mental health as contributing factors. Screening tools such as the Mood Disorder Questionnaire are one way to increase identification of patients at risk for bipolar-spectrum disorders and subsequent postpartum psychosis and initiate referrals to appropriate mental health services.

Local Problem: One outpatient women’s health center has a 94% screening rate for the Edinburgh Postnatal Depression Score (EPDS) for anxiety and depression but has not implemented a screening for bipolar disorder. การวิจัยกึ่งทดลองครั้งนี้ มีวัตถุประสงค์เพื่อเปรียบเทียบอาการทางบวกของผู้ป่วยจิตเภทในชุมชนก่อนและหลังได้รับโปรแกรมการจัดการความเครียดโดยการมีส่วนร่วมของครอบครัว และเพื่อเปรียบเทียบอาการทางบวกของผู้ป่วยจิตเภทในชุมชนระหว่างกลุ่มทดลองหลังได้รับโปรแกรมฯ และกลุ่มควบคุมที่ได้รับการพยาบาลตามปกติ กลุ่มตัวอย่างคือผู้ป่วยจิตเภทอายุระหว่าง 20-59 ปี อาศัยอยู่กับครอบครัว ซึ่งเข้ารับการรักษาที่แผนกกลุ่มงานจิตเวชและยาเสพติด โรงพยาบาลองครักษ์ จำนวน 60 ครอบครัว โดยการคัดเลือกกลุ่มตัวอย่างแบบเจาะจงได้มาจากเกณฑ์คุณสมบัติที่ผู้วิจัยกำหนด แบ่งเป็นกลุ่มทดลอง 30 ครอบครัว และกลุ่มควบคุม 30 ครอบครัว กลุ่มทดลองได้รับโปรแกรมการจัดการความเครียดโดยการมีส่วนร่วมของครอบครัว และกลุ่มควบคุมได้รับการพยาบาลปกติ เครื่องมือที่ใช้ในการวิจัย ได้แก่ 1) โปรแกรมการจัดการความเครียดโดยการมีส่วนร่วมของครอบครัว 2) แบบสอบถามข้อมูลส่วนบุคคล 3) แบบประเมินอาการทางจิต (PANSS-T) มีค่าสัมประสิทธิ์อัลฟาของครอนบาคเท่ากับ .82 และค่า CVI ได้เท่ากับ 1 4) แบบวัดการรับรู้ความเครียด มีค่าสัมประสิทธิ์อัลฟาของครอนบาคเท่ากับ .85 และค่า CVI ได้เท่ากับ .92 5) แบบสอบถามการมีส่วนร่วมของครอบครัว มีค่าสัมประสิทธิ์อัลฟาของครอนบาคเท่ากับ .85 และค่า CVI ได้เท่ากับ 1 เครื่องมือทุกชุดผ่านการตรวจสอบคุณภาพโดยการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ จำนวน 5 ท่าน วิเคราะห์ข้อมูลโดยใช้ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติทดสอบที ผลการวิจัย พบว่า 1. ค่าเฉลี่ยของอาการทางบวกของผู้ป่วยจิตเภทในชุมชนหลังได้รับโปรแกรมการจัดการความเครียดโดยการมีส่วนร่วมของครอบครัวต่ำกว่าก่อนได้รับโปรแกรมการจัดการความเครียดโดยการมีส่วนร่วมของครอบครัว อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (t = 15.74, df = 29, p

การวิจัยนี้เป็นการวิจัยกึ่งทดลองใช้รูปแบบการศึกษาสองกลุ่มแบบวัดซ้ำ มีวัตถุประสงค์เพื่อเปรียบเทียบค่าเฉลี่ยคะแนนอาการสมาธิสั้นทั้ง 3 ด้าน (อาการขาดสมาธิ, อาการอยู่ไม่นิ่ง/หุนหันพลันแล่น และอาการดื้อต่อต้าน) ของเด็กสมาธิสั้นวัยเรียนในระยะก่อนได้รับโปรแกรมฯ หลังได้รับโปรแกรมฯทันที และหลังได้รับโปรแกรมฯ 1 เดือน และเปรียบเทียบผลต่างค่าเฉลี่ยคะแนนอาการสมาธิสั้นของเด็กสมาธิสั้นวัยเรียนที่ได้รับโปรแกรมเสริมสร้างการปฏิบัติหน้าที่ของครอบครัวแบบผสมผสานกับกลุ่มที่ได้รับการพยาบาลตามปกติ กลุ่มตัวอย่าง คือ เด็กที่มีอายุระหว่าง 6-12 ปี ที่ได้รับการวินิจฉัยจากจิตแพทย์ตามเกณฑ์ ICD-10 ว่าเป็นโรคสมาธิสั้นและครอบครัวที่มารับบริการแผนกผู้ป่วยนอก โรงพยาบาลพระศรีมหาโพธิ์ จำนวน 40 ราย โดยคัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจงตามคุณสมบัติที่กำหนด กลุ่มทดลองได้รับโปรแกรมเสริมสร้างการปฏิบัติหน้าที่ของครอบครัวแบบผสมผสานและกลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการวิจัย ประกอบด้วย 1) โปรแกรมเสริมสร้างการปฏิบัติหน้าที่ของครอบครัวแบบผสมผสาน 2) แบบประเมินอาการสมาธิสั้น SNAP-IV และ 3) แบบประเมินการทำหน้าที่ของครอบครัว การวิเคราะห์ข้อมูลใช้สถิติการวิเคราะห์ความแปรปรวนแบบวัดซ้ำ (Repeated Measures ANOVA) และการทดสอบค่าเฉลี่ยด้วยสถิติ Bonferroni กำหนดค่าระดับนัยสำคัญทางสถิติที่ .05 ผลการวิจัยสรุปได้ดังนี้ 1. อาการสมาธิสั้นทั้ง 3 ด้านของเด็กสมาธิสั้นวัยเรียนที่ได้รับโปรแกรมเสริมสร้างการปฏิบัติหน้าที่ของครอบครัวแบบผสมผสานในระยะหลังการทดลองทันทีและหลังการทดลอง 1 เดือน ลดลงกว่าก่อนการทดลองอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. อาการสมาธิสั้นทั้ง 3 ด้านของเด็กสมาธิสั้นวัยเรียนที่ได้รับโปรแกรมเสริมสร้างการปฏิบัติหน้าที่ของครอบครัวแบบผสมผสานกับกลุ่มควบคุมที่ได้รับการพยาบาลตามปกติเฉพาะในระยะหลังการทดลอง 1 เดือนอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การศึกษานี้เป็นการวิจัยกึ่งทดลองแบบสองกลุ่มวัดก่อนและหลังการทดลอง มีวัตถุประสงค์เพื่อเปรียบเทียบทักษะทางสังคมของเด็กออทิสติกวัยเรียนก่อนและหลังได้รับโปรแกรมวีดิทัศน์แสดงตัวแบบส่งเสริมแรงจูงใจทางสังคม และเพื่อเปรียบเทียบทักษะทางสังคมของเด็กออทิสติกวัยเรียนระหว่างกลุ่มที่ได้รับโปรแกรมวีดิทัศน์แสดงตัวแบบส่งเสริมแรงจูงใจทางสังคมกับกลุ่มที่ได้รับการดูแลตามปกติ จำนวน 42 คน แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 21 คน กลุ่มทดลองได้รับวีดิทัศน์แสดงตัวแบบส่งเสริมแรงจูงใจทางสังคมต่อทักษะทางสังคมในเด็กออทิสติกวัยเรียน ขณะที่กลุ่มควบคุมได้รับการดูแลตามปกติ เครื่องมือที่ใช้ในการวิจัยประกอบด้วย 1) โปรแกรมวีดิทัศน์แสดงตัวแบบส่งเสริมแรงจูงใจทางสังคมต่อทักษะทางสังคมในเด็กออทิสติกวัยเรียน 2) แบบสังเกตทักษะทางสังคมของเด็กออทิสติกวัยเรียน 3) แบบประเมินแรงจูงใจทางสังคม โดยใช้แบบประเมิน Stanford Social Dimensions Scale(SSDS 1.0) (Phillips et al, 2019) เครื่องมือทั้งหมด ผ่านการตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ โดยแบบสังเกตทักษะทางสังคม และแบบประเมินแรงจูงใจทางสังคม มีค่าความตรงเชิงเนื้อหาเท่ากับ .89 และ .98 ตามลำดับ นอกจากนี้ ตรวจสอบความเที่ยงได้เท่ากับ .92 และ .92 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงบรรยาย และสถิติทดสอบที ผลการศึกษาพบว่า 1.คะแนนค่าเฉลี่ยทักษะทางสังคมของเด็กออทิสติกวัยเรียนหลังเข้าโปรแกรมวีดิทัศน์แสดงตัวแบบส่งเสริมแรงจูงใจทางสังคม ( X = 13.81, SD = 2.63) สูงกว่าก่อนเข้าร่วมโปรแกรมวีดิทัศน์แสดงตัวแบบส่งเสริมแรงจูงใจทางสังคม ( X = 10.24, SD = 1.89) อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (t=-8.82, df=20, p<.05) 2. คะแนนค่าเฉลี่ยทักษะทางสังคมของเด็กออทิสติกวัยเรียนหลังเข้าโปรแกรมวีดิทัศน์แสดงตัวแบบ ส่งเสริมแรงจูงใจทางสังคม ( X = 13.81, SD = 2.63) สูงกว่ากลุ่มที่ได้รับการดูแลตามปกติ ( X = 9.38, SD = 1.46) อย่างมีนัยส าคัญทางสถิติที่ระดับ .05 (t=6.597, df=31.331, P<.05)
