BACKGROUND: Studies show obesity decreases risk of tuberculosis (TB) disease. There is limited evidence on whether high body mass index also protects against TB infection; how very high body mass indices influence TB risk; or whether nutritional status predicts this risk in children. We assessed the impact of body mass index on incident TB infection and disease among adults and children.

METHODS AND FINDINGS: We conducted a prospective cohort study among household contacts of pulmonary TB cases in Lima, Peru. We determined body mass index at baseline and followed participants for one year for TB infection and disease. We used …

Every state in the United States has passed legislation allowing students to self-carry rescue inhalers in the school setting. Many organizations are stakeholders in respiratory issues, school health issues, and pediatric issues, recommending the support of this practice. Students’ ability to self-carrying rescue inhalers in the school setting has been impacted by school nurse perceptions and decision-making. This study addressed the questions: (a) What are school nurse perceptions and attitudes in regard to children self-carrying inhalers in the school setting? (b) How do school nurses decide whether children can self-carry inhalers in the school setting? and, (c) Are school districts …

Avoiding Hospital-Acquired Conditions: A Qualitative Analysis Of Early Top Performers, Tammy Doolittle 2016 University of San Diego

Avoiding Hospital-Acquired Conditions: A Qualitative Analysis Of Early Top Performers, Tammy Doolittle

Dissertations

Preventable medical errors in hospitals cost our nation an estimated 180,000 annual deaths and tens of billions of dollars. The Patient Protection and Affordable Care Act mandated improvements in quality and patient safety. The Centers for Medicare and Medicaid Services, acting on these imperatives initiated legislation-mandated programs halting reimbursement to hospitals for specific hospital-acquired conditions (HAC) coined as Never Events. To date, studies have focused on quantifying incidence of specific HAC and assessing policy impact without correlating findings to actual outcome performance. The purpose of this study was to develop an understanding of organizational, contextual, process, and other factors occurring …

Exploring The Lived Experience Of Male-To-Female Transgender Youth Accessing Trans-Related Healthcare In Los Angeles, Marcel Fomotar

Dissertations

Background: According to the National Institutes of Health (NIH) (2015), LGBTI individuals delay healthcare treatment because they fear stigmatization or because they believe many healthcare professionals lack knowledge and experience working with LGBTI individuals. The phenomenon of living as a transgender person and accessing trans-related healthcare is understudied.

Aim: To explore the perceived meaning of accessing transgender-related healthcare among transgender youth.

Method: A phenomenological qualitative design grounded on Husserl’s descriptive phenomenology was used to explore the ascribed meaning associated to accessing trans-related healthcare services among transyouth as they experienced and perceived it. A purposive and thematic sample of Male-to-Female transyouth …

Transitions To Assisted Living Within A Continuing Care Retirement Community, Judith H. Scott

Dissertations

Purpose/Aims: The purpose of this study was to explore the lived experience of older single women who moved from their independent homes to the assisted living setting, both situated within a Continuing Care Retirement Community (CCRC).

Rationale: Anecdotally, CCRC transitions have been viewed as either one dreaded possible step toward the end of life or a supportive place to open a new chapter in older adults’ lives. However, no studies have actually examined this experience among older single women, who comprise a large proportion of people making this transition.

Background: Older adults move to assisted living within a CCRC when …

The Effect Of Organizational Commitment, Organizational Based Self Esteem And Work Engagement On Nurses' Perception Of Medication Errors, Vanessa V. Wertheim

Dissertations

Background: Organizations rely on the accurate reporting of medication errors or omissions by bedside nurses to improve the medication delivery system in order to prevent further errors from occurring in the future. In a time where nurses are managing more complex patients with shorter lengths of stay, full engagement in the role is essential. Literature from other disciplines suggests that many factors can influence work engagement (Chughtai & Buckley, 2011; Schaufeli, Salanova, González-Romá & Bakker, 2002; Schaufeli & Bakker, 2003). Two of these factors include organizational commitment and organizational-based self-esteem (Allen & Meyer, 1990; Bowling, Eschleman, Wang, Kirkendall & Alarcon, …

Telehealth: Improving Quality Of Life In Veterans With Congestive Heart Failure, Marcia Callender Callender

Walden Dissertations and Doctoral Studies

Congestive heart failure (CHF) affects an estimated 5.1 million Americans over the age of 20. The purpose of this quantitative study was to determine whether there is a difference in the Quality of Life (QOL) for Congestive heart failure patients receiving care through telehealth compared to patients receiving face-to-face care (usual care). Guiding this project was the Self-Care Model of Chronic Illness because the primary outcome of the self-care model is illness stability, well-being, and quality of life. Seventy-seven veterans with Heart Failure (HF) from the Washington D.C. Veterans Affairs Medical Center (VAMC) participated in the project. Forty reported they …

The Watson Room: Managing Compassion Fatigue In Clinical Nurses On The Front Line, Crystal Denise Crewe 2016 Walden University

The Watson Room: Managing Compassion Fatigue In Clinical Nurses On The Front Line, Crystal Denise Crewe

Walden Dissertations and Doctoral Studies

The concept of compassion fatigue (CF) emerged in the early 1990s in North America to explain a phenomenon observed in nurses employed in emergency departments. A precursor to burnout, CF is a well-known phenomenon associated with emotional exhaustion, depersonalization, and an inability to work effectively. In nurses, CF has been shown to reduce productivity, increase staff turnover and sick days, and lead to patient dissatisfaction and risks to patient safety. The aim of this study was to determine if the use of a Watson Room designated as a 'quiet zone' with warm colors on the wall, massage chair, and soothing …

Nursing Distance Learning Course Comparison Of Assignments And Examination Scores, Jennifer Mundine 2016 Walden University

Nursing Distance Learning Course Comparison Of Assignments And Examination Scores, Jennifer Mundine

Walden Dissertations and Doctoral Studies

Nursing programs have embraced distance learning in their curricula, but discussion is ongoing about course assignments and grading criteria to increase examination scores in nursing distance learning courses. Because course examinations are a predictor of success on the postgraduate licensing examination (NCLEX-RN), the purpose of this study was to determine whether differences existed in student examination scores between nursing distance learning courses with and without points aligned to assignments. The theoretical framework was Knowles's theory of andragogy, which highlights adults' motivation and self-direction to succeed. The quantitative causal comparative study included a convenience sample of 164 students to compare archival …

Reducing Maternal And Child Morbidity And Mortality Through Project Recommendations, Dominique Valentin 2016 Walden University

Reducing Maternal And Child Morbidity And Mortality Through Project Recommendations, Dominique Valentin

Walden Dissertations and Doctoral Studies

Haiti is a Caribbean island with a humanitarian medical center providing healthcare services to 90,000 residents. Pregnant women visiting the medical clinic for prenatal care often do not return for delivery; instead, they return home to deliver alone or with the assistance of a traditional matron. Home-birth practices increase maternal-child health morbidity and mortality in an already fragile country. The purpose of this project was to gain a deeper understanding of Haitian pregnant women's preferences to deliver at home or at the healthcare clinic. The transtheoretical model for behavior change and the Johns Hopkins nursing evidence-based practice model guided the …

Walden Dissertations and Doctoral Studies

Asthma is a leading cause of pediatric hospital admissions. Parents of children under the age of 18 with asthma require education to recognize and manage the signs and symptoms of the disease. Parent education has shown to decrease their children's hospital admission and readmission rates. The purpose of this pilot project was to develop an asthma educational module for the parents of children with asthma and obtain parent feedback on the content. Families with children under the age of 18 who had been admitted to the hospital with a diagnosis of asthma, an asthma exacerbation, or status asthmaticus were invited …

Effectiveness Of Cognitive Screening For Heart Failure Patients, Comfort Nkengla 2016 Walden University

Effectiveness Of Cognitive Screening For Heart Failure Patients, Comfort Nkengla

Walden Dissertations and Doctoral Studies

Cognitive impairment is commonly seen in the elderly population. It is unclear if cognitive deficit in heart failure (HF) patients is a primary factor for higher hospital readmission rates in this population. The Centers for Medicare and Medicaid Services have established strict guidelines for reimbursement on readmissions that occur within 30 days. It is imperative that organizations identify and rectify issues that impact readmissions. The aim of this project was to determine if there is a reduction in HF readmission after patients are screened for cognitive impairment. Orem's self-care model guided the project by providing a framework of inquiry regarding …
