Sleep disturbance is common in patients with heart failure (HF) and the family caregivers. Sleep disturbance is known as a predictor of poor quality of life (QoL) in individual level. The manner in which patients’ and caregivers’ sleep disturbances influence each other’s QoL has not been determined. Objective To develop and evaluate a best practice model of general hospital acute medical care for older people with cognitive impairment.

BACKGROUND: Although successful aging is most often defined by the absence of disease and disability, older adults consistently report aging successfully even in the presence of chronic illness and functional limitations. A more holistic way of looking at old age suggests transcendence may be an important missing criterion for successful aging.

The purpose of this pilot study was to explore the relationships between nursing staff knowledge of extended care resident personhood and resident cognitive status. Eighty nursing staff from two similar extended care facilities in Arkansas completed a self-administered questionnaire to assess their knowledge of 21 residents, including information related to residents’ past jobs/careers; likes/dislikes/ interests; family members; other non-specific knowledge; and total knowledge. The Brief Interview for Mental Status (BIMS) was used to evaluate resident cognitive impairment. It is estimated that the incidence of pressure ulcers range from 0.4 to 38% in a hospital setting. Overall prevalence in the U.S. was 12.3% in 2009. A CMS analysis reported that pressure ulcers were responsible for over $2.41 billion dollars (Niederhauser et al., 2012). A large body of literature related to pressure ulcer incidence and prevalence reflects that education is not always the principal influencing factor for prevention; social and organizational barriers are just as critical when adapting clinical practice guidelines. In the United States, the number of older adults will double during the next 25 years (United States Census Bureau, 2008). This dramatic demographic shift is changing the landscape of health care practice as more people live longer with multiple chronic conditions. To better prepare nurses to care for this future population, the John A. Hartford Foundation partnered with the American Academy of Nursing in 2000 to launch the Building Academic Geriatric Nursing Capacity (BAGNC) program. A growing body of nursing research addresses the transitional period for older adults entering long-term residential care. However, gaps exist in our understanding of nursing’s role in the transition process. Our aim is to examine the available evidence and develop a model of nursing strategies and interventions to ease the transition to long-term care. Objectives: The study aims to describe the characteristics of patients presenting to an ED with a fall and evaluate multidisciplinary Care Coordination Team (CCT) referrals on patient outcomes.

Methods: A single-centred retrospective analysis of electronic data at an adult tertiary hospital was performed using data from 2004 to 2009 of presentations for patients aged 65 years or over with a fall. The primary outcome measure was representation to hospital within 30 days, comparing patients referred to CCT and those not referred. Secondary outcomes were differences in demographic characteristics, mode of arrival, triage score and readmission.

Between 2000 and 2025 there will be a 30% increase in the prevalence of Alzheimer’s disease (AD) in Indiana (Alzheimer’s Association, 2010). Many individuals with AD and related dementia are cared for in long term care communities. Adequate knowledge and expertise of nurses caring for older adults, particularly those with dementia, is important for the delivery of quality care and the promotion of quality of life for residents living in long term care. This pilot project examined the benefit of a resource support program to encourage Gerontological Nursing certification for registered nurses in the long term care setting.

Dementia and obesity are significant public health concerns. Alzheimer's disease affects 5.3 million adults, while 72.5 million adults are obese. Emerging evidence linking body mass index (BMI) and dementia suggest that, although a high BMI in midlife is associated with a greater risk for dementia, a high BMI in late life is considered protective and should not necessarily be considered a risk factor for dementia. Beginning in midlife, practitioners should trend patient BMI numbers. We build on the current consistent vehicle routing problem literature by formulating a novel multi- objective mathematical model of the home health scheduling and routing problem that includes the option of assigning some patient visits to remote monitoring devices, with the objectives of mini- mizing total cost, achieving nurse consistency and creating balanced nurse workloads. Hip fractures in elderly patients have very high postoperative mortality rates and the number of hip fractures in the United States is expected to increase exponentially before 2030. Early surgery is essential to improve outcomes, but patients on clopidogrel (Plavix) have a high risk for increased bleeding if surgery is performed within the usual 72 hour window. This paper presents literature and research addressing the dangers of delaying surgery versus undergoing surgery before the effects of the clopidogrel are gone.
