Mental health services in the United States were designed for European American consumers, but with burgeoning multicultural populations these services have proven inadequate and underutilized. This paper examines research on cultural competencies of agencies and clinicians relevant to a mental health practice model, the Multicultural Assessment-Intervention Process model (MAIP). This model was modified for systematic application in a California agency, the Tri-City Mental Health Center, to provide a flexible blueprint for major alterations in agency practice and programs that affect the entire system of care. This review examines the literature on narrative and trauma in order to summarize and integrate findings on the role of narrative in adaptation to trauma. First, an introduction to the trauma literature, narrative theory, and how narratives have been studied in the context of psychotherapy is presented. Then, studies examining the characteristics of trauma narratives and their relationship to the resolution of trauma are presented and critiqued. Then, important methodological issues and the various measures used to study narratives in therapy and trauma and its sequelae are discussed. Abstract: Hospital closures occurred nationally with increasing frequency between 1970 and 1990. In particular, large urban areas experienced a dramatic number of closures. Of the 61 general hospitals operating in Chicago in 1970, 22 (36%) had closed by 1991. While a growing body of literature has examined the etiology and determinants of hospital closure over the last two decades, few empirical studies have focused on the neighborhood correlates of closure, and none have examined specific health outcomes associated with hospital failure. Using Professionally Trained Actors In Social Work Role-Play Simulations, Helen E. Petracchi

The literature available to social work educators contains limited discussion of the use of role play simulation in the classroom. This paper presents the experiences of first-year MSW students in a foundation generalist practice class who were evaluated using role play for their final examination. In a unique arrangement, clients in these simulations were role-played by firstyear acting students from the university's theater arts department. The reaction of the social work students to role play with professionally trained actors is described and discussed from voluntarily submitted descriptions of the experience.

Adolescent sex workers potentially have many health needs but are included rarely in health assessments; therefore, little is known about their needs or how to reach them. This study examines the motivations, social context, and health needs of street-based adolescent sex workers. The results show these youths are more likely to experience negative health outcomes, such as unintended pregnancies and sexually transmitted infections, when they are using drugs or are depressed. These findings suggest the need for comprehensive programming to include individual, interpersonal, and greater environmental interventions. Examples for reaching adolescent sex workers are discussed.

Objective: To assess the association between physical violence during the 12 months before delivery and maternal complications and birth outcomes.

Methods: We used population-based data from 6143 women who delivered live-born infants between 1993 and 1995 in South Carolina. Data on women's physical violence during pregnancy were based on self-reports of partner-inflicted physical hurt and being involved in a physical fight. Outcome data included maternal antenatal hospitalizations, labor and delivery complications, low birth weights, and preterm births. Odds ratios and 95% confidence intervals were calculated to measure the associations between physical violence, maternal morbidity, and birth outcomes.

Rehabilitation For Deafened Adults: A Puzzle With Missing Pieces, Miguel Oswald Aguayo

This study explored the psychological and social impact of adventitious deafness, and the rehabilitation services that were offered to individuals who acquired this condition in adulthood. Qualitative interviews were conducted with a sample of eight deafened adults, who were recruited from across Ontario. The results show that medical interventions (e. g., audiometric testing, hearing aid provision, and cochlear implantation) were relied on exclusively as a rehabilitation approach in providing care to deafened people. None of the study participants were offered individual, family, or group counselling by their hearing health service providers. This lack of attention to the socio-emotional needs of …

The purpose of this study was to determine whether women with breast cancer and the presence of the drug tamoxifen, are more severely depressed than women with breast cancer and the absence of tamoxifen. Average Beck Depression Inventory (BDI) scores did not differ between the two sub-groups; however, two significant group differences were found. First, 10 women in the tamoxifen group had scores of zero (versus 4 in the absence of prescribed tamoxifen group). Violence during pregnancy directly impacts the mental and physical health of pregnant women. We assessed the prevalence and correlates of physical violence around the time of pregnancy in a representative sample of 6,718 women in South Carolina. Physical violence, defined as "being physically hurt by husband or partner" or "being involved in a physical fight" was reported by 10.9% of recently pregnant women. These were correlates of violence: experiencing increased numbers of stressful life events, being unmarried, having increased parity, being on Medicaid, and having an unwanted pregnancy. Violence against women is a significant public health problem which impacts women, men, and children. Little is known about the frequency or correlates of violence against women in Africa. In this cross-sectional study, we found that 66.7% of 144 women surveyed in a study of AIDS knowledge, attitude, and behaviours, report being beaten by an intimate male partner and 50.7% report having ever been forced to have sexual intercourse; 76.6% of women report either forced sex or intimate partner violence. Circumcised women were most likely to report intimate partner violence and forced sexual intercourse. To improve the health of women …

The issues revolve around the impact of varying social conditions, changing ethos, differential respondent receptiveness, and evolving research technology. Despite the efficacy of community-based treatment, many emotionally and behaviorally disturbed youth still receive restrictive service placements. This is quite problematic for minority youth since studies have reported they are more at risk of out-of-home placements. The purpose of this study was to examine differences in referrals, out-of-home placements, residential placements and length of service for African-American and Caucasian youth. This historical cohort study was a secondary analysis of data from the Comprehensive Services Act in Virginia. The sample consisted of 2,883 youth ages 10-19. The massive influx of Indochinese refugees and immigrants to North America since the end of the Indochina war, especially to the United States of America, has resulted in numerous multi-disciplinary efforts to document and study their mental well-being. As a group, Indochinese Americans arrived from war-torn countries where many had experienced various forms of trauma, poverty, and oppression.
