A Study Of Local Organisations In Mine Action, Gichd
Global CWD Repository
This Study of Local Organisations in Mine Action, which was requested by the United Nations Mine Action Service (UNMAS), is an assessment of the successes and failures of local mine action organisations, including their competence, effectiveness, efficiency and sustainability. It examines how, and to what extent, national governments have encouraged and supported such organisations, and whether they have given priority or preferential treatment to such rganisations in issuing contracts or funding. It examines how much external assistance has been needed to reach the necessary standards of competence, and whether the newly-formed organisations have been able to implement national standards derived …

A BM-12 smoke shell was reported near the Safe UXO Store of [Demining group] teams working in Logar province. The acting TL briefed the SL to take the UXO away and demolish it. Mr. Paya-I-Din was climbing a hill while taking the UXO to the demolition pit, slipped and the accident happened. It is worth-mentioning that the UXO he was carrying did not explode and apparently a BD RPG7 fuse caused the injuries.

The primary cause of this accident is listed as a “Field control inadequacy” because it seems that the Victim was burning off an area that needed to be cleared and did not maintain an appropriate safety distance when the fire might have initiated a mine/device. The secondary cause is listed as a “Management control inadequacy” because the management of the demining group declined to make the accident details available. Although this is sometimes done to protect the Victims, in this case the Victim’s name was among the limited detail made available. It is possible that the managers have chosen to …

The Victim marked a detector reading and when he investigated it, found some fragments. He put the detector aside and moved to collect stones to construct a Turning Point and mark the cleared survey lane. During supervision of demining activities in Shin Dand district of Farah Province, Victim No.1 found an unknown device (probably a warhead) on 10th April 2000. He wanted to investigate the device further, so took it to Herat. The next day, together with a “radio mechanic” he took it into the office and attempted to defuse and dismantle it. It exploded in the office. The two men died immediately.

The accident occurred on land described as “agricultural, soft ground”. The demining group was engaged in Surveys (Levels 1 and 2). He found mines one on each side of the boundary lane and showed these to the supervisor. He was told to move the markers indicating the safe lane. As he turned, he stepped on a missed mine that was between the other two.

The Victim was working in a breaching party making a lane to the top of the hill. He had been working for 20 minutes when he registered a detector reading. He then either moved to mark the reading and stood on the mine or marked the reading and stepped back, stepping on the mine.

The investigators determined that the victim was investigating a detector reading wearing his helmet and in a squatting position as he and his partner cleared an uphill breach. The investigators determined that the victim and his partner were clearing a lane into the edge of a canal that was partly filled by a collapsed wall. There was a sheet of metal partly buried in the canal so the victim started to clear the earth away from it with a pick "layer by layer". At 08:45 he detonated a mine with the pick and sustained injuries to his eyes and hands.

Two dogs were run over the accident site and both signalled positively. The victim started searching with a detector where they had indicated and he got a reading. He marked the spot and started to prod in a "prone" position. As he approached the second marker he paused to remove the loosened earth with his hands. [It is not clear whether he raised himself up to do this.] As he removed the soil a device exploded, amputating his thumb and two fingers on his right hand. The victim stood up after the accident.
