On 06 Sep 2010, when Mr. [the Victim] was working in his clearance lane and excavated a detected signal, during the excavation drill his excavation tool touched the mine and caused it to go off. On 1st of June [Demining group] started clearance operation on mentioned task, they cleared 3350 sqm and found/destroyed 150 anti-personnel and 90 UXO till the accident time. On 23 June 2009 at 08:40 am when [the Victim] was working in his clearance lane a PMN2 antipersonnel mines went off and caused the accident. It is the conclusion of investigation team that the involved deminer was excavation a signal in an area where the ground surface was hard with dense vegetation, and considering the crater created as a result of the explosion it seems that the deminer was excavating the signal about 30 cm on right side of the working lane where he was not in a stable position for excavating the signal by scraper; the excavation of this lane required to be done in next clearance lane.

On the day of the accident at approximately 11:45hrs, the Victim was commencing a clearance lane up the side of a hill. He was investigating a reading from his metal detector by prodding, and was in the kneeling position at the time. Whilst prodding he detonated a PMA3 anti-personnel blast mine.

The investigators determined that the deminer was "excavating" to investigate a detector reading when the accident occurred. He was using a shovel to do so. Investigators were unable to visit the accident site [due to deminer mobility and seasonal redeployment], so the report relied on interviews and written statements.

The investigators determined that the victim was squatting to prod with a bayonet and applied too much pressure. He used the presence of bamboo to explain why he squatted to prod. The mine was identified as a PMN from "found fragments". His visor was shattered in the accident and his bayonet was "lost".
