Task# 132 is part of security belt of Bagram Regiment -40 which is contaminated by AP mines. Deminer [the Victim] using a hand trowel excavated onto an AP mine and caused it to detonate. As a result of this detonation he suffered minor injuries to his right hand and more serious to his face and both eyes with serious corneal defect to right eye, first degree burns to his face and ear damage.

After he excavated a signal and found nothing he went to the safe area for rest and when he was coming back on the lane that was cleared by him, suddenly the explosion occurred. A mine blast incident occurred at approximately 08:35hrs on Sunday 01/07/2007 in the minefield 69270030. The deminer detonated a M35 Belgium anti personnel blast mine due in direct contact to the top of the mine. At the time of the incident the deminer was wearing the PPE (Body vest and Goggles). Deminer tried to lift the top soil crust by using heavy rake, The rake slipped and struck the mine resulting in mine blast. The deminer sustained no injury.

On 18 June 2007 at 1140 hrs (local time), [the Victim] a (Commercial demining group) Deminer, inadvertently caused an item of explosive ordnance (EO), which is considered to be a No. 4A anti-personnel mine, to activate, while conducting the manual mine clearance full excavation procedure. As a result of the explosion [the Victim] was thrown backwards by the blast wave and suffered a traumatic amputation of the right hand, a fractured right humerous and critical head and eye injuries.

On 04 June 2007 at 1103 hrs while the deminer was working in his clearance lane, his trowel struck the top of a PMN anti personnel mine, causing the mine to detonate. According to the AMAC Northeast’s investigation report, the accident occurred while the deminer was excavating on a reading in a cross lane (cut lane). As a result of this accident, the deminer received some light injuries on both hands. According to the Assistant Team Leader, the mine position had changed and the deminer’s bayonet touched the top of the mine and detonated.

A. On 19 April 2007, while the [International demining NGO] loader (MDU # 9) was excavating the ground by using Gill bucket and then the soil inspecting for Antitank mines and UXO but an antipersonnel YM-1 mine has passed through the grids of the Gill bucket and missed from inspection. While the Loader has been levelling the loose dragged soil, in reverse position the front left was tyre driven on the mine and exploded it.

On 19th April-07 at 08:10 hrs while the Deminer was excavating the area by pickaxe suddenly an anti personal mine has hit by pickaxe and exploded caused the accident and Deminer got superficial injuries on his right arm and neck. During our observation from the accident point it was seemed that the Deminer was working with pickaxe not by bayonet/prodder, because the pickaxe handle was available in the accident point.

On 10 April 2007 at 1155 hrs while deminer [the Victim] with employ No. 4094 was working in his clearance lane after 3 m progress in the lane his trowel hit top of a PMN-02 anti personnel mine, as a result the mine exploded and the deminer sustained some injuries. Since the deminer was fully dressed with PPE and down visor, the deminer injuries are superficial. The victim right hand wrist and two fingers were injured.

On 24 May 2007, in this area while the SITA construction company was levelling the roadside (road shoulder), the top of a TC-2.4 (Italian Antitank) mine has been appeared. The company’s representative reported that case to the AMAC and the AMAC sent the QMA to the area for further investigation. The mine found 80 Cm from the paved road in had 90 Cm depth. According to local reports, after mine plantation in this area, about 100 Cm extra soil has gradually aggregated over the original surface of the ground.

The deceased deminer detonated the PMN mine with an unidentified object. He was in a kneeling position with his right side facing towards the mine. The blast struck him on the chest and right hand side, causing a fatal wound to the right under arm (armpit). He suffered extensive bleeding from this wound and also from the mouth and nose at the time. He was evacuated by ambulance but died en route to the hospital.

A mine blast incident occurred at approximately 07:27hrs on Monday 05/02/2007 in the lane No-2 in the minefield 71390013. The deminer detonated a No-10 Israeli anti personnel blast mine due to hacking. At the time of the incident the deminer was wearing the PPE (Body vest and Goggles). The 26th of January 2007, approx 12.25 (local time SL), the HQ in Jaffna was called up by the Team leader of Team 5, working in a heavy dense (P4s) minefield, LK-23B.The message he gave was that one of his deminers had set one mine of in his lane while conducting mine clearance. The deminer was clearing a lane of the minefield (69270024) when he initiated a mine with his Heavy rake and the solidified layer of soil [crust] slid onto the mine. He did this by raking in the site and trying to remove the solidified layer. The mine was approximately 5cm deep and set into hard baked soil.

On 29 Oct 06 an accident involving a staff member of KEC (The Indian contractor currently engaged to refurbish the power-line) detonated a PMN2 AP whilst he was excavating at internal base of Tower No 60. Towers 60 and 62 are located in what was addressed as MF Code 01-0103-11-041 during the 1996 clearance. The [name removed] survey data of the time corroborates the information supplied by Mr [name removed] that some towers were considered clear and others were considered to be mined.
