Background and Objectives

Intentional weight loss, by reducing insulin resistance, results in both better glycemic control and decreased need for anti-diabetic medications. However, not everyone who is successful with weight loss is able to reduce anti-diabetic medication use. In this retrospective cohort study, we assessed the predictive accuracy of baseline triglyceride (TGL)/HDL ratio, a marker of insulin resistance, to screen patients for success in reducing anti-diabetic medication use with weight loss.

Methods

Institute for Veterans and Military Families

This study investigates how the severity of PTSD symptoms relates to employment and earnings, it found that Veterans with more severe symptoms of PTSD, were more likely to work part-time or not at all rather than have full time work. In practice, treating PTSD, even if some symptoms remain, could result in better employment outcomes for veterans. In policy, Policy makers should consider providing previously successful programs for veterans with PTSD for individuals with other mental health conditions. This brief is about the link between childhood trauma and servicemember suicide risk within the US Army. In policy and practice, communities should be willing to discuss abuse with servicemembers who experienced childhood trauma, healthcare providers should tell veterans and servicemembers about their treatment options, and family members should assist with the mental health treatment of veterans and servicemembers. Policymakers should create programs that encourage social networks and increase the number of veteran and military support groups. Institute for Veterans and Military Families

This brief is about weight change among veterans before, during, and after discharge from the military. In policy and practice, physicians should discuss strategies to prevent unhealthy weight gain with veteran patients; the VA should better tailor its MOVE! weight management program to younger veterans. Suggestions for future research include looking at the reasons behind weight gain in discharged servicemembers, analyzing the strategies used by members of the Reserve/National Guard to limit weight gain, and using more samples of younger veterans to generalize results.

Following several political upheavals, the country of Madagascar has become one of the poorest countries in the world. This level of poverty affects many aspects of life, especially access to healthcare services. The availability of both allopathic and traditional healthcare to the impoverished citizens of Madagascar was examined through discussions, interviews, and journal articles. It was found that although both the allopathic and traditional medicinal systems do not charge their patients for general consultations and care, there is a high cost for pharmaceutical medications in the allopathic system. These medications are sometimes too expensive for many Malagasy patients to afford. …

We report the development of a prototype Decision Support Tool (DST) for modelling the risks of spreading of non-indegenous invasive species via ballast water. The DST constitutes of two types of models: A 3D hydrodynamical model calculates the currents in the North Sea and Danish Straits, and an agent-based model estimates the dispersal of selected model organisms with the prevailing currents calculated by the 3D hydrodynamical model. The analysis is concluded by a post processing activity, where scenarios of dispersal are combined into an interim estimate of connectivity within the study area. The latter can be used as a tool …

Objective: To assess the extent to which the observed racial disparities in cardiac revascularization use can be explained by the variation across counties where patients live, and how the within-county racial disparities is associated with the local hospital capacity.

Data Sources: Administrative data from Pennsylvania Health Care Cost Containment Council (PHC4) between 1995 and 2006.

Study Design: The study sample included 207,570 Medicare patients admitted to hospital for acute myocardial infarction (AMI). We identified the use of coronary artery bypass graft (CABG) and percutaneous coronary intervention (PCI) procedures within three months after the patient’s initial admission for AMI. Climate change requires action at multiple levels of government. We focus on the potential for climate change policy creation among small rural governments in the US. We argue that co-production of scientific knowledge and policy is a communicative approach that encompasses local knowledge flowing up from rural governments as well as expertise and power (to coordinate and ensure compliance) flowing down from higher level authority. Using environmental examples related to land use policy, natural gas hydro-fracturing, and watershed protection, we demonstrate the importance of knowledge flows, power, and coordination in policy creation. For this study, we conducted 7 focus groups in the Los Angeles area with a total of 70 participants (42 Latinos and 28 African Americans) recruited from 3 senior centers and a church. Overall, participants were not well prepared for the future in terms of their health care financing. African Americans in the study tended to participate more in the formal financial sector and show a high level of sophistication when managing their finances than Latinos. African Americans also were more likely to save than Latinos, but their level of saving was not very large. We also find that participants …

This research project consists of an investigation of responses to homeless encampments on rights-of-way owned by Departments of Transportation (DOTs). While DOTs are not housing or social service agencies, their role as major public landowners involves them in dealing with the consequences of homelessness. The research goals included analyzing the prevalence of the problem, documenting how DOTs are responding, and culling from this data information that could be used as a basis for creating a best practices guide.
