Consumer direction, a model of long-term care service delivery where service recipients and their families/advocates have enhanced control and choice over the services that they receive, is a growing phenomenon in United States. As it becomes greater utilized, it is important to understand the model and study its impacts. This dissertation details the history of the consumer direction movement, describes the current landscape of consumer direction in the United States, and presents a comparison study of users of consumer-directed (CD) services in three of Virginia Home and Community Based Services (HCBS) Medicaid Waivers. Like the private managed care plans offered under Medicare Advantage, private fee-for-service (PFFS) plans are paid more per beneficiary than those individuals would be expected to cost if they were enrolled in traditional fee-for-service Medicare. However, PFFS plans are not required to provide the same type of coordinated care required of Medicare Advantage plans. Payments to PFFS plans in 2008 average 16.6 percent more than costs in traditional Medicare, or $1,248 for each of the 2 million enrollees in PFFS plans—a total of nearly $2.5 billion in extra payments. Accurate measurement of digital divides is important for policy purposes. Empirical studies on broadband subscription gaps have largely used cross-sectional data, which cannot speak to the timing of technological adoption. Yet, the dynamics of a digital divide are important and deserve study. With the goal of improving our understanding of appropriate techniques for analyzing digital divides, we review competing econometric methodology and propose the use of duration analysis. We compare the performance of alternative estimation methods using a large dataset on DSL subscription in the U.S., paying particular attention to whether women, blacks, and Hispanics catch up to others in …

This presentation begins with a review of the overall funding structure for early childhood education and care in Ireland. It moves on to look in detail at the impact of the changes to the social inclusion funding measures. It concludes with a review of the reaction of various Irish stakeholders to these changes that have resulted in a 'missed opportunities for children'.

The purpose of this study is to explore the use of interstate inmate transfers (IITs) by prison wardens and the administrative intent that guide their use. This study assesses the explanatory power of the new penology in three cases and asks three broad questions of two prison wardens and the DOC: What correctional goals do you hope to accomplish with interstate inmate transfers? Why? And what contextual factors (if any) are felt to inhibit or facilitate these goals?

This report represents a "second wave" follow-up to a "first wave" study whose purpose was to measure the effects of the Deficit Reduction Act's citizenship documentation requirements on health centers and their patients. The earlier study, conducted six months after implementation, found that the law had a widespread impact, including delayed applications, interrupted enrollment, disruptions in care, and at least anecdotal evidence of a growth in the number of uninsured patients as a result of the denial or loss of Medicaid coverage. This "second wave" survey underscores the existence of serious, ongoing problems more than a year after implementation. Specifically, …

With health care costs increasing, some policymakers have sought to make patients better health care consumers through increased cost-sharing linked with greater information on the cost of care. These may be successful cost containment strategies in the short term. But patients are just as likely to forgo necessary as unnecessary care, which ultimately leads to greater demand for more intensive and expensive care in the long term. Patients can, however, play an important role in preventing the onset of chronic conditions or preventing deterioration in health once they have been diagnosed with a chronic condition. The article discusses various aspects of ancestry-based drug prescribing and contemporary medical ethics in the U.S. Ancestry has been used by researchers to identify a subgroup of people most likely to have a certain allele linked to an adverse drug response. Also discussed are carbamazepine labeling, the U.S. Food and Drug Administration's (FDA) Alert on drug prescribing based on allele testing and the ethics of genetically categorizing people.

his paper seeks to explore the conflict existing between two ethnic groups, the Bali-Nyonga and the Bawock, as a case study for inter-ethnic land dispute. Through the process of this study the researcher tried to look past arguments of logic in order to uncover the struggle for autonomy, the struggle for respect, and how damaging the muddy relationship between tradition and modernity can be. She found that ultimately models of mediation and dialogue were the most constructive in helping the conflict’s actors heal among themselves, and to develop the tools for interacting with a globalized society.

By studying how the American-Vietnam War is taught in high schools and universities across Vietnam, the official government policies regarding the memory of the War can be uncovered. The Vietnamese Government completely controls the education system; therefore, the government is able to control the information that is taught to future generations regarding the War. What does Vietnam’s Government want its people to remember? And what facts does it want its citizens to forget? Looking at the government-approved “message” that is present in every Vietnamese textbook and history lesson, one is able to find the answers to these questions.

Increasing incidents of incivilities and in some cases outright violence are well documented in K-12 through undergraduate educational setting as well as in the workplace across all sectors, yet the academic and professional literature that reflects and informs public affairs education is striking in its omission of how incivilities necessarily impact our teaching and program administration. The Federal Highway Administration (FHWA) has set a high priority on the use of existing dynamic message signs (DMS) to provide travel time estimates to the public. The Oregon Department of Transportation (ODOT) currently has three DMS in the Portland metropolitan area configured to display travel time information. In the near future, ODOT would like to make travel time estimates available on additional DMS, over the Internet on tripcheck.com and via 511. Travel time estimates are valuable to the traveling public; however, the estimates must be accurate to be useful. The FHWA indicates that 90% accuracy is ideal and suggests …
