With approximately 22 people dying due to opiate-related harms everyday in Canada (Government of Canada, 2023), there is a great need to offer low-barrier, evidence-based services for people who use substances across various disciplines, including healthcare, social services, and psychotherapy. This qualitative study examined the experiences of Canadian social justice-informed counsellors who work with clients who use substances from harm reduction frameworks of care. Seven counsellors were interviewed on their personal experiences implementing harm reduction in psychotherapy. Personal beliefs and values conjoin with professional training to influence clinical practice. This article examines the role of religion and spirituality (R/S) through the lens of the author’s personal experiences and illustrates the confluence of faith, belief, identity, and practice in professional life. An autobiographical “glimpse” introduces the author’s formative experiences as a Roman Catholic and illustrates how religious narratives furnished conceptions of suffering, forgiveness, and transcendence that contributed to authentic hope for the client. Although often seemingly silent, R/S may influence psychotherapy practice. Clinical supervision provides a context to examine these personal factors.

Marginalized groups are less likely to seek out mental health services than non-marginalized groups. There are various reasons why marginalized groups, such as Black Americans, are less likely to seek out mental health services, one of which is the cultural barriers between a clinician and client. Research suggests that Black Americans feel that clinicians struggle to overcome these cultural barriers. The Self Determined Learning Model of Instruction is an evidence-based intervention for supporting self-directed goal setting and problem solving. Traditionally, the SDLMI has not been applied in the psychotherapeutic context, however we propose that the SDLMI is an approach that could be integrated into such a context to support self-determination, goal setting, and goal attainment. I conducted a meta-method study to explore the methodological and reporting characteristics of qualitative studies on therapists’ experiences conducting psychotherapy. Articles were identified through a PsycINFO search, and through a review of article text their methodological and reporting features were coded and quantitatively analyzed. Consideration was given to standards of qualitative research in psychology, especially methodological integrity. Results showed increases in the number of these qualitative studies from the 2000s onwards. This rise seems to be above that in psychology, but comparable to other psychotherapy literature. Current North American public discourse is strongly influenced by widespread sociopolitical divide. Given that sociopolitical events seem to permeate conversations in the therapy room (Farber, 2018; MacLeod, 2013), it is essential that therapists are prepared to navigate conversations with clients who have beliefs different from their own. Researchers have demonstrated that therapist attachment is associated with therapeutic relationships and client outcomes (Steel et al., 2018). Yet, results have varied and, at times, seemed contradictory. This study explores how therapist attachment might influence the working alliance and symptom change throughout psychotherapy and uses the recently validated, transcript-based Therapist Attunement Scales (TASc; Talia & Muzi, 2017). The TASc was developed to measure in-session attachment-related verbal behavior. The third session from each pair of 23 therapists-in-training and their clients was transcribed and coded. Clients completed the OQ-45.2 and the Working Alliance Inventory prior to each session.

This study examines the social and cultural considerations in accessing mental health treatment in The Gambia, West Africa. Participants were recruited from The Gambia for a qualitative study that included semistructured interviews (N = 17). A team of analysts identified five themes. The results highlighted social and cultural conceptualizations of mental health and mental illness, sociocultural determinants of health, interventions, barriers to care, and the legal framework to support mental health change. These findings are important for counselors to understand different perceptions of mental health and mental illness and the associated stigma. The current study examined the effects of classist beliefs on trainee attitudes toward their client based on perceived social-class status. This study sought to determine whether classist attitudes contribute to meaningful differences in clinical decision making. A sample of mental health trainees (n = 147) attending graduate-level programs in the U.S. were recruited and randomly assigned to one of two clinical vignette conditions. Both vignette conditions included identical data regarding a hypothetical client’s presenting concerns (e.g., sleep disturbance, worry, rumination, loneliness), but differed on indicators of client socioeconomic status (SES). While this has been impactful, there is evidence that cultural ruptures, microaggressions, and racial/ethnic disparities in psychotherapy outcomes persist. Aversive racism theory, which provides explanations for the racist tendencies typically associated with progressive White individuals, postulates that a conflict between explicit egalitarian beliefs and implicit negative racial biases impedes White individuals from adequately addressing and acknowledging underlying biases. Ruptures in the therapeutic alliance have shown to be a significant predictor of therapy outcomes, with non-repaired ruptures resulting in increased dropout rates of therapy, and repaired ruptures resulting in positive therapy outcomes. This study investigated the impact of alliance ruptures on outcomes of therapy among youth and adolescents, with a specific focus on military youth and adolescents. Strengthening therapeutic alliance through social identity matching has been a strategy used to reduce psychotherapy dropout among racial/ethnic and sexual minority clients. Limited research has examined social identity match by manipulating social identity (e.g., race/ethnicity, sexual orientation) in an analogue therapy setting. This article explores the role of therapist self-disclosure in clinical settings. Distinctions are made between the enmeshed concepts of privacy, secrecy, and confidentiality to elucidate the role of ignorance in maintaining the power dynamics in therapeutic relationships. While some measure of privacy is essential to counseling practice, secretive behavior (in which the counselor divulges too little about themselves) can have a negative impact on the therapeutic relationship and the client’s therapeutic outcomes. Taking a humanistic and existential counselling stance, this brief review of heroism and the heroic experience begins by discussing the utility and structure of Joseph Campbell’s (1949) monomythic narrative of the hero’s journey, whilst considering the Jungian conceptualisation of the archetype and the collective unconscious. With their shared assumptions about transformation and growth, modern psychology and the therapeutic practice of counselling and psychotherapy are reviewed in terms of their utilisation of the hero-journey as a developmental metaphor for clients, particularly in trauma recovery. The aim of the current study was to examine client perceptions of the use of mindfulness activities in the therapeutic process. For this current study, 21 participants completed a seven-item demographic questionnaire, a 39-item Five-Facet Mindfulness Questionnaire (FFMQ), and a 10-item Mindfulness Experience Questionnaire. The present study investigated three research questions. Research Question 1 looked at how clients perceived the effectiveness of the use of mindfulness activities in therapy. Although therapist absences undoubtedly impact the therapy process, the topic has not received sufficient attention to produce helpful guidelines.
