Determining The Drivers Of Quality Of Life In Hiv Patints Treated With Antiretroviral Therapy In Subsaharan Africa, Oubote Sangbana

Dissertations & Theses (Open Access)

During the last decade, HIV in sub-Saharan Africa has shifted from a fatal disease to a chronic condition thanks to the expansion of antiretroviral therapy (ART). As a result, the focus of care has transitioned from survival to quality of life (QoL), and the latter has become a significant outcome measure during patient follow up. Yet, the determinants of QoL among HIV patients in western and central Africa are not well known. The objective of this dissertation was to determine the main factors associated with QoL among ART-treated HIV adult patients in western and central Africa to ensure healthcare providers …

Statistical Evaluation Of An Extension Of Linear Discriminant Analysis In Multiclass Settings With An Application For Detection Of Cervical Neoplasia, Frances A. Brito

Dissertations & Theses (Open Access)

Although a pathologists’ review of Papanicolaou smear cell samples has been successful in decreasing cervical cancer incidence, it is often costly and time-consuming. Quantitative cytology is a promising semi-automated method that measures cell features for further analysis or classification. There have been several advancements in classification algorithms, but many do not account for the nested data structure seen in quantitative cytology. Further, histologic diagnoses are separated into five or more classes, yet, multiclass classification has not been investigated. Here, we compare the predictive performance of macrolevel discriminant analysis (MDA) to traditional discriminant analysis methods in multi-class settings on cervical quantitative …

Utility Of Plasma Biomarkers In The Validation Of Reported Loc & Pta For Gcs 13-15 Patients: A Track-Tbi Pilot Study, Jeffrey Brennan May 2020

Utility Of Plasma Biomarkers In The Validation Of Reported Loc & Pta For Gcs 13-15 Patients: A Track-Tbi Pilot Study, Jeffrey Brennan

Dissertations & Theses (Open Access)

Depending on a brain injury patient to accurately report their duration of unconsciousness and amnesia is clearly flawed. We constructed biomarker-based models to validate these self-reported measurements using a GCS 13-15 subset of participants from TRACK-TBI, a multi-institutional study on traumatic brain injury. Potential covariates were assessed for significant interactions with biomarker level. We report that the predictions of LOC and PTA failed. GFAP exhibited the most consistent serum level increase between categories of both LOC and PTA. Given a number of issues with the study sample, we recommend continued investigation on the prediction of LOC and PTA using a …

Physician Hospital Arrangement Influence On Nonprofit Hospital Quality,Financial Performance And Community Benefit Expenditures, Andrea Cervin May 2020

Physician Hospital Arrangement Influence On Nonprofit Hospital Quality,Financial Performance And Community Benefit Expenditures, Andrea Cervin

Dissertations & Theses (Open Access)

Federal policy has encouraged hospitals to provide certain quality outcomes and cost containment. Hospitals have responded by forming physician hospital organizations in attempts to achieve quality outcomes and capture reimbursements. At the same time, federal policy has awarded hospitals with nonprofit status with the requirement to provide community benefits in exchange for the tax exemption they receive. With the formation of physician hospital organizations, there remains the question of whether hospitals are achieving desired quality metrics and whether hospitals are financially viable. There is also public and media interest recently regarding whether nonprofit hospitals fulfill their obligations for community benefit …

Cancer care is changing rapidly. Understanding of the increasing subtypes of cancer and exponentially increasing therapeutic interventions are unprecedented due to the rapid pace of scientific discovery and clinical innovation. This immense change within the field, lends itself to quality control initiatives, especially among general oncology providers who see a wide array of cancer types as general oncologists will see many different tumor types, and most of which have several potential treatment choices that have grown over time. Evidence-based pathways are an effective way to nudge quality control by presenting choice architecture at the point of care to facilitate guideline …

Statistical Methods For Functional Annotation-Based Rare Variant Association Analysis, Yiding Ma May 2020

Statistical Methods For Functional Annotation-Based Rare Variant Association Analysis, Yiding Ma

Dissertations & Theses (Open Access)

Despite ongoing large-scale population-based whole-genome sequencing (WGS) projects such as the TOPMed program, WGS-based association analysis of complex traits remains a tremendous challenge. External biological knowledge, such as functional annotations based on the ENCODE, Epigenomics Roadmap and GTEx projects, may be helpful in distinguishing causal rare variants from neutral ones; however, each functional annotation can only provide certain aspects of the biological functions. Our knowledge for selecting informative annotations a priori is limited and incorporating non-informative annotations will introduce noise and lose power. In the first part of this dissertation, we propose FunSPU, a versatile and adaptive test that incorporates …

A Mixed- Methods Examination Of The Healthcare, Social Support, And Religious Needs Of Sexual And Gender Minority (Sgm) Older Adults In Texas, Jennifer Lynn Holcomb

Dissertations & Theses (Open Access)

Shared historical and social contexts influence a generational group’s needs, risk factors, and resiliency mechanisms. Sexual and gender minority (SGM) older adults have experienced structural exclusion and systematic discrimination throughout their lifetime. The minority stress model posits that SGM individuals experience stressors, like discrimination, unique to their social status which affect physical and mental health. SGM older adults experience worse physical and mental health outcomes than their heterosexual peers suggesting unique needs for SGM older adults. Research also points to resiliency mechanisms that offset the impact of stressors on SGM older adults. The examination of these needs and resiliency mechanisms …

Community Psychology's Impact On Public Health And The Experience Of Marginalization, Katie Hudick Apr 2020

Community Psychology's Impact On Public Health And The Experience Of Marginalization, Katie Hudick

Richard T. Schellhase Essay Prize in Ethics

Vulnerable and marginalized populations face a series of risks and dangers throughout their daily lives. This is not simply limited to explicit forms of discrimination and hatred, but systemic forms of oppression and discrimination which limit those not belonging to more privileged and affluent socioeconomic or sociocultural groups. Community psychology operates as a means of analyzing how environmental and social factors impact specific demographic groups within a larger population and by extension the experience of mental health specific to these groups (Townley, Brown, & Sylvestre, 2018). In the application of this field, it is critical to understand the dynamics of …

Greater Social Cohesion Is Associated With Lower Body Mass Index Among African American Adults, Adolfo G. Cuevas, Ichiro Kawachi, Kasim Ortiz, Mariam Pena, Lorraine R. Reitzel, Lorna H. Mcneill

Publications and Research

Obesity remains a public health issue, especially for Blacks (or African Americans). Obesity is thought to reflect a complex interaction of socioenvironmental, biological, and cognitive factors. Yet, insufficient attention has been given to psychosocial factors like social cohesion within the African American community. Using multivariable linear regression, we examined the association between social cohesion, measured by the Social Cohesion and Trust scale, and body mass index (BMI) with cross-sectional data (n = 1467) from a cohort study (2008–2009). Greater social cohesion was associated with lower BMI (b = -0.88; 95% CI: −1.45, −0.32) in an unadjusted model. The association was …

Patient, Hospital And State-Level Policy Characteristics Associated With Prevalence Of Treating Opioid Use Disorder In United States Emergency Departments, Rafeek Adeyemi Yusuf

Dissertations & Theses (Open Access)

The opioid crisis in the United States was declared a public health emergency due to escalating and untoward human, financial, and systemic consequences and effects on the nation. Opioid use disorder (OUD) comprising opioid abuse and dependence is devastating because of its associated chronic relapsing nature, overutilization of healthcare services, rising morbidity and mortality rates, and high cost of care. Efforts to address this have not made significant positive impacts. It is thus imperative to reassess the influence of factors associated with OUD. This study answered the question, what patient-, hospital-, and state-level policy factors were associated with prevalence of …

Identifying Risk Factors For Chronic Kidney Disease Of Unknown Origin In Six Central American Countries And Houston, Texas, Erika Figueroa-Solis Apr 2020

Identifying Risk Factors For Chronic Kidney Disease Of Unknown Origin In Six Central American Countries And Houston, Texas, Erika Figueroa-Solis

Dissertations & Theses (Open Access)

Chronic kidney disease (CKD) is a public health problem that is recognized worldwide. The type of CKD in Central America does not fit the “usual” CKD encountered in countries with upper middle-income economies, and is also often referred to as CKD of undetermined cause (CKDu). Evidence suggests that CKDu is a disease of occupational origin that can be brought on by a combination of exposures: hot environments, high physical job demands, inadequate hydration protocols, and effects of other nephrotoxic agents. Many gaps remain in knowledge related to CKDu and its putative risk factors, geographic distribution, distributions of renal function in …

Leveraging The Power Of Mutual Aid, Coalitions, Leadership, And Advocacy During Covid-19, Daniela Domínguez, Dellanira García, David A. Martínez, Belinda Hernandez-Arriaga

Psychology

The COVID-19 pandemic has shed light on the norms, patterns, and power structures in the United States that privilege certain groups of people over others. This manuscript describes COVID-19 as an unprecedented catalyst for social transformation that underscores the need for multi-level and cross-sectoral solutions to address systemic changes to improve health equity for all. The authors propose that the American Psychological Association and its membership can initiate systemic change, in part, by: (a) supporting mutual aid organizations that prioritize the needs of vulnerable communities; (b) leveraging the efforts and strides APA psychologists have already made within the association, in …

“Is Therapy For Me?” Perceptions Of Therapy Inclusivity And Willingness To Seek Help Among Black Emerging Adults, Randl B. Dent Jan 2020

“Is Therapy For Me?” Perceptions Of Therapy Inclusivity And Willingness To Seek Help Among Black Emerging Adults, Randl B. Dent

Theses and Dissertations

Mental health issues are prevalent among Black emerging adults; however, they tend to underutilize mental healthcare services. The goals of the current study were to examine whether: (1) perceived therapy inclusivity would predict willingness to utilize mental healthcare services and (2) the relationship between perceived therapy inclusivity and willingness would be moderated by two indices of racial identity (i.e., centrality and private regard). Results provide evidence that greater perceptions of therapy inclusivity are associated with greater willingness to seek mental health services even after controlling for factors, such as gender, self-stigma, and previous mental healthcare utilization. Additionally, there was no …

Background: The objectives of this study were to first understand whether a dialysis center’s ownership status modified the association between informing end-stage renal disease (ESRD) patients about transplantation options and patients’ transplantation status; and second, to understand whether Medicare’s Prospective Payment System (PPS) implementation affected the cost of care for ESRD patients from Medicare’s perspective.

Method: This study used the United States Renal Data System (USRDS) data containing information on all ESRD patients in the US, linked to Medicare claims. To address the first objective, a multi-level analysis using mixed effect multinomial logistic regression model was conducted with …

The Relationship Between Social Media Engagement And Psychological Well-Being In College Students At The University Of New Hampshire, Emily G. D'Antonio Jan 2020

The Relationship Between Social Media Engagement And Psychological Well-Being In College Students At The University Of New Hampshire, Emily G. D'Antonio

Honors Theses and Capstones

Social media use has increased substantially in recent years, and for the college-aged population, social media is often the leading method of communication. Research indicates this reliance on digital connection could have a negative impact on the health of young adults (Bagroy et al., 2017). The college years are a time of personal growth and defining actions, yet can also be burdened by mental health issues related to stress, anxiety, and depression (Hunt & Eisenberg, 2010). Acknowledging these trends, the current study explores how college students’ specific frequency and intentionality while interacting on social media relates to their psychological well-being. …
