Understanding Change In A Therapeutic Community For People With Severe Mental Illness: An Interpretive Phenomenological Study, Julie Caroline Keys

In order to gain better understanding of how therapeutic communities instigate change, this interpretative phenomenological study explores the recognized agents of change as reported by current and recent community members of a particular therapeutic community. Four superordinate themes were discovered: the structure of the program, engagement in the community, intentionality/mindfulness, and applied knowledge. While the results from this study are similar in several ways to the current literature, they also expand the literature about therapeutic communities in one important way. The change agents reported as leading to healing strongly resemble current best practice for the treatment of trauma. Limitations and …

Approximately 25% of children in the United States will witness or experience a traumatic event before age 4, and individuals with a history of trauma, particularly traumatic events in childhood, have a much higher likelihood of developing psychopathology in adulthood. Prior research indicates that the vast majority of individuals with a serious mental illness, particularly those in community mental health centers and psychiatric inpatient settings, have experienced at least one traumatic event in their lifetime. These individuals require special consideration in treatment planning, and a large range of neurodevelopmental and environmental factors must be taken into account when interpreting results …

Research has suggested that consumption of alcohol in the presence of elevated posttraumatic stress symptom (PTSS) may serve an avoidant function to cope with negative emotions. These coping-related motives for use are theorized to both maintain PTSS and relate to poorer prognoses in treatment for alcohol use disorders (AUDs). Treatments utilizing coping skills training, which typically also involves educating clients about the negative consequences of drinking alcohol to cope, suggest the utility of targeting coping behaviors to reduce alcohol use. These studies, however, have not attempted to isolate the effects of psychoeducation on alcohol-related factors. The current study investigated the …

Using narrative theory, the chapter examines how ideas and attitudes about mental disorder are shaped by cultural values and stereotypes, and how the experience of trauma can shatter the narrative of self and world. Placing the question of illness, differentness, and personhood within a social justice perspective, it challenges clinicians to consider how the vocabulary of illness is used to frame experience and, in many cases, to minimize, marginalize, or discount the individual's own lived experience.

The issues of rising terrorism, violence, and scarcity of basic needs will increase in the coming decades, and children that need psychological services in disaster areas around the world will also increase (Alim, 2008). The study utilized the House-Tree-Person (HTP) projective test to examine the adaptation and maladaptation of Haitian children who lived in extreme urban poverty, broken infrastructure, and relocation camps in the aftermath of the 2010 earthquake. Theory and research suggest that post traumatic stress disorder (PTSD) symptoms may disrupt intimate relationships of survivors of trauma. Nevertheless, little empirical research has examined the role of PTSD and how it affects intimate relationships of survivors of sexual assault. This study utilized selected measures to examine the degree to which symptoms and treatment(s) for PTSD contribute to relationship satisfaction in female survivors of sexual assault. Overall, the severity of PTSD predicted relationship maladjustment at baseline, which is consistent with past research findings. More specifically, avoidance and arousal were found to be the most robust predictors of relationship maladjustment. This …

Not all individuals who experience intimate partner violence (IPV) victimization experience clinically significant negative outcomes following IPV exposure. For those that do experience negative outcomes, researchers need to identify the mechanisms through which they develop and the manner in which negative symptoms may develop differentially across individuals. This paper provides a review of risk factors associated with negative outcomes following IPV victimization. Accumulated lifetime maltreatment experiences and maladaptive cognitions are both proffered as potential risk factors for Post-traumatic Stress Disorder (PTSD) outcomes following IPV exposure. This study examines manifestations of mental health concerns in response to Gender Based Violence (GBV) specifically rape in a Non-western post-conflict setting. The population is a sample of girls aged 13-18 years residing in an Internally Displaced Person’s camp in Northern Uganda. Through semi-structured interviews, the girls shared their experiences of Gender Based Violence. Using Interpretative Phenomenological Analysis, 30 transcripts were analyzed to explore what these girls’ experiences have been with regard to rape, mental health and cultural and contextual stressors. Findings show that the girls described experiencing symptoms similar to those outlined in the DSM. Additionally, the girls described …

Operation Enduring Freedom and Operation Iraqi Freedom have created a new generation of military veterans and military families, many of which must manage and cope with psychosocial challenges such as posttraumatic stress, depression, anxiety, and alcohol abuse induced by the psychological trauma(s) faced during war. Risk factors, buffering factors, and war zone stressors influencing the development of PTSD following military-related trauma will be reviewed. Research indicates that traumatic experiences can impact college students’ mental health, academic abilities, and relationships with peers. Trauma and associated symptoms of PTSD can lower students’ well-being and increase the risk of withdrawing from the university. Research also emphasizes the importance of psychological help-seeking as a way to experience posttraumatic growth. This study examines traumatic experiences, help-seeking attitudes, barriers, and behaviors, and posttraumatic growth in a sample of 168 undergraduate college students. Results indicated an overwhelming preference for informal help-seeking resources and the importance of traumatic severity in the decision to seek help. Trauma experiences are, unfortunately, a common part of childhood in the United States and are connected to serious health-related concerns throughout childhood and adulthood. A primary symptom of trauma exposure and posttraumatic stress is re-experiencing, which often occurs in the form of nightmares. Though cognitive behavioral treatment (CBT) is currently the most well supported treatment model for trauma-exposure, it does not specifically address nightmares. Left untreated, trauma-related nightmares may become chronic, impairing quality and quantity of sleep, and exacerbating and perpetuating trauma symptoms. Current literature on the relationship between posttraumatic symptoms and dissociation that occurs during the time of a trauma, or peritraumatic dissociation, appears to be contradictory and inconclusive. Conflicting findings in the empirical literature that disagree on the nature of this association (whether peritraumatic dissociation is a risk factor for PTSD or a neutral or even protective evolutionarily-derived phenomenon) may originate from the lack of conceptual clarity regarding the construct of dissociation, and lack of differentiation between peritraumatic dissociation and dissociation that persists after a traumatic event. One hundred percent of evidence-supported psychotherapy treatments for trauma related disorders involve the therapist learning from and retaining fidelity to a treatment manual. Through a hermeneutic qualitative textual interpretation of three widely utilized evidence-supported trauma treatment manuals, I identified themes that suggested a particular constitution of the contemporary way of being—a traumatized self—and how this traumatized self comes to light through psychotherapeutic practice as described by the manuals. The manuals included: 1) a trauma focused cognitive-behavioral therapy for children; 2) an eye-movement desensitization and reprocessing therapy for adults; and, 3) an early intervention and debriefing therapy series for post-traumatic stress …

Child abuse and neglect can have serious negative physiological and psychological effects on the developing brain. Children who suffer from early and ongoing abuse and neglect often develop further problems as they mature, even if they are subsequently in safe environments. Many trauma-based therapies have been created in order to help these children develop increased emotional and social regulation, and decrease their behavioral problems. No study has taken an updated and comprehensive review of Donnel Stern’s writings. An investigation of his philosophical assumptions, locating Stern’s work socioculturally and historically, along with an elucidation of Stern’s background in traditional psychoanalytic literature and clinical practice, brings out the meanings and enigmas present in his theories of dissociation, enactment, unformulated experience, multiple self-states, and reflection. Stern has offered one of the best-integrated theoretical models in relational psychoanalytic theory. This study examined the role of traumatic distress as a mediator in the relation between trauma exposure and somatic malaise and healthcare utilization. Resilience was examined as a moderator between trauma exposure and somatic malaise and healthcare utilization, then between trauma exposure and traumatic distress. A total of 206 female participants recruited from an obstetrics and gynecology specialty practice completed measures of trauma exposure, traumatic distress, resilience, somatic malaise, and healthcare utilization. Multiple hierarchical regressions were performed to test the hypothesized relations. Results indicated that trauma exposure was a significant predictor of traumatic distress and resilience, and of somatic malaise …

Writing about personal traumatic experiences is associated with beneficial effects on physical and psychological symptoms compared with writing about emotionally neutral events. However, not everyone benefits from trauma writing to the same extent. The present study hypothesized that the effectiveness of trauma writing may be moderated by emotion regulation, as indexed by respiratory sinus arrhythmia (RSA). Research also shows that greater physiological reactivity is predictive of better trauma writing outcomes. Given the importance of physiological output in emotional processing, response training was developed and found to increase appropriate physiological reactivity. Posttraumatic stress disorder in maltreated youth is a young and burgeoning field. Lemos-Miller and Kearney (2006) were among the first to propose and test a theoretically and empirically-based model of PTSD in maltreated youth. The present study replicated and evaluated the Lemos-Miller and Kearney model within a larger and more diverse sample of maltreated adolescents. First, the Lemos-Miller and Kearney model was tested via structural equation modeling (SEM). Second, the model was evaluated across age, gender, and ethnicity. Third, trauma history, family environment (i.e., cohesion and conflict), dissociation, and trauma-related cognitions were evaluated as potential mediating variables within the model. …

This survey-based, comparative study investigated the percentage of students at a suburban Chicago therapeutic day school who meet criteria for clinically significant levels of PTSD as compared to students in a general education setting. The directional hypothesis was that students placed at therapeutic day schools have a higher prevalence of PTSD than a general population of students. The method used was a survey assessment called the Child PTSD Symptom Scale (CPSS) given to 16 students at a suburban Chicago therapeutic day school. Juvenile sexual abusers commit a significant proportion of the sexual offenses perpetrated in the United States each year. Researchers have identified several variables related to the etiology of sexual offending behavior in adolescent males. For example, there is evidence that both trauma and deviant sexual arousal have been independently linked to sexual aggression committed by adolescent sexual abusers. The population consisted of 9,000 Gulf Coast Licensed Professional Counselors. Surveys were returned by 609 participants. In the researcher-developed demographic survey, 586 individuals responded to the questions regarding age, gender, ethnicity, and years of counseling experience; 585 individuals responded to questions about exposure to prior trauma, and personal Katrina-related losses; 578 individuals responded to the question about the percentage of their work week that was spent counseling victims, and 579 individuals responded to questions regarding the type of mental health care strategy they participated in. One of the most difficult tasks for a therapist is to determine whether a client’s difficulty is psychologically based or spiritually based. Scripture shows us that virtually any physical, psychological, or social symptoms might be attributed to demonic spirits. The enemy attacks on all possible fronts, which complicates the whole process of diagnosis. The most accurate diagnoses come not from looking only at symptoms, but at predictive life experiences. Writing about a personal traumatic event has been found to have psychological and physical health benefits. Focusing on traumatic memories in writing may be a form of exposure. In imagery exposure and trauma writing, greater physiological reactivity was predictive of better outcomes. Given the importance of physiological output in emotional processing, response training was developed and found to be effective in increasing appropriate physiological reactivity in imagery exposure. If response training amplifies physiological reactivity and the benefits of writing, the hypothesis that writing is a form of exposure would be strengthened, and training may be a valuable tool to improve …

A number of studies have suggested that the occurrence of past trauma can increase an individual's chance of developing PTSD from a new traumatic experience later in life. Trauma that occurs during childhood appears to have a particularly strong effect on this risk increase. Furthermore, conditioned fear responses have been shown to incubate over extended periods of time in animal models. To further investigate the role these phenomena play in the development of PTSD, this study exposed juvenile and adult mice to 7 days of chronic variable stress (CVS).
