This study examined the impact of parental substance misuse on young adult development and relationships by interviewing young adults about their experience being raised by a parent who misused substances. A qualitative design based on constructivist grounded theory and informed by constructs from attachment theory was used. Participants consisted of 10 young adults, ages 18-26, who endorsed being raised by a parent who misused substances. The interview questions developed for the study were informed by a measure of adult attachment. A posttraumatic stress disorder (PTSD) diagnosis requires that the youth has directly experienced a traumatic event, witnessed a traumatic event occur to others, or learned that a traumatic event has occurred to someone close. The clinician is in a diagnostic quandary, however, when the youth presents with the PTSD hallmarks of hyperarousal, avoidance, and emotional dysregulation, in absence of such life experiences. A parent’s style, that is their approach to interacting and guiding the child may provide children with warmth and security, or elicit fear, distrust and disappointment, both of which in turn impact behavior. However, the parent-child relationship is not unidirectional; it is dynamic. Objective: To evaluate the efficacy of attachment-based family therapy (ABFT) compared with a family-enhanced nondirective supportive therapy (FE-NST) for decreasing adolescents’ suicide ideation and depressive symptoms. Method: A randomized controlled trial of 129 adolescents who are suicidal ages 12- to 18-years-old (49% were African American) were randomized to ABFT (n ¼ 66) or FE-NST (n ¼ 63) for 16 weeks of treatment. Assessments occurred at baseline and 4, 8, 12, and 16 weeks. Trajectory of change and clinical recovery were calculated for suicidal ideation and depressive symptoms.
