Transitioning youth from Children’s Mental Health (CMH) services to adult care poses a major challenge for our mental health and health care systems. This dissertation used a mixed-methods approach to explore the challenges of caring for youth with ongoing mental health problems into young adulthood. A Grounded Theory qualitative study consisted of interviews with youth (i.e., ages 12 to 15) treated in CMH, their parents, CMH providers, and family physicians. Children and youth frequently receive services for mental health issues from multiple service sectors but little is known about the rates of multi-sector involvement over time. Thus, the prevalence of multi-sector service use for children in contact with Ontario mental health agencies, and the influence of demographic, familial, and need variables on child multi-sector involvement, were examined. Secondary data analyses were performed on chart reviews of clients (N=355; 67% boys; ages 4 to 13) from six mental health agencies. Approximately two-thirds of clients had multi-sector involvement. Continuity of care, which is how a patient experiences care over time as coherent and linked, has been identified as an indicator of health system performance and is considered an ethical principle of care. Yet, no instrument exists to measure continuity of care as experienced by families receiving services through the children's mental health (CMH) system. A new measure, Continuity of Care in Children’s Mental Health (C3MH), is presented. The project involved four phases: item generation, pre-testing, pilot testing, and validation.
