Creating A Common Trajectory: Shared Decision Making And Distributed Cognition In Medical Consultations, Katherine D. Lippa, Valerie L. Shalin

The growing literature on shared decision making and patient centered care emphasizes the patient’s role in clinical care, but research on clinical reasoning almost exclusively addresses physician cognition. In this article, we suggest clinical cognition is distributed between physicians and patients and assess how distributed clinical cognition functions during interactions between medical professionals and patients with Multiple Sclerosis (MS). A combination of cognitive task analysis and discourse analysis reveals the distribution of clinical reasoning between 24 patients and 3 medical professionals engaged in MS management. Findings suggest that cognition was distributed between patients and physicians in all major tasks except …

Despite its longevity there is still not widespread agreement regarding how it should be defined or how it should guide the delivery of services. Shared decision making places an emphasis on patient understanding and engagement. However, when it comes to treatment selection, research tends to focus on how doctors select pharmaceutical treatments. The current study is a qualitative assessment of how patients choose among three common treatments that have varying degrees of scientific support and side effects. We used qualitative data from 157 undergraduates (44 males, 113 females; mean age = 21.89 years) that was collected as part of a larger correlational study of depression and critical thinking skills. Barriers to accessing primary care, including lack of transportation and inadequate appointment times, are common reasons for non-urgent emergency department (ED) use yet even when these barriers are addressed, the problem persists. This study explored non-urgent ED use by Medicaid enrollees through interviews with patients and providers and sought to identify themes beyond the commonly mentioned logistical and access issues. Qualitative interviews with 23 Medicaid enrollees and 31 PCP and ED providers utilizing a semi-structured interview guide focused on reasons for seeking care in the ED and issues associated with PCP appointments. ABSTRACT

The present study analysed 7490 citations appended in the 60 Ph.D. theses of Public Administration submitted to Panjab University, Chandigarh, for the award of doctoral degree during 2002-2012. The study analysed the several parameters such as authorship pattern of the citations, form of literature cited, half-life of books and journals and compiled a rank list of journals in Public Administration. The study found that the highest number of citations to research work by single authors was higher in case of both books (2782, 87.02%) and journals (1785, 87.12%). THIS ARTICLE PRESENTS selected findings from the ‘Media for Diversity and Migrant Integration’ project (hereafter MEDIVA), a European Union funded project involving six Member States (Ireland, Greece, Italy, the Netherlands, Poland, and the UK), which aimed to assess the capacity of media to reflect the increasing ethnocultural diversity of European societies. The specific focus of the project was on Third Country Nationals (TNCs) or persons without European Union citizenship. In this article we present the project’s content findings for Ireland, focusing specifically on representations of TNCs in a range of national print and broadcast outlets.
