On 2nd Jul 2007, two sub-munitions (1x M77 & 1xM42) were reported as found by the [International commercial company] completions team during a final inspection of CBU 771, before handover to the MACC SL. These items were located by the local land owner (woman) while harvesting her wheat crop. They were located in an area immediately adjacent to her house (within the task site) which had been reported as cleared by [International commercial company] sub-surface. A mine blast incident occurred at approximately 08:35hrs on Sunday 01/07/2007 in the minefield 69270030. The deminer detonated a M35 Belgium anti personnel blast mine due in direct contact to the top of the mine. At the time of the incident the deminer was wearing the PPE (Body vest and Goggles). On 04 June 2007 at 1103 hrs while the deminer was working in his clearance lane, his trowel struck the top of a PMN anti personnel mine, causing the mine to detonate. Activity being performed when accident occurred: [the Victim] was performing excavation with a bayonet probe (prodder) from left to right (left handed) and hit a PMN mine. It is not clear why was he using his bayonet prodder for excavation, or if he probed the soil before excavating. No MDD or MDU were in support of DT #4 on the day the accident occurred.
