Purpose: The purpose of this project is to increase the number of open patient appointments by implementing a validated process in which stabilized behavioral health patients are repatriated to primary care. Currently, there is an increased demand for behavioral health services and decreased supply of behavioral health clinicians at the Nashville Veterans Affairs Medical Center (VAMC).

Aims: This project aims to teach behavioral health providers how to identify appropriate patients and complete successful discharges while helping primary care providers integrate the repatriation process into their workflow. In recent years, there has been an increase in transitioning programs for military personnel. Since the September 11, attacks on America, millions of U.S. troops have been deployed to the Middle East. The wars in Afghanistan and Iraq have jointly proved to be the longest running conflicts/campaigns in the history of the U.S. Over the past 15 years, approximately, 2.3 million sevicemembers have served in the U.S. military during this conflict.
