These proceedings are from a seminar organized by the Department of Obstetrics, Gynaecology, and Perinatology, Obafemi Awolowo University, Ile-Ife (Nigeria) in collaboration with the Population Council. The primary purpose of the multidisciplinary seminar was to identify the determinants of the high rate of mortality and morbidity from unsafe abortion in Nigeria. The specific objectives were: 1) to identify measures that could be undertaken on a short- and long-term basis to reduce the rate of abortion-related mortality, and 2) to set an agenda for research into abortion in Nigeria. This article explores noncriminals' admiration for the lawbreaker. Drawing on literature, films, history, and psychoanalysis, the article seeks to delineate and explain this paradox. Each part of the article adopts a different approach to the subject of admiration for criminals. Part II, "Reluctant Admiration," sets the stage by presenting evidence that such admiration, and conflict over it, are pervasive. Parts III and IV present two quite different strategies that noncriminals employ to cope with their inner conflict over criminality. Thus, Part III, "Rationalized Admiration," depicts noncriminals who express undisguised enjoyment in, and reverence for, criminals. These noncriminals justify their attraction …

This presentation to the 1991 Berzelius Symposium, Stockholm, Sweden, explores the issue of maternal risk—the probability or chance of dying or being seriously injured in pregnancy—as it is used in maternal health care. This concept of risk has been a useful tool for research and medical and epidemiological education, but its use as a tool for service delivery design has been more problematic. In order to construct a risk system, one has to have reliable data on the relationships between individual characteristics and the outcome being studied—something that is difficult to develop with regard to maternal health. Reported animal rabies in Arkansas is reviewed for the years 1982-1990; providing an update from 1950-1981 (Heidt, 1982). Total cases ranged from 39 in 1990 to 168 in 1986, with a mean of 123.1. Wildlife accounted for 93.4% of the total cases. A total of 16 kinds of mammals has been implicated in reported rabies (individual species of foxes and skunks have been combined). The four taxa accounting for the highest incidence are: skunks (82.6%); bats (10.1%, all seven species combined); cattle (2.8%); and dogs (1.5%). Skunks, the most prominent species, ranged from 71.8% in 1990 to 90.2% in 1987. …

In a presentation at the 1991 Berzelius Symposium in Sweden, a Population Council researcher described the limitations of healthcare systems, specifically during pregnancy, in terms of their effect on maternal health status. These limitations stem from an inability to improve health because of social conditions—poverty and illiteracy, overwork, inequality in sexual relationships—that cannot be solved by medical interventions. Maternal ill health originates before pregnancy and endures beyond it, whereas the window of contact with women during pregnancy is small. Nevertheless, recent evaluations suggest that the impact of prenatal care is in the caring process more than any specific aspect of …

This article returns to a war waged virtually throughout this century--a war between the theories of punishment and rehabilitation in curtailing the drug epidemic. Today, the terms of the war are recast as supply-side policies based upon law enforcement; destroying crops in source countries; interdiction and increased sentencing; and demand reduction based upon prevention, education, and treatment. The war on drugs has reached a feverish pitch. This paper analyzes the law, ethics and public policy concerning "genetic discrimination," defined as the denial of rights, privileges or opportunities on the basis of information obtained from genetically based diagnostic and prognostic tests. The Human Genome Initiative will enhance the ability to gather and organize information that may predict a person's future potential and disabilities. Enormous human benefits may ensue from understanding the etiology and pathophysiology of genetic disorders, including disease prevention through genetic counseling, and treatment of the disorders through genetic manipulation. Drug dependence and human immunodeficiency virus (HIV) are America's two most pressing epidemics, interconnected by a cycle of urban poverty, physical dependence and a culture of sharing needles and syringes. Extant political strategies to curb these interconnected epidemics involve two traditional approaches. The first--law enforcement and interdiction--is designed to limit the supply of illicit drugs to the marketplace. This strategy is advanced by broad criminal sanctions against importing, selling, distributing, medically prescribing, or possessing illicit drugs or drug paraphernalia. The second strategy to combat the drug and HIV epidemics involves reducing the demand for illicit drugs. Education, counseling, and treatment …

In the summer following graduation a sample of 125 female college graduates (mean age = 28) completed Cohen & Wills' ISEL (1985) which includes scales measuring four social support functions: belonging (social companionship), appraisal (availability of confidants), tangible (instrumental), and self-esteem support. In the summer and fall subject status on two outcome scales was ascertained: the Psychophysiologic Symptom Scale and the Center for Epidemiologic Studies Depression Scale (CES-D). Reliability of the difference scores suggested that the ISEL scales do not measure entirely different constructs and the ISEL Self-esteem Scale is operationally redundant with the Rosenberg Self-esteem scale and the CES-D. …

Four coprolites were excavated with Burial 5 at Ventana Cave. a partially mummified five-year-old child. Two coprolites were granular and dark in color and two were fibrous and light in color. The coprolites are remains of the child's intestinal contents and were submitted for dietary and parasitological analysis. No parasites were found. The fibrous coprolites proved to be remains of highly masticated mesquite pods (Prosopis). The granular coprolites consist of seeds of saguaro cactus (Carnegiea gigantea). Pollen analysis reveals two dietary pollen types, both derived from cactus. No evidence of cultivated plants except for a trace amount of …

This paper provides a halting first step in organizing a set of ethical guidelines for the conduct of population-based research, surveillance and practice. These principles are not distinct from, but an expansion of, traditional ethics. Research ethics, which matured significantly from the Nuremberg Code through to the Helsinki IV and the Council for International Organizations of Medical Sciences (CIOMS) guidelines, nourished the individual human spirit. Ethical principles should have a similarly profound impact in the development of science and the protection of human populations in the 1990s and beyond.

Seventy-two male college students classified as Type A or Type B on the basis of scores on the Jenkins Activity Survey - Form T (JAS-T) were given the Cook and Medley Hostility Scale and divided into four groups on the basis of test scores: A/High Hostile; A/Low Hostile; B/High Hostile; and, B/Low Hostile. Subjects were then randomly assigned to either Group I: Controllable Event or Group II: Uncontrollable Event. Pre- and post-experimental one-minute time estimates and pre- and post-experimental one-minute key tapping sessions were recorded for all subjects. Language delay and phonological delay have been shown to coexist. Because they so often co-occur, it is possible that they may interact, sharing a relationship during the child's development. A group of children who were "late talkers" as toddlers, achieved normal development in their syntactic ability by the preschool period. Because their language abilities are known to have increased rapidly, data on their phonological development could provide information on the relationship between phonological and syntactic development.

Post-surgical cold applications are beneficial to orthopedic surgery patients. However, their use is limited because of the difficulty in maintaining a constant temperature, increased need for attention, and unwanted leaks or condensation from ice bags. Twenty-one arthroscopic surgery patients were divided into three groups: a Dura*Kold cold pack group, an Aircast Cryo/cuff compression dressing group, and a control group. A Redefinition Of The Problem Of Homelessness Among Persons With A Chronic Mental Illness, Donald M. Linhorst

Two definitions of the problem of homelessness among persons with a chronic mental illness are examined, along with their implied solutions and ramifications for policy. Homelessness among this group is first viewed as the result of deinstitutionalization, and secondly, as the outcome of a critical shortage of low-income housing. Solutions stemming from the deinstitutionalization definition of homelessness, reinstitutionalization or improvement in the mental health system, are seen as inadequate to deal with the problem of homelessness among the mentally ill. Instead, state departments of mental health are called upon to provide a leadership role in the development of affordable housing.

Effect Of Therapist-Initiated Versus Self-Initiated Vestibular Stimulation On Vocalization In Children With Autism, Carol L. Maddox

Therapist-initiated vestibular stimulation was compared to self-initiated stimulation to determine their relative effects on vocalization in children with autism. Vocalizations produced during table-top activities were also compared to those produced during vestibular stimulation. Two children with autism participated in the study. Vestibular stimulation was provided by a hammock swing. All vocalizations were recorded during a total of thirteen 15-minute test sessions.

Results indicated that in this study: (a) There was no significant difference in quantity of vocalizations produced during self-initiated versus therapist-initiated vestibular stimulation, and (b) there was a significant increase in vocalizations during vestibular stimulation as compared to during …

This study assessed the validity of the matching law in an applied setting. It manipulated extraneous reinforcement and measured the rate of rocking in a 24-year-old developmentally disabled man. The resulting data were compared to that predicted by the matching law.

The results were important in three ways: (1) extraneous reinforcement systematically affected rocking in an applied setting in the manner predicted by the matching law, (2) a clinically undesirable behavior decreased in frequency, and (3) this decrease occurred in the absence of direct intervention on the target behavior.
